@@sk8_bort Hello Carlos, sorry that this happened to you. I now feel fully recovered just over a year later and here was my diagnosis. My orthopaedic surgeon told me that I had between a grade 2 and grade 3, meaning no surgery could be performed and instead a lot of rehab and time will heal it. It probably wouldn't ever be the same and until now, there will be days where I still have some clicking and stiffness, but as an athlete, I've consistently been rehabbing it ever since and am back playing sports, lifting weights and sometimes I forget that I even had the injury. I was in a sling for the first 6 weeks, really babying it, but then progressively focused on getting ROM, strength and mobility back. The importance is mindset and commitment to you rehab and you will heal faster.

Loved the 3D and the explanation. Had a huge crash on riding MTB and ended up with a broken left wrist and a level 5 separation in the right clavicle. Had surgery on both areas the same day a week after the crash. Had a surgen tell me that the clavicle was just estetic and surgery was not needed, but I wanted surgery. Even after healing without surgery its something that is just going to hurt or be uncomfortable when lifting weights or doing activities. I would have prefered a broken clavicle bone insted on ligaments but that is what I got. I'm in my 2weeks of recovery and pain is gone but movement is very stiff. Doctor told me not to move or use shoulder with force for 3 to 4 months, so that is do, I dont want to damaged the area.

Thank You for posting such a beautifully detailed explanation. I just had a ski accident last week, diagnosed in both the clinic and by 2 separate doctors as a grade III AC joint separation. I gotta say I'm really nervous about the surgery, which is odd given that I've had 7 other ortho surgeries in the past. This one just feels more uncertain than the others, due to the wide variety of techniques and historical failure rates (just based on my own research). The Dr. said it was completely up to me whether I should do the surgery or not, as he said he really was on the fence himself about just seeing how it goes, but did comment that I would at least benefit from the bone not sticking up. Now I'm left to decide, and have no idea what I'm going to do. Hope this patient had a good recovery though.

Great video. Thanks for sharing this. I have a grade 3 AC separation with no surgery. Large bump. In sling for 2 weeks and then PT. Slow at first building up strength with bands and the weights. I am at 5 months now and can do everything I could before injury. I hurt it falling off dirt bike on shoulder and I ride fine now. I can sleep on it etc. My pain when I have it is like 2 or 3. I am lifting weights now. Bench press and push-up 30+ without pain. I will say listen to your body. When it hurts stop and let it heal for 24-48 hours then try again. All injuries SUCK. But this one can be manageable without surgery. I like to describe it as UGLY and AWKWARD. It is gruesome to look at in certain positions. The end of collarbone just sticking out. It really looks like it should hurt more but it doesn’t. The joint is also different then normal one. A bit more clunky and pops etc. hopefully long term it continues to stays strong and stable and painless. Best of luck to everyone with this injury and stay positive and do the PT 3 times a day until it hurts then stop. It should get better with strength and time.

Hello, this is by far the best video I have seen related to AC Joint separation. I've suffered from AC Split Type 3 about 6 months ago. I'm an experienced athlete and have began to build dense and rigid muscular mass to combat this issue. Is this sustainable? Also, Is it possible to build ligaments back into place from scar tissues by aligning shoulders via muscular correction over surgical correction? What are some long term disadvantages of surgical AC Joint Separation reconstruction?
@HiFLEXHealthPerformance
@HiFLEXHealthPerformance 2 жыл бұрын
Thanks so much! Education is key and I feel there is poor education out there for patients to consume and visually understand what’s going on. Without seeing you in person, take this very loosely. Since you have a base as an athlete and are likely very in tune with your body, you may be able to sustain it but I would be incredibly diligent and work with a skilled therapist to make sure everything is going ad smoothly as possible: You will develop scar tissue, but im not sure I would go so far as to say it would attach and almost mimic the ligament. Be sure to include nerve glides in your training, as your nervous system KNOWS there is a need for stability so it may add tension or the nerves can become irritable. I REALLY need to organize and clean up this channel now that people have found it! If you need help locating the nerve glides let me know

I got a grade 4 injury, 3 weeks ago in a car accident. I heard from 5 doctors that I need surgery, but the doctor ("one of the best" who barely speaks with me) that was assigned to me only fixated my shoulder and arm with an elastic bandage and said to wear it for two weeks , they also applied pressure on my sticking bone with a roll of a bandage. I had severe pains and after 11 days of having those bandgaes ( 16 days in total from the accident cause they didn't do anything on time) a decubitus formed from the pressure. When i went to a check up to my doctor he said I don't need an operation we just need to heal the wound. I was wondering what will happen, I have both ac and cc ligaments torn, and I've read that it can lead do chronic pain, overall shoulder instability and that injuries will be able to reoccurr more easily and in few years osteoporosis and arthritis can occur. I am a 27 year old woman, so I'm not someone with a lot of muscles. Will the recovery and maintenance without operation mean that i will have to keep exercising all the time to not have issues? And also the state medical system in my country is very bad cause apparently the better doctors need to be offered money to even take you seriously and talk with you about the issue and offer constructive advice, so should I go and try to get the operation at a private clinic ? or trust the doctor who barely tells me anything, cause of course I always consider the option of me overacting and professional doctors being sick and tired of people and not having time to talk with every patient but say something in less than an minute and send them off.

Love the 3d model! What scenario do you recommend surgery? When it’s grade 5 and all the ligaments are torn? I’m starting to use mine again at week 2 but I’m contemplating surgery. Do you think not having surgery would make it easier to hurt other things in the shoulder due to the instability of how much it has moved in a grade 5? Or with enough PT and strength it would be strong again for dirt bike riding and future crashes

@@mikel87 Hey Mike, thanks for asking! It has been a battle. I had my operation on Feb. 1, 2022. The AC separation surgery was on my right shoulder. I have not been the most consistent with PT. However, i started weight lifting pretty early as soon as my doctor gave me the clear to lift and carry up to 20lbs. This operation took a lot volume out of my Trapezius muscle and upper pecs. I still suffer from shoulder instability due to the lack of strength in my right shoulder. It has become more difficult to grow my shoulder and upper pecs strength and volume compared to the other side of my body. It's extremely tough to warm up my shoulders for a proper exercise. I feel slight tingle and pinch in the cold. Mobility and the range of motion is hindered. There are times that I completely regret getting the operation. But, I've been able to sleep on my shoulder. But, I've been able to look at myself in the mirror without a bone poking out of my shoulder. But, I've been able to start exercising. It's tough to say. I believe that it's still too soon for me to make any comments about regretting the surgery. Ultimately, do a thorough research before surgery. What is OK for me may not be OK for you. I was originally diagnosed with a grade 2/3 however when I have a meeting with a surgeon, he said my injury was closer to a grade 5. I had the injury over 14 months ago and made a decent recovery. I've been on the surgery waiting list for around 10 months now in order to stabilise my joint as often times by collarbone can shift and move, creating a more visible lump. Part of me is reluctant to get surgery as I cope with normal life but the other part of me can't wait to get this surgery done in order to return to normal activities and build up strength and stamina again to how it was before.

I am so glad it is helpful! I would definitely advise seeing a skilled physical therapist as there are tests they can do to assess for tear. It is very difficult to asses specific ligament, but if you have a very prominent elevation it is likely there is some significant ligamentous disruption.

Unfortunately for me, the pain and popping persist, so I'm guessing surgery will be in my future. @chelseasmith1556 4 жыл бұрын
Hey! Glad it was helpful. I try to share my videos I make for my patients incase it can help anyone else. So that is not a black and white answer unfortunately. It depends on your activity, how your rehab goes, etc. A grade 3 is pretty significant and theoretically tore all ligamentous connection there. There is obviously much additional musculature that can hold things in place, but my best assumption is that you'll have a solid "bump" that forms right at the AC joint of scar tissue and the collarbone might stay more elevated than the other side. Be very diligent with rehab because the stronger the surrounding musculature is endurance wise, the more support that joint has in its more aligned position to heal.

I have a 20 year old separated ac joint thats been an issue since day 1, even after 2 surgeries including a modified weaver and dunn that i never let heal . I was young and dumb when it happened. i didnt understand the injury and i never let it heal before going back to normal activities.. . its very unstable and i live in chronic pain that effects my quality of life.. im in great shape physically over all, i work out almost daily and target my ac join with physical therapy exercises .but it effects pretty much all my exercises .. when i do pull ups it feels like im not hanging evenly because theres so much instability in that shoulder that its reach is longer when hanging, its very frustrating. .. my shoulder blade position is different than my non injured side, its lower and sits flat almost .. i have achy back pain on that side daily and the appearance of a difference in leg length/hip height that i feel is all stemming from this shoulder injury..its like my entire injured side is effected ...do you have any advice for me ? i dont even know where to begin . thanks bud
@HiFLEXHealthPerformance
@HiFLEXHealthPerformance 3 жыл бұрын
First off, let me just say that I am sorry you have to deal with that! I have a fair amount of chronic pain from hypermobility and can empathize up to a certain extent. Instability really makes a huge effect on the rest of the body, as it is all connected. What you're describing absolutely could all be stemming from that. Where are you located? If you can, it would ABSOLUTELY be worth it to see a very skilled doctor of physical therapy. I am pretty connected all over thanks to mentorships and online connections. I'm happy to see if I know anyone near you! This is when the internet redeems itself...When people start self advocating and finding exactly who they need to work with! The generalized system can fail a case like yours SO HORRIBLY. It's unfortunate. If I can recommend one thing, it's that you treat your body like a car. If you'd replace tires and repair your engine....heck....be willing to invest in your body the same way. For example, MRI + surgery is going to cost people $2-5k, as you've already experienced. Comparatively, a plan of care with a practice like HiFLEX consisting of evaluation + 8 visits is $1520 in our case...very similar for many of my colleagues...and typically has great outcomes. If your AC joint isn't completely detached and clavicle floating... you should be able to have success with pain control, stability, and general function with a good PT. Best of luck!! Let me know if I can help further. Shoot me an email at chelsea@hiflexhealth.com and I can email you some suggestions.

I just separated my shoulder, I’m really worried if it will never be the same. Do you have advice for finding a physiotherapist for this? I mean what can they even do really? Just strengthen the surrounding muscles in your chest and delt and deal with it like that forever? It just sucks it seems like. Even if it wasn’t as bad what would be the risk going forward for the injured shoulder in particular like is it more at risk for impact or in certain positions?
@HiFLEXHealthPerformance
@HiFLEXHealthPerformance 9 ай бұрын
So sorry to see this! There are a lot of manual interventions to make sure secondary muscle guarding of the neck and shoulder blade do not kick up and cause additional pain and discomfort . But yes, a massive focus would be on strengthening of the proper musculature to ensure as much support as possible to the stretched ligaments. if there is complete tear of ligaments, surgical intervention may be warranted. If there is only partial tearing of the ligaments, often times, significant strengthening can be very helpful and avoid surgical intervention of course. Without seeing you, there is no way for me to give any specific advice like that. Where are you located? Sometimes the best route is to ask on social media and see who people in your area have gotten great results from. This is not always, but typically an orthopedic is going to send to someone in-house or someone who markets to them a lot (they may get great results don’t get me wrong but usually physician offices are not as familiar with out of network providers who have more 1:1 time).

This is an incredible explanation! Best one I've seen. I injured my AC joint about 6 weeks ago. Type 1. After about 7 days I was able to do most daily tasks again, but nothing strenuous. Now at week 6 I have good strength, but still some positions and movements give me trouble (especially reaching my hand behind my back and stretching across to other side). I have resumed very mild exercise activity in the form of solo basketball dribbling and shooting drills with very minimal explosive activity. So obviously shooting a basketball does not hurt anymore...only when I do a crossover dribble behind my back or dribble hard and fast. Occasionally when I'm exercising, the should does bother me slightly. I'm still concerned. I don't want to eff my shoulder up permanently. I feel like I should be further along after 6 weeks. Two questions: (1) What other indicators are there besides level of pain for knowing when it is safe to resume contact sports activity and that the injury has fully healed? (2) Is it safe to resume activity up to where the pain is tolerable or should I stop everything altogether until there is no pain? I know you don't know the details of my situation exactly, but any general advice would be greatly appreciated!

@@GhettoMountainsJr SO frustrating. Unfortunately sometimes pain is very complex even outside of the direct injury due to nerve irritation or other muscular guarding. Bummer that PT did not knock pain level down lower. Have you tried any nerve glides?

@@HiFLEXHealthPerformance I know ! It's a tough case and with my line of work, I need my shoulder functioning properly, pain free. I actually got a cortisone shot a few weeks back and that has helped with the pain but it's not 100%. I usually do cupping on and around and that will tie me off for a few days. I haven't tried that technique no, do therapist typically offer that ? Thanks for the reply !

So I had a grade 3 separation about 8 years ago on my AC Joint. I got surgery… I believe they put a pin in pull the clavicle back down. HOW COMMON is it for the pin to slip it’s position. I was moving the other day , had my should reaching in the back seat of my car lifting a heavy bag and did something to my shoulder… Could not move my should up on my side or front for 3 days. Inflammation went down and now my clavicle is sticking out… did I re separate my AC joint?? thank you

The problem isn't weakness. The problem is when you flex your muscles to raise your arm, it pulls your shoulder blade and collar bone together and grinds and binds, and pinches things between it, and will pop as they are grinding past each other. When the AC is there (particularly at the end of the collar bone)- it acts like a rubber bump stop, and doesn't let the two bones grind together. This is equally as bad of a problem as it no longer being able to support weight. I've had a torn AC for over a decade, and it never really gets better. I've seen people have good results with stem cell injections, but otherwise it never grows back and is very easy to reinjure.

Do the muscles on the clavicle stay attached if the clavicle is shattered?

Who the fk wants that ugly bump on his shoulder for ever ?? Nasty Grade 3 gets operated on, as i did. Rehab is same almost and it gets properly treated. Cosmetically and medically. If you have the money do the operation.

A friendly suggestion: please try to improve your technical skills, as well as your English, for "this guy" and "that guy" is not a proper way of communicating and leaves a poor presentation of what you wanted to show.

Hey! this video was made specifically for a patient I already had performed a professional evaluation on and established rapport. This channel is simply where I upload them to share with my patients and made them public at request. If I begin making content specifically for this channel and a general audience I will change my communication up, but at this time all I do is house videos for specific patients here and therefore speak to them in the video in laymans terms they enjoy. Thank you for this feedback though!

Hello! These videos are made for specific patients and for their specific pronouns. This was made for a male. I simply upload these here due to length so I can easily send to my patients. I keep my page public so that others seeking information may find help. I will not be changing the way I speak. 👍🏼
