I don’t think the gap is an issue. Your decision for application will be based on how you can related your current work field experience to the course you studied in the first place. If you choose to start afresh too, there is no problem.

PhD application is more or less like the masters application, so this also applies. The only thing you might need to confirm is if you really need to get a supervisor before getting admitted.

Unofficial transcripts are simply the combination of your results from 100level. I am sure you have that, just scan them together and upload it.

It is compulsory for some schools while others don’t really care, I think the reason mostly is so your supervisor can secure funds for your research if they really approve you from the start.
