This is SO important to talk about, especially the whole bit about antibiotics not working against viral illnesses. On the pharmacy side we are 100% always trying to limit the use of antibiotics as much as possible. At my community pharmacy we've even had to have conversations with local doctors about unnecessary prescribing of antibiotics. It's a huge issue that's really become bigger and bigger in recent years so thank you so much for bringing it up!

I want to say as a patient, I so appreciate the idea that doctors and pharmacists may work together like this to help patients avoid unnecessary antibiotic prescriptions. I never reall thought about how critical it was to have effective (i.e., not totally overprescribed) antibiotics on hand until I needed them for my own treatment. I was super healthy as a child and only took antibiotics about two times (strep once, an ear infection another time). Fast forward to last year and, thanks to a wound I was receiving treatment for, I wound up in septic shock and my poor OHSU medical team discovered that I'm allergic to whole branches of antibiotic types (like anything stemming from penicillin, and at least three other options), and they were struggling to find anything that could help address my alarming white blood count... not to mention the C. diff I got from the initial rounds of antibiotics. I'm so thankful that there were so many antibiotic options to try and also that I wasn't so overexposed to them that I'd either unnecessarily risked my health or no longer found any of them useful when I needed them. Oral vancomycin may taste awful, but it sure did me a lot of good when it was properly administered!

I will only take antibiotics when absolutely necessary. That means I have only used them approximately half a dozen times in nearly 50 years.

@@oregonchick76 Sound like C&S was not performed for you in the initial stage. To get c.dif and super bug.wow. Vancomycin is a pain in the ass effective high end antibiotic.

Just don't keep them from your patients when they need antibiotics. I know when I have a bacterial infection that I need them and I shouldn't have to persuade doctors to prescribe them.

I went to a doctor once for generic throat symptoms and fever. She suspected it was mono because the rapid strep test came up negative. She ordered a different strep test and prescribed me amoxicillin anyways. I learned very quickly that taking a penicillin based drug during a mono flare-up causes a full body rash that lasts for several days after the medication is stopped. I really wish that she had waited a day or two to get the results for the strep test she ordered to come back before prescribing antibiotics for me. Having a full body rash is not fun.

That sounds like an allergy, and it really sucks. I had the same issue with the medication that was helping me keep my BPD under control, and it sucks when you have to choose between treating a serious problem or clawing your skin off.

With antibiotic resistance going up, it's important to only give antibiotics when it actually is a bacterial infection rather than a viral one. I wish more doctors would say no.

katana_92 and especially important that when people start a course of antibiotics... FINISH THE ENTIRE COURSE and don’t just stop cause you “feel better”, otherwise you also increase the risk of resistance.

Well its not that simple. Viral infections can normally turn to bacterial as a complication. You may develop a serious illness from a normal flu in a given circumstance. Thats why you need a doctor. Trust me im a former professional

@@darugdawg2453 Yes that's fair. I'm just speaking for the instances where it is relatively clear that it is a viral complication rather than a bacterial and the patient still wants antibiotics. It's both the doctors and the patients responsibility in the end to ensure there is no unnecessary antibiotic use, or at least be pretty sure.

@@games68775 just be careful not to self diagnose and self medicate. The only problem is cough cold and fever are common signs and symptoms of many illnesses (bodys response). During 3rdyr college my classmate died from typhoid in just a week because she thought twas just a flu

Could you talk about migraines? I feel like it's rarely talked about and quite downplayed although it can have an effect on work and private life. I remember the first time I had a migraine with an aura I actually thought I had a stroke. Thank you!

I have migraines with auras, too. My doctors have always blown them off as "meh" until recently. I went to a specialist and she became upset that my doctors did not treat it seriously as I have been on the combination birth control pill for years and could have died from a stroke! Why don't doctors take it seriously?!?!

I agree some of the people who know me don't understand or think that migraines can be quite serious, I have regular chronic migraines with auras and a rare form called hemiplegic migraines that is a migraine that mimics a stroke and I can be paralysed for up to 3 days unfortunately, I have been told that because of these 2 types of migraines as well as a brain injury and other health conditions I could die from the more rare ones but have to be extremely careful with what medication I can take and what I can't

They have actually been working to deter doctors from prescribing antibiotics for illnesses that are suspected to not have any bacterial origin, or over-prescribing for things that the healthy body can fight of perfectly fine, like a boil. Not sure how much progress they have made with it, but I know that all the hospitals I have been working at lately are focused on lessening the amount of antibiotics being prescribed for minor things due to the rise in superbugs.

That's so wild. You know until I started learning about American healthcare shit I never heard of doctors prescribing antibiotics for anything other than a bacterial infection. Idk if it's because of our universal healthcare (so patients don't feel as entitled to shit from their doctor bc they're not paying for the visits, they get taxed regardless so it's just "I'm not feeling well better see what the doctor has to say about it") or the different cultural attitude (Canadians are kinda known for not being imposing lol) or bc of our education system (we're #2 in the world whereas USA is bottom ranking in the developed world) but yea.... It's weird.... Like until my 20s I thought it was well known that cold and flu = virus. And that antibiotics are only any good for bacteria. Like that's 9th grade science class at most. Like I mean I remember my 9th grade science teacher going in depth about that shit, but previous years did cover it too.

@@anniebe4992 Every patient is different. He is most likely older and probably has some sort of condition that lowers his immune system a little more than a healthy adult, so the doctor gives him more antibiotics to help. In the medical field, it's all relative.

When we were covering antibiotics in pharmacology, our lecturer had said something about the treatment of viral upper respiratory tract infections along the lines of: “if we treat it, it’ll be gone in a week, but if we don’t, it’ll resolve itself in 7 days”.

Dr. Mike! Can you make a step by step tutorial on how to properly clean your ears? Everyone says not to use q-tips but no one demonstrates how you should do it! Thx!

You should only clean the outside with a damp towel. The outer ear canal basically cleans itself. The reason you shouldn't use q-tips is because it pushes the earwax further into the canal and it may get clogged that way. Also, you could hurt your tympanic membrane if you push a q-tip too far in. Very few people need to have their ear canal cleaned and it should only be done by a professional (Ear, nose and throat doctor). Hope this helps!

Bella R Cleaning the inside of your ear should only be done at a Doctors office. Your PCP can do a basic cleaning for excessive wax and maybe a very mild case of impact, or get you to an ENT doctor for anything else. Remember you NEED some wax in your ear to protect your ear. Only clean the outside the earlobe and do not go near the ear-canal.

I have excessive ear wax, it was so bad one time I went to the doctor and they pulled a glob out of my ear the size of a rat turd, but since then I use an ear syringe and warm water and that keeps it from getting clogged usually. There's also ear wax removal drops you can get that is just a mild acid to help loosen the wax then use an ear syringe to help flush it out.

Doctors dont know what they are doing with this large scale experiment that is vaccination. Life has survived millions of years without vaccines. We dont know the long term consequences of having a vaccinated population. Life has survived through adaptation and most importantly, natural selection that gets rid of inferior specimens and genetics. Vaccines goal is to circumvent natural selection, this could have devastating long term effects. Vaccines could just be short term gains, we need to create a separate population and test the long term consequences of vaccinating, and not experiment on everyone.

I got really really sick about 4 years about. I ended up in the hospital for an insane amount of time. I also had both MRSA and VRE. There's only one antibiotic out there left that works for me. Once that stops working there's nothing left. I'm thankful every time my doctor doesn't give me an antibiotic as a first resort.

@Kevin Prima Indeed, and that's a terrifying thought. Almost no one talks about it, though! There needs to be public awareness campaigns, by the government, by hospitals, by clinics, because we are royally fucked and not doing anything will only speed the timeline for it happening up.

My 3yr old and I live at the Urgent Care lol. Best advice ever given: Doc says, "If you're sick all over it's viral and wait it out. If you're sick in an isolated location such as ear, kidney, eye it's probably bacterial and need antibiotics." This advice has saved me a fortune and lots of time!

I agree with you 100%. I am a mother of four and grandma of eight. When my children were young their pediatrician and I had this same talk. I already knew about the differences in bacterial and viral infections. Therefore, the doctor knew my stand on unnecessary medicine was in line with his. Now my adult children have the same knowledge and they are taking their children to the same pediatrician they went to. Keep up the good work.

I had concerns about epidurals as well. (The dangers of Google-fu.) When I was in labor my contractions were crazy (15 seconds long with 30 second breaks) but I tried to power through them. The L&D nurse finally asked why I was refusing and I started going through my concerns, (forgetting the most important one, that I react super strongly to anesthesia) and she told me that the only patient she'd had in 15 years of nursing that had a bad epidural was a 16y/o who wouldn't stay still. My epidural was fine, though I was far more mobile than they expected. (Not able to walk, but I had full control of my limbs.) Discuss your concerns with your OB and the L&D staff. They have the training and experience.

I had an epidural with my daughter it didn’t hurt that bad it was more like a bee sting if you stay still and breathe throughout it you’ll be fine

@@corpselizzy Not necessarily true, the doctor is human and as a human does make mistakes, the doctors hand could slip or some other error occurs and the inject the medicine into the wrong part of your spine causing complications after for a lifetime like pain, numbness and even paralyzation. However, these complications are not incredibly common, but they DO happen. I myself would never get an epidural for giving birth for a couple reason. Although the risks aren't common, they're still pretty serious risks that to me don't outweigh the benefits, a possibility of being paralysed isn't worth pain relief for a couple of hours to a day or so. Also when you have the epidural you don't have control over pushing, you must be on your back, you're more likely to need help delivering, it could lead to a C section and you cannot move your legs until the epidural has worn off and once it has, I don't feel like feeling the pain after I get sensation in my legs back. IMO I'd rather just accept the pain, pain is temporary, complications from getting rid of that pain are permanent.

But also for me numbness freaks me the fuck out, I hate it to the point that whenever I get dental procedures done I always refuse numbing, I'd so much rather just feel the pain because the numbness sends me into a panic attack personally.

I actually love it when doctors don't give me antibiotics they always make me feel so ill, and I'm 100% sure we don't need them near as much as they are given. I rarely will go to a doctor because they so happy to hand them out... Drives me crazy. So good job Dr Mike 👍🌸

I went to see a young doc who just took over a practice. I had the flu and a bit of a cough. He immediatly wanted to prescribe antibiotics and i had to turn him down. We are at a point where we need to educate doctors the way we did with patients for the last 15 years.

Wait...If you knew you had the flu, or just a viral thing, why did you go to the doctor? I mean, unless your projectile ejecting from both ends everything that goes in, theres no reason to go in if you Know that your suffering from a viral infection. In fact, its best to Not go in and spread it around. So, why did you spend the money to go in and have them do nothing but try to give you a drug you say you already knew you didnt need? Beyond the fact that you wanted to try make a point that young doctors are idiots who have to be taught.

@@jesiahmalaory3686 at that point i had to work outside in pretty harsh conditions. Where i live you don't just call in sick, you need a doctor to approve. I was having a decent fever and continued work outside in late autumn would have been just torture. And i did not spend extra money, we have universal healthcare. That is how the civilized parts of the world roll.

@@jesiahmalaory3686 actually, it's good to go in if you have the flu, especially if it's within the first 24 hours, you can go in to urgent care, they can run a test confirming you have the flu and then they can give you a vaccine (I forget what my doctor called it exactly) that will greatly reduce your symptoms AND help you heal faster or something like that. BUT, you HAVE to get in as SOON as you notice the symptoms (weakness, sore muscles, fatigue, etc.) because it's no use after the first 24 hours. (Or maybe it was 48, I'll have to ask my doctor again).

Viral infection, huh? That makes me remember when I was still in the pharmacy school and one of my classmates got infected by chickenpox... Her doctor prescribed her antibiotics. She blankly looked at the doctor, asked if he was sure, when he didn't change the prescription, she called our teacher and told him the medicines that the doctor prescribed to her. And my teacher was like, "You got chickenpox and got antibiotics?" and then the whole class went, "But antibiotics can't work on viruses, right?" People who work in the med field should know viruses are technically NOT *Biotics,* right??? Btw, my teacher told her to not consume the antibiotics and gave her the appropriate medicine. (Ofc, my teacher is a licensed pharmacist, so don't worry!)

Brittany Gabaldon this. Omg .-. I’m so glad I’m not the only one. I was fighting a bug, that they had no idea what was to begin with, and their snap choice was penicillin. Uhh.. no. Next. Went through the whole list. Settled on just a higher pain med with the hope I’d get better on my own. Three months later, I came out of it. But it was terrifying.

@@artemistify I am lucky I haven't had anything serious. Worst is when I had a kidney infection and there was like only 3 meds i could take for it. But yeah lots of ppl are allergic to penicillin, my father is allergic to it as well. Some just get a rash, I stop breathing.

My ex just suffered sever burns. He is allergic to the cillians and he had an infection. It was a very rough week and the doctors handled it well Even with all of the complications he threw their way. Thankfully after a week they sent him home. On 2 different anti-biotocs.

Coming from another country, patient care here is extremely different in the US and this video hit the nail on the head with regards to treatments I have been offered. Several times, I have been prescribed, what I feel, are extremely strong medications. Being someone that rarely takes anything more than an Advil or cough syrup, I can't tell you how many times I have been given pain pills, narcotics, etc for mild symptoms. After telling different doctors that I was hoping to learn about more "natural" methods of treatment, most don't know what to suggest. I wish there were more doctors like you in my area to suggest naturally proven methods like green tea with honey! Too many look at me like I'm crazy when I say I don't want to take pills if it's not necessary. It's pretty sad.

4:16 coming from a low income family my mom didn't have us established with primary care and didn't like taking us into the urgent care. I had been sick for over a week and it seemed to be getting worse. I was complaining about a sore throat which progressed to having a sore low back. Finally on a outing to a bookstore with my grandma I passed out and she demanded my mom take me in. My temp was 104°F. Turns out the strep had spread to my kidney/complication involving my kidneys to where the doctor told her if she would've kept waiting it out I would be dead. Definitely made my mom rethink caring about money over health.

I have IBS and my dietician said the following. Your large intestine is braided as such there are places where food can get stuck if it isn't working properly, this causes gass to form in the large intestine and you bloat, because the large intestine can't really inflate, this causes pain. There is a lot of causes of IBS, high levels of Stress, traumatic episode, continuous antibiotics. There are also different types of treatments, I need to use probiflora daily, drink at least 2 - 3 liters of water a day, and I had to change my diet.

I got C diff from antibiotics and it wasn't pretty! I chose to do H Pylori eradication therapy with 2 antibiotics and Omez for 2 weeks. A week later I started with signs of C Diff. I was sick for months but I have a history of nervous tummy due to anxiety. I went to my doc once a week for a month before my doc agreed that I may have contracted C diff. I was put on Flagyl which made me so ill but I killed the c diff. It took months of probiotics, L glutamine, and a very strict elimination diet to get my bowels back to "normal". Now I am so weary of taking antibiotics my doc and I sit and discuss the pros and cons and alternatives to antibiotics before making the decision - and we usually opt for no antibiotic!

I love this so much, once a patient leave my office mad because I didn't prescribe antibiotics, and came one hour later with a prescription for 1 gr of intramuscular ceftriaxone, apparently she went with other doctor who will prescribe what she wanted, she came back 3 days later with no improvement but still didn't accept my treatment

You have to be the smartest doctor I've ever seen. Most of the others like to just prescribe meds for anything and everything, when there are cheaper, better, and less problematic alternatives. I've seen too many people given drugs for things that could've been solved through something like simple vitamins and herbs, and maybe some aspirin. But then these are patients will have a cocktail of meds to go through 5x a day, and it's ridiculous. I'm just glad you're aware of the fact that medicine isn't actually required for everything. Very few doctors I've seen/heard about aren't like that at all.

I've gone to the doctor on the first day of feeling sick for two reasons, we had a really bad flu tearing through my work and I wanted to get ahead of it if possible, and secondly I live in Australia, a doctors visit costs $70, less if you're eligible for a concession.

I'd rather hear that than an arrogant doctor who thinks he knows everything and gets angry and defensive when you question his diagnosis, the efficiency or risks of his treatment or prescriptions, even when he's wrong, which happens A LOT. Like doctors who tell you opiates or benzodiazepine aren't addictive.

I would personally choose a doctor who is known not to over subscribe antibiotics over one who does. Personally, I think that prescription of antibiotics is a greater problem than the opioid epidemic, since that mostly affects the user, not everyone else, especially the most medically vulnerable. I think that significant legislation should be brought in to sanction any medical professionals, who cannot back up a prescription of antibiotics with a well reasoned risk/benefit analysis (with risk being comprised not only of risk to an individual patient, but society and medicine as a whole). In summation, im very glad you're a dressing this topic, and in a way that makes it easy to understand for those who don't have a background in science.

My precise phrasing was ‘mostly’ which I stand by in comparison to overuse of antibiotics. With substance abuse, the user dies. In comparison, overuse of antibiotics creates resistant strains of infections, which usually affect the most vulnerable first and most. So where opioids affect the user and their direct surroundings, overuse of antibiotics affects other people and those we should be protecting most as a society.

I experienced a really arb situation not too long ago... Same story - working insane hours and being stressed out to the point I got really ill... I contracted the flu and then got nailed with viral labrynthitis, so I reluctantly went to the doctor (I usually rely on my immune system to clear up any minor bugs). Since viral labrynthitis can't be treated with antibiotics, he sent me home with instructions to inhale steam several times a day (head over a basin of boiling water with a towel over my head), and my condition got progressively worse (I couldn't walk because of the dizziness and nausea). After a month I went back to the doctor and they discovered that I also had a nasty sinus infection that was obviously making me feel even worse. In the end I got prescribed some antibiotics and I eventually got better. That was one of the roughest months of my life because viral labrynthitis alone is an absolute bitch of a bugger! I totally agree that taking antibiotics unnecessarily (or any drug for that matter), should be avoided, however, the doctor really needs to be sure that there aren't any other underlying conditions... Unique situation, though.

I remember a patient I was drawing blood for, because of chest pain, said 'i thought i could come in a get antibiotics'.... for chest pain?? I'm only a phlebotomist but I don't think antibiotics help for chest pain unless its caused by bacteria but i dont think it was in this case. Some people arent educated enough to know that antibiotics arent always the answer. I'm glad your educating people on this topic.

thanks for advocating, dr mike. my immune system and gut micro biome is completely destroyed after 1. being given two rounds of antibiotics at age 2 for swimmer’s ear and 2. being given 7 different kinds of antibiotics for 1 & 1/2 years straight for Lyme disease and co-infections (which still haven’t resolved after 8 years and are worse than ever). i now have sibo, candida overgrowth, leaky gut, tons of severe allergies, and a bunch of other (probably unrelated) problems like thyroid, adrenal, and immune deficiencies and widespread neuropathy. i’m glad people are finally educating not only other physicians, but also patients about the dangers of antibiotic overuse.

Cool video idea people have big misconceptions about antibiotics!! I remember learning about antibiotics in biology. We were 12/13 y-o. And i remember the teacher teaching us the same motto the government was using in an add on tv: «les anti-biotiques, c'est pas automatique» So far (i'm 23) this belief that antibiotics are not automatically efficient has stayed with me.

I'm no doctor, but chiropracty is literally a made up pseudo-science thought up by a 19th century grifter. It has the same effects as getting a massage.

Everyone needs to watch this video. The overprescription/overuse of antibiotics is one of my biggest pet peeves. It's crazy how many people don't understand the dangers.
