I think the best way they could pull it off is with Doom as Haward stark bastard in the f4 universe.

He can have a very similar backstory as the original Doom, but coming to university with reed also to meet Haward, he prefers Reed and Victor going back to take over Latveria.

On in end of F4 there is incursion and he goes to 616 to destroy it and save his world. He tries to trick avengers by saying he's a Tony variant.

I for one am meh on the RDJ casting as a variant of Doom, because in the MCU this is our first intro to him I can see if we had him already fleshed out and then brought in variants but starting it here with a variant is different when the foundation for the character isn’t even built yet

It should be a drugged controlled or distraught stark...so that he can redeem him self and RETURN TO BEING IRONMAN... he won't be called Vic von doom
