They’ve likely all killed someone, except maybe Usopp, ironically enough. It’s just that the ones they did kill weren’t pointed out, and the main antagonists usually manage to survive. Oda prefers to have Luffy’s enemies crushed and forced to live with the fact that their dreams will never come to fruition. So everyone who survives does so not because Luffy pulls his punches (he doesn’t), but because Oda says so. Although, Luffy probably killed Kaido. I doubt even he could survive a dip in a magma bath. Which, ironically enough, is exactly what Kaido wanted.

Personally, Luffy's voice complements the show so well, when you consider the show's *silly, cartoony style and tone* and the world he inhabits, nearly every character have very exaggerated designs or features. His voice make it his own distinct exaggerated feature.
