PCRI's Alex asks medical oncologist, Mark Scholz, MD, questions from our KZbin comments on whether you can reduce the size of the prostate with dietary changes, why Flomax causes nasal congestion and for how long, and whether it is advisable to switch from Zytiga to Erleada to avoid having to take prednisone.
0:05 Can you reduce the size of a prostate with dietary changes?
1:42 Why does tamsulosin (Flomax) cause nasal congestion and how long does this side effect last?
2:52 I'm 57 and was diagnosed with advanced prostate cancer 18 months ago (PSA was 178 and Gleason 3+4). I've been on Zytiga for 18 months and my PSA has been 0.01 for almost a year. Recently, Erleada was approved in my country, and I want to know if it would be advisable to switch to that from Zytiga to avoid the side effects of prednisone.

Flomax side effect discussion should include the most common one - retrograde ejaculation. Most men will get some degree of this. Of course it is harmless but sometimes bothersome or concerning to men who have not been warned

Along with that, expect vague, unsatisfying orgasms. Look into "prostate artery embolization" for relief without side effects.

Great info. I started taking .04mg 1hr before bedtime. It does help & keeps me from getting up 4-6 times a night. However, it caused a tiny hole in my Dad's left eye, according to our well respected Family Eye Doctor in Houston. It started blurring my vision, causing dry eyes, extreme irritation, nasal passage congestion, dry ejaculation, etc. I just stopped taking it. Not worth it for me & the Men in my Family. Our Eye Doctor advised us to stay away from Flowmax!

I was having frequent and urgent urination issues. I had been having my PSA tested regularly for several years and was always fine. Then, one day, my doctor put me on Flomax (Tamsulosin) .4mg daily. Sometime after that, my PSA went from being normal, to 12, in about a 4 month period. After an MRI, I was told that I had Stage 4 Metastatic Prostate cancer. I didn't know prostate cancer was a know and potential side effect when I started taking Flomax. I do now!... but now it's too late for me. I'm strongly advising none of you use Flomax.

I know you won't believe me but two different doctors more than three years ago diagnosed me with prostate cancer and were prepared to put me through various procedures. I said no, did much research, and cured it myself! Yes I did. My doctors were confused, and I have never told them how I did it. Ever since, I have been free of cancer and my urological system has been just fine.

I'm 29 male and I've been on this medication for the past 8 years (since I was 21). However, not for an enlarged prostate but a psychogenic bladder (shy bladder). It certainly helped and I no longer really take it as I have reduced the psychogenic bladder with exposure therapy. The only side effects I noticed are retrograde ejaculation and when starting the drug tiredness and fatigue. This resolved within a few weeks.

As prescribed by doctors that I have prostate cancer in one of 12 areas tested by the doctor. My parents was going up within a 3 tests from 4 to 12 to 19. Doctor advice was to wait and watch. I moved to another urology specialist who had red the tests and when tested psa was 19 confirmed gave me the option of removing the prostate , chemo or laser which I choose. That was in 2012 and radiation treatment was for 42 radiation treatments in a 10 month period. After that was having no medications but follow up in with another doctor. 2019 came back to michigan to my old doctor and since then following up with him. Psa is now 0.02 with enlargement , medicine f1inasteride and tumlosin is now 74 down from 125. The doctor is giving me injection in my belly since last year every 6 month. Stream of urine is not bad but I go more than 10 times in 24 hours. What is next? Any help

@ThePCRI 2 жыл бұрын
For BPH, his philosophy is similar to most internists and doctors in general. Try the medications to see if they are effective without side effects since the side effects are reversible. Then, if it doesn't work, consider procedures that have risks of permanent side effects. For prostate cancer, that's not true. He has a lot of videos advocating for aggressive upfront treatment in people with high risk of developing metastases so that they will not have to remain on medication indefinitely.

@@ThePCRI I have BPH and have been on Tamsulosin while I explore lifestyle changes which have thus far resulted in normalized cholesterol (Dr. insisted I take a statin...I said FU), lowered ALC and improved blood sugars and much less insulin needed to manage. My RA has also subsided and meds significantly reduced. End of the day I have either been able to reduce or remove several Rx meds. I have even been able to reduce my Tamsulosin in dose and frequency. My point in all of this was that this "Doctor" immediately said lifestyle changes could not improve BPH, while I stand here saying, Yes, it does. Quit lying to us already. Doctors with their god complex tell us pills, pills, pills and more pills. Let us kill you with pills because it "helps." Doctors know damn good and well lifestyle makes all the difference in the world when it comes to preventing and reversing disease. God and nature has provided us with everything we could ever need. To support this, look at the vast majority of Rx meds. They are based on naturally occurring sources. Now, if you are really sick it makes sense to take the meds while exploring lifestyle and mindset changes.

@ThePCRI
@ThePCRI 2 жыл бұрын
That is true for some people, but the medications are effective in some men, and the TURP has risks of permanent side effects whereas the side effects of medications are reversible, which is why most doctors advise trying medications first, before committing to a potentially risky procedure.

Typical urologist - he won't even mention PAE prostate artery embolization. I had that done two months ago. It is a minimally invasive procedure done by interventional radiologists. It reduces blood flow to the prostate. It is safe and highly effective in reducing the size of the prostate and provides for relief of lower urinary tract symptoms without the nasty side effects of Finasteride or Tamsulosin.
@ThePCRI
@ThePCRI 2 жыл бұрын
He's a medical oncologist, and he does not discuss PAE because it was a question about the side effects of Flomax--it was not meant to be comphrensive about the treatment of BPH. We have this video in which he discusses PAE: kzbin.info/www/bejne/nHbFemluh82Xlas. We also have a few other videos that are more comprehensive about BPH that you can find by searching "PCRI BPH."
