I had grade 3 AC joint separation and ortho adviced me to go for surgery ( i would personally advice if one can go for surgery for grade 3 he should , however it does feel a big deal at first but it helps in two ways 1. To regain the natural contour of the shoulders and 2. Surgically operated shoulder will give more strength in future than non-operative one.) I had surgery after 4 days of injury and he used the best technique in which less incisions were made i think it is called dog bone button technique. 1. Stitches removed after 2 weeks 2. Review at 3rd week, doctor told me to remove sling and start physiotherapy. 3. It has been 5th week and I have full range of motion with no pain at all. 4. Till 3 months I will give it time for recovery with no heavy weight lifting and after 3 months I'll get back to normal pushups,pull ups can also play sport but will wait till 6 months. But I would suggest one should play any contact sport after 6 months. ( I want to be extra careful in this to regain full strength and healing) In this surgical method two clips are inserted which are bio-compatibale and are permanent but if one wants to remove, he may get them removed after 2 years. In x-ray both shoulders seem alike but obviously two clips are visible in the operated one and yes initially there used to be a llate placed over clavicle that had to be removed and could be felt if touched but these two clips are are small and can be placed permanently. I too went through the same state of mind that probably you are going through, I watched almost all videos on yt and thought I should share my personal experience and opinion so that anyone might get comfort and hope. Wish you best of luck, it happens but what matters is how you go through it. Willpower is the only thing that is required as it is a time taking process. Stay composed and determined 🤝🙌

Wrecked my dirtbike yesterday, and believe I have a stage 1 AC separation, I’ve done this before on the same shoulder. I felt no pain initially when I hit the ground but when I tried to push my body up, I felt a slight pinch. I’m now less than 24 hours out, and I have limited range of motion due to swelling, “hopefully” but no Pain we’re not moving the shoulder. No lump above the shoulder beside some minor swelling. Praying that I can recover quickly! Been icing like a madman, and also taking BPC-157 subcutaneously. we’ll start taking TB - 500 soon, don’t have any on hand. Edit: thankfully, I didn’t hit the ground directly on my shoulder and was able to do a front tumble so the back of my shoulder blade first made contact with the dirt, but I believe this pushed my shoulder upwards, which stretched the AC joint. If I would’ve fell straight on my shoulder, probably would’ve caused a lot more damage. Thankfully I was able to do a tumble like you would see in gymnastics or karate which minimizes the impact when you hit the dirt. Hit the ground going maybe 10 mph on soft sandy dirt mix. If you’re a person of faith in Christ, please pray for me when you read this! I have tremendous belief in the healing power of prayer, especially when multiple people in the body of Christ Pray.

I would give my entire savings to have a grade 1 from my dirt bike wreck a month ago. I have a grade 3 and I wrecked my shoulder. My collar bone is taping my shoulder daily. I have been using KT tape is that is complete ass I finally got some good tape it’s called leukotape and it’s much stronger I taped the hell out of my shoulder and now I’m out of a sling. My biggest issues is the cringey feeling of my collar bone being loose and all over the place. Psychologically this injury is worse than anything.

I have had an MRI of my left ACJ and was told there was no damage but my clavicle will sometimes pop up over the other bone and come back in place. I was told it was due to swelling under the joint this was over a year ago and I was told to use OTC anti-swelling agents and ice. Which only helps as long as I continue to use them. What I found that has given much better results is doing dumbbell lateral raises to strengthen the joint.

I’m a petite, middle age (44yrs old) female, 5’1” & weigh 105lbs, but I have very defined, bigger than normal muscles & 6pack abs from weights & pull up type stomach exercises (power tower). I have pain on my right shoulder & unable to determine where exactly. My clavicle clearly sticks up or rather has a knot. I was worried I wouldn’t be able to lift so I started searching. I’m unsure when I injured it but thought maybe chin ups done it. I’m giving the lateral dumbbell raises a try!! Thank you! 🙌🏼🤩

Wow! Thank you I’ve tried shots and on oral medication now. I don’t feel pain after shot just crackles and pops with pushups etc. I was told no heavy lifting above shoulder another month or so and pretty much hope it doesn’t get aggravated once I return to my activities it’s a grade 3. I walked into the ER with my shoulder falling off. They gave me a $3 blue sling and showed me the door. With enough complaining I finally got referred to a "specialist"- who walked in for literally 6 seconds, push my shoulder around in 3 directions as I'm screaming in pain-- and directly walks out without even saying a word to me. and he sent my a $600 bill. Here I am a decade later and I'm still suffering from my shoulder with binding and grinding and constant pain and restricted movement, with my collar bone sticking up out of my shoulder. The "health care" system in the US is a f^cking joke. They DO NOT help people without money.

I just did the same Easter Sunday. At first i thought it was dislocated, had by brother pull on it attempting to put it back in place but no luck. I went to the ER and doctor quickly assessed it was a separation not dislocation. The ER Doc showing me X-ray said it was bad and likely needed surgery. I saw an Orthopedic Surgeon the next day for consult. He said it was a grade three separation, to deal with the bump and you’ll be ok with PT because I’m not a pro athlete. That didn’t sit well because i work in construction, and am an avid kitesurfer……I sent my X-ray to multiple doctors through friends who all said it should be fixed. So i got second opinion from the head of orthopedic at Columbia University in NYC. He said i had a grade 5 separation that requires surgery, the bone went through my deltoid and my scapula shifted toward my spine 3/4”. I Am having surgery May 11. Hopefully they can put Humpty Dumpty back together again, it looks horrible and it’s my dominant arm. I also have no insurance, missed the sign up date in January. I have to pre pay for surgery it will likely cost 25-30k, but better than 30 years of pain and poor function. There are some really bad doctors out there, get multiple opinions, research doctors and the reconstruction technique they use.

When I was a child, I went to sit down at one of those hard top wooden tables that fold up an da boy in my class, pulled the chair out from under me. My full weight went down on the area directly under my collar bone. It was broken, but because I didn't cry, it went overlooked until I had xrays for something else as an adult. NO one ever talks about sharp blows under the collar bone or blows that drive upward and or inward. This has been an ongoing issue for me all my life and I have never had effective help for it. I would really appreciate it being discussed and there are probably other people in this position as well. Thank you.Following. I've had some bad injuries due to bullying. No one like to talk about it.

Great video. Since you're so good at explaining, I wish there could be a bit more added on what takes place during healing when surgery is not performed. For example, how do those Trapezoid and Conoid ligaments re-attach themselves if the bone has 'stepped' upward creating a gap? How do they reach each other to re-connect? Same with the Acromioclavicular joint. How does it reconnect when it's no longer aligned? Does the set of ligaments reconnect first pulling bone back downward? Then the Acromioclavicular connects? in cases when surgery is performed, how are those ligaments reconnected, and how are they held in place? Also what amount of healing time is to be expected for the surgery and non surgery options? Thanks.

My same questions. I injured my AC joint in a fall last week. I have the typical bump on top of the shoulder from the AC dislocation. I want to know how to access if my injury is grade 3 or grade 5.

When you do not have surgery, the ligaments do not ever reattach. The deformity will usually stay. The treatment is usually PT to strengthen and stabilize the joint. I had a grade 5 separation and required surgery. It is a VERY painful and complex surgery and recovery takes a very long time. To repair this, the Dr. will use a cadaver tendon to replace the torn ligaments or synthetic “bolts and cords” to cinch everything back together. After I had surgery it required 4 months of no movement in a sling and then another 5-6 months of PT to get back to close to normal

Trust me, you must continue to go to gym. For example, famous professional fighter Nate Diaz have that injures at BOTH sides. Famous extreme sport man Levi Lavalle have it to. They don't care for doctors recommends, doctor don't have that experience 😊

Grade 3-4 here. My injury occured 7 years ago but now that i strength train regularly it is becoming a problem. All the grinding and flip flopping of bones is irritating as heck. Strongly considering the surgery now.

I have after long term shoulder pain nearly 2 years, developed a small painful nodule which i am very scared is cancer. It did not show up on an x ray and the radiographer noted normal shoulder with no evidence of healed fracture or break etc

GI fell 2 months ago, landing on my left shoulder, I went in the next day in pain. They took X-rays and didn't find a thing. I went back a week and a half later still in pain and can't lift much. They took X-rays again and this time they said it was a rotator injury. I went to my regular Dr. And was referred for an MRI. i went, got the MRI..now my shoulder still hurts even worse with a burning sensation at the joint moving down my upper arm and pain moving to my right shoulder. My results said sharply lateral downward sloping type II acromion, I'm still trying to figure out what it means. I can't lift, reach, reach behind my back with my arm without screaming In pain

I've been diagnosed with an ACJ injury in my right shoulder, and my doctor recommends keyhole surgery. I've had mild pain, about 3/10, for the past year, with discomfort and a clicking sound during certain movements. The pain occurs only at specific angles, and I can still exercise at the gym, though with caution. Should I proceed with the surgery, and is this really an ACJ injury? Thanks

I had a stage 4 with huge displacement, I had a tie down operation but they failed to remove enough of the clavical end so I got bad interference between the pieces. I had a second operation to trim the end remove the tie down and suture the clavicle rearwards slightly to prevent future interference. The blocking injection was horrible I had a dead arm for two days.

I felt out of my bicycle. I have grade V, after 3 months of constant pain and general weakness on my right arm (I am also right handed so is more annoying), I have been referred for surgery. Hopefully after this surgery, things will improve.

I had the tight rope surgery on a grade 3+ separation on my right . Not an easy procedure but necessary if you want full use of the arm/ shoulder. I started combat sports a year or so later. My right is stronger than my left now with same range of motion

I have never experienced any injury nor the AC joints have any discomfort, and I can actually resume the alignment of the AC joint by actively contracting my shoulder muscles (i guess supraspinatus?). This was so impressive and helpful, thanks.. I think I have grade 2 but the bone is sticking up above my left shoulder enough to be noticeable... would that make it grade 3? I'm in no pain...but if I press down on the bone it can feel a bit sore...but I have a full range of movement and can still life weights (but I dont lift heavy anyway).. my question is do you think I need to fully rest for a few weeks and will that make the bone thats sticking up go back down to look normal...(or will the bone never go back to normal) or will it take longer without exercise? Or should I continue to do exercise with bands and not weights? Sorry for the long message would really appreciate your reply...thanks! Grant

I suffered a grade IV a few years ago in a taekwondo injury (heavy fall onto shoulder). It looks awful (big step!), but has mostly been pain-free. I've started calisthenics recently and have discovered that pushups are almost impossible due to severe pain in the ACJ. So now I'm wondering if my injury was too long ago to be treated without surgery, or even with... 3 month wait to get back into shoulder specialist.. :/

Hey there, I have a long reply because I struggled too after my injury in a similar way, and I found that it is quite common. You may have to "relearn" a few things when it comes to getting back into training. Remember how completely f'd your shoulder mobility was after your injury and the weeks following recovery and therapy? After I got to some decent mobility back I attempted to restart my usual training, but like you there was definitely pain in upper body presses. Bench press, incline, and shoulder press. Surprisingly, decline was not painful, I learned later this is due to decline benchpress primarily recruits the lower, or sternocostal portion of the pectorals, and not the section that inserts onto the clavicle, or collar bone. I hate to tell you this, and this isn't medical advice of course every case is unique so definitely get a professional opinion, but from what I understand if surgery wasn't necessary after injury or during therapy, later intervention will likely not help... but you should definitely still go an rule out anything that might be related, i.e., impingements in the glenoid, rotator cuff, etc. I was lucky to have a great orthopedic surgeon walk me through the clinical outcomes of surgery and indulged me in how to educate myself to understand what is different about my body moving forward. Most docs don't do that. He pointed me to resources that would help me learn (or relearn now that our shoulders are no longer directly connected to our collar bones) how to do things like the importance of positioning the scapula and developing muscle memory when doing certain movements and lifts. I will regularly rotate days of training that solely focusing on shoulder stability muscle groups so there isn't any discomfort in compound upper body presses. I'd recommend you look into this, you can google around for some athletic trainer programs used in rugby and american football in rehabbing AC shoulder injuries. Training and conditioning programs for tennis players also focus heavily on strengthening obscure shoulder muscle groups. I focus a lot on compound movements like face pulls, as well as many isolating movements to specifically target rear delts, different sections of the taps, rotator cuff, and to some degree the lats (difficult muscle to isolate... it's huge). In other words, focusing on your back with isolating as well as compound movements will become essential to your training. The biggest breakthrough I had was becoming aware of how my collarbone remains relatively static while my scapula drifts and moves in certain movements and learning how to isolate that movement. Things like using your opposite hand to feel how your shoulder blade moves as your lat stretches when raising the other arm in front of you from the waist and then above the head. Your goal is to stretch and strengthen every muscle that inserts on the scapula and clavicle until supporting muscle groups "become" the replacement for the AC joint that is no longer connected. I know this is long as hell, but hopefully it's a peace of mind you aren't alone with what you're experiencing. All the best

@@clv603 thanks for the detailed reply. I’ve been doing some of what you suggest - working on the remaining support structures. I can once again do some push-ups without too much difficulty. Diamond push-ups seem to be strangely the most comfortable. Overhead presses with light weights are getting better too. I’m 54 so trying to take things gently, but my brain is still 21 so that’s a constant challenge..Scapula dips (ie straight arms) feel good and I think are definitely helping, as are scapula push-ups. Scapula pull ups are the next step and I’ve already tried a few without incident. I’m not sure a full planche is ever going to be in my future, but I can dream. I recently got some rings and just focusing on a basic support hold has been ok as well. Thanks for the additional guidance, and positivity!

@@clv603great post. This makes a lot of sense. Looks like something I would write to help the curious minded sufferer. I know some injuries our bodies will never quite be the same from and we have to learn to work with/ around the limitations. This was probably a latent injury triggered while skating bmx or mountain biking in my teens and never noticed until taking calisthenics, boxing, and handstand press movements more serious. When pain really flares up is after repeated or extended shadow boxing rounds, more specifically after throwing jab and left hooks. The least effective is a hook to the body because it extends down and out further than a tight arcing hook at the head. Also left uppercut. It just feels like the shoulder isn't completely connected or has a microsecond of catch up behind the rotation and rapid bodyweight shifting when throwing the punch at full speed/ power during hooking combinations. It's really slowing down the ability to flurry and because the left is slightly more painful as time accumulates in the rounds, my right which was never very powerful comparatively speaking is becoming more dominant by default. So frustrating because the left hook was the quickest most powerful punch in the arsenal and the jab the most useful. That also is my mouse hand at work and I carry a lot of tension in that shoulder. Fortunately a combination of ice and hot Epsom salt baths can kill any pain basically until whenever the next day of training is. So frustrating because I've made a ton of progress on speed and form and the only way to stop injuring it is to stop training. Right when the left jab, left upper, left hook combo was coming into shape. Argh! I'm going to use winter to train all the supporting muscles as was mentioned while adding a ton of stretching. Train boxing at only 30% going very slow (very hard to do) at two days on with one day off between to keep the mobility and muscle memory. And slowly ease into it as we get further into winter or when spring comes. For anyone reading this far I feared a rotator injury but have none of the pains associated with those tests. The pain goes from the upper pectoral through AC (which has a slight separation lump compared to opposite shoulder that used to get tender when doing tricep dips) through front deltoid, through bicep, and stops down at the inner elbow joint. I found this by moving the arm in every possible position to trigger the discomfort then pressed into the surrounding muscle fibers and followed the soreness. It's a different kind of muscle soreness from the surrounding tissue. I presume due to these particular connective tissues having to compensate for the lack of proper function of the AC joint. Even now typing this the surrounding deltoid has a fatigue soreness that feels out of the ordinary for the amount and type of exercise performed. Also doing rotator and impingement exercises do nothing to simulate the pain. Everything seems to originate at the AC and go out in either direction.. If anyone cares to check back and compare notes I would be happy to share what is and isn't working..

I had a MRI both for my neck and shoulder the result was "supraspinatus tendinitis with acromioclavicular synovitis" it's been two years now. tried everything but when ever I lift (even a light weight) or exercise i'll have to deal with the pain for weeks unable to do simple tasks ...

Thanks for the video. As I am going through youtube this was the first video that made me understand what happened to me really. My doctors said I have a grade 3 injury. I am on rehab now feels okay for every day movements. The only question that I cannot get an answer for still is that will I be able to go back to boxing? Can these ligaments reattach without surgery only by themselves/my body? Will it hold enough for me to land a bomb left hook comfortably? I would appreaciate an answer because my doctors says I don’t need surgery but when I ask these questions they just try to ignore it. Thanks in advance!

Sounds like what happened to me. ER doc said the 1 torn ligament would heal by itself in 2 weeks and I would recover 100%. it was a pseudo-dislocation or ac joint separation. Let me know if u can box well. I would like to know. I enjoy lifting weights. I'm at week 3

Hi! Have problems with my right shoulder after lifting weight in the gym. No fysio seem to help after over 8 months. I've been MR scanned. And the following has been described. [ Small focal fluid accumulation corresponding to the tendon attachment and medial to the coracoclavicular ligament. ] What does this mean? And what can be done for treatment? Hope you can help point me in the right direction. The pain I got, prevents me from riding Mountainbike(enduro) and pressing in gym like benchpress. I can't lift my arm above 90 dregrees. I feel pain in my pec minor when I shrug or roll my shoulder.

I have pain ( I would rate about 2 on a scale of 1 to 10) at the lateral end of clavicle. The pain is present on trying to horizontally abduct arm against resistance ( like rear delt fly) There is no pain or minimal pain on palpation or pulling the arm downwards or on doing bench press. However it has been a week now. Is it likely to be only a minor sprain of the AC joint? How long will it take to heal?

i recently had surgery for ac rupture grade 3. i really hope i will be able to bench press heavy again. The clavicle seems to have been connected well with the coracoid, but it still slightly looks like a grade 2, and i can push down the clavicle a few milimeters, surgeon says the ligament will be as strong as the other side. do you think i will be able to press heavy (100%) with the shoulder blade retracted, i can't seem to figure out what factors may limit me?

6 weeks in from a grade III, no surgery and steadily improving. Pain-free and regained full range of movement. Still feels a bit strange in certain positions. Early days. Good luck with yours.

Is it possible to have this type of injury but not feeling any pain until the following day? I awoke the day after a gym session and my shoulder was sore, to the point where I couldn't lift a frying pan and struggled to hold the steering wheel. My shoulder was red with a slight lump. Two weeks later it had subsided and I had no pain so tried a light workout thinking it was just a sprain. I wasn't 100 though particularly on the bench, so I decided to lay off for a month. I've noticed a slight bony bump in which oddly pops up a bit more if I brush my teeth. 😊 I'm at the doctor's shortly so maybe it's this type of injury.

All of a sudden I had extreme pain just moving my arm and shoulder. Also painful to touch the bump on my collarbone. I had no idea what caused it. No accidents or falls. The day before I did some dips. That might be what caused it, but the dips weren't a strain to do, and there was no indication that anything was wrong. Thankfully the pain went away in three days. But now I'm anxious about doing pushups and I don't know when I should try dips again.

If u r in constant pain u need to go to surgery, but it hurts u only when u r doing gym then u may not opt to surgery,I have also same problem I didnt went to gym for 2 yrs I was also a regular gym goer n I know it is frustrating to not to do workouts but life be sometimes be very unfair I want to say that dont go to gym until its recovers so meet some professional orthopedic surgeon n have talk with him.

I had a grade 3 from a bike accident as well. I couldn't do chest or shoulders in the gym for about 8 months which was very infuriating as a frequent gym goer. I do believe after time my muscles developed in such a way I was able to start light and work my way back up the weights slightly but it never felt great underloads on my bad side and also caused a lot of my back muscles to over size in comparison to my good side. I ended having the surgery 3 days ago as I didn't want miss the opportunity while I'm still young, and if it would cause problems later on in life.... plus the ugly look of a massive lump poking out. I got mine done through the NHS tho.... so I didn't have to front any costs. If I remember I can let you know how I'm feeling in a couple months when I try get back to he gym

Got the dog bone surgery on my grade 3 separation last December 8th. I'm almost 1 year post op and I feel great!! The bone isnt sticking out anymore and most of the time I don't notice it. I was back to work in construction after 5 months. It bothered me up until 10 months post op then it felt normal. I still baby it because I heard it takes a solid year to fully heal. Get the surgery rest and eat well during the healing process and you will get back to feeling normal in no time. Keep your head up!!

@@bobcat2938 it doesnt tear easily, when I was doing double bar dip i went very down n it happened that time n it was paining a lot i did mri n it came thier my doctor told me to rest 8 months n stop lifting weights so I took off from gym but it didn't completely healed now also whenever I do bar bel press shoulder press it hurts me so I stopped doing those n I also feel weak in left arm so I m wondering that it will ever be the same or it regains strength but I dont think that it will bcome normal bcz nothing changed even I took physiotherapy n pills so I doubt it.

What about AC joint degeneration? I injured this joint 8 years ago when kayaking. I've been to see 2 physio's and a shoulder surgeon, I've had an MRI scan and done physio for years. There's been a slight improvement but not much. I have mobility and strength of the shoulder but I am in pain for days after doing press ups, pull ups or anything that requires me to hold my arm out in front. There's very little information about this injury. The physio's keep telling me that my problem isn't that bad. It's bad, it's limiting my life.

I believe I suffer from grade III, however, I do not know what is causing that. In the past weeks, I see this elevation on the right side of my shoulder, and the pain is increasing each day, but no doctor can tell me what I am doing which is enabling that. What could be the potential causes? I want to avoid that to become a grade IV, and higher.

It’s unlikely that a Grade III ACJ injury occurs without any known reason, a trauma or clear mechanism of injury is how they occur. As a result, it is not something that develops from a Grade III to a higher grade just like that

im 11 days into my stage 3 separation, and i still feel like if i drop my arm my shoulder is gonna fall apart 🤣 i cant get it off my mind, its almost drivin me crazy lol. but im hopin within a month from injury, im back to 95% at least. its just such a weird injury/separation..such a weird thought that your whole shoulder is kinda held together by those 3 or 4 tendons.

U need to stop doing heavy lifting u need to exercise with free weight n do some yoga u should not lift for 8 months n start rehabilitation of ur shoulder with physiotherapy u do give it rest to heal.

USA health care system is a joke so sorry to hear…I live in a south American country and here the system don’t work so good…but if I compare both I choose Chilean healthcare system any day…here surgery is free may be all the time is real that is not fast but it works…in the USA ppl dies on ER because they don’t have insurance…that is an abomination and the ppl of the greatest country in the world can’t accept something like that…Hope you change it soon…
