Hai Lan calls Ruyi 姐姐 (which literally means older sister) because they are really close. Ruyi never asked her to respect their positions and calling someone who is not from your family 姐姐 denotes a really close relationship. When they are with other consorts they do not trust Hailan calls Ruyi 皇后 (Empress).
