where on a new setup should the barrel adjuster be all the way in or out?

I learnt in the bike shop to turn it all the way back in and then back out 2 turns before going to the derailleur end and pulling firmly and fixing in place with the bolt. This gives you some scope to adjust then

I need the plastic screw that you removed to insert the cable, then reinstalled after inserting the cable. I haven't been able to find one at a bike shop or online. This is for a 2012 XT shifter. Can you help?

Are you using the shimano cables? I mived to another brand as i found the coating was causing jamming as it wore. probably not as bad with mtb but even so i use aftermarket on road and mtb with no issues.

Glenn, personally I always use shimano cables the polymer coated ones do damage very easily though!!
