The first time I heard this song, I fell inlove with it in an instant (thought 'twas a western artist' song). I've been listening to it for almost a year only to find out it was actually KZ's song.. Yes you're absolutely right, both have different meanings , if I'm not mistaken the other "scared to death" is a figure of speech that for instance like you are so scared that you might think of it as the end of it or death. And the other "scared to death" are like saying that you are scared to die/death. Well in this scenario this video concludes funeral,accident,crying,dead body and other. So in my opinion i consider this a scared to death/die

Scared to death means you're so terrified you feel like you'd die. The song speaks of fear of losing someone --- be it thru death, or that someone simply leaving. From the music video though, the girl died. But if you've noticed, the two are already losing each other (they were fighting) even before the accident. So, the song could mean both.
