I went the physical therapy route after my first dislocation which caused a slight tear in my labrum. I ended up dislocating it several times after that because THE LABRUM WILL NOT HEAL BACK TO THE BONE WITHOUT SURGERY. Physical therapy will only strengthen the muscles around the shoulder to help keep your shoulder in place but it will never fix issue. Physical therapy is beneficial for mobility, but it does nothing to fix a tear in your labrum.

Correct, no conservative care (or potentially even surgery) will heal the labrum. However adhesion release could make the labrum a non-issue if the muscles are adhesoin free and can be strong.

I've torn mine and the doc was putting me off surgery and told me to rehab it I'm 28 now and 2 years down the line and it's still bad and actually causing other pains in my traps/ shoulder blade area🙄

I'm confused. What does a Labrum tear have to do with an adhesion? If someone has been diagnosed with a Labral tear from an MRI, isn't that the issue? I'm confused why you are stating not to get surgery, but come to you for help with adhesions. Do you help with tears? When you find an adhesion and fix the adhesion, does that help the tear?

I was a chronic shoulder dislocater with a torn labrum, and a broken humerus had surgery, and man on man do I feel better now pain just stiffness! Surgery can really help

I have damage to just about everything in my shoulder which couldn’t be fixed without multiple surgeries. The only real surgical remedy would be a complete replacement. I have yet to read or speak to anyone who has had a successful result from any surgery. And the recovery time is too long in every case. I have been doing rehab, PT, and strengthening the muscles around it and that has improved the usage and pain. At some point you just have to live with it and work around it.

It sounds like you've been through a lot with your shoulder. Keep doing what works best for you, and hopefully, you'll continue to find ways to navigate and adapt with your shoulder.

* Thickening and mild increased signal intensity of supraspinatous tendon near its insertion - suggests tendinosis. * irregularity of the anteroinferior labrum with linear hyperintesity extending from the labral surface to the subperiosteal region of the anterior scapular neck---- suggests possbility of labral tear (anterior labroligamenotus periosteal sleeve avulsion) * An elongated multioculated cyst commencing near the anteroinferior glenoid margin and tracking aneroinferiorly---- suggests paralabral cyst. * Mild fatty infiltration of the few fibres of the subscapularis muscle in the inferolateral aspect suggests focal chronic atrophy. I have a labrum tear and bursitis. MRI: "small amount of high signal in the anterior rim of the glenoid labrum that likely reflects a tear of the anterior rim of the glenoid labrum." I do physical therapy 2x each week for the last 5 weeks and I don't feel any different. There's no pain in the shoulder, only in the tricep. It does pop a little when I lift my arm a certain way. Does that sound like it needs surgery? I miss doing handstands.

You could need surgery, it would be better if you can get checked for adhesion first. May be able to prevent surgery.

Great information! Thanks. I had my injury 3 months ago and just now got my diagnosis. I believe i have a anterior labrum (my pain is on the upper upper bicep) i have not had an MRI (i am a cash pt). Doctor told me healing window was closed and i need surgery. I really don't want to get one. Is there anyone you recommend located in Houston for a second opinion or try some therapy first? I'm 20 yo, I tried to do PR in Incline bench press and a week later my right anterior shoulder started to pain. It all happened in July, 2022. I tried few strengthening excercises but no results. Then went to Physiotherapist in November, they did adhesion release 3-5 times (I haven't had any MRI or any test at that time). Felt a little better but after that right rhomboid started to pain, maybe because of bad sitting posture or rounded shoulders. Currently I have both shoulder and rhomboid pain (rhomboid can be tight I think) I had MRI scan of shoulder yesterday, here's what it says - - Thickening of anterior joint capsule along with superior, middle and inferior glenohumeral ligaments. - SLAP tear from 11 O clock to 3 O clock position. - Supraspinatus tendinosis What's the best thing to go for from here? Should I go for adhesion release again and with these MRI reports, or should I go for surgery? What's best? 🙏🏻

I don’t trust where you got adhesion rleeasw based on what you’re saying. Sounds like an incomplete diagnosis. I’d try again If you can find someone reputable. You may need surgery regardless

So I have had a biceo tenodesis to alleviate the pain i have from a type 4 slap tear . The surgeon said the slap tear was really bad but all he did was a debridement. Its been 6 months and getting a new MRI because I still get a ton Of pain in the shoulder joint. Do you think labrum repair surgery would be worth it. I Have done over 24 physical therapy visits already and nothing helps.

I wouldn’t expect PT to help regardless. If you really want to avoid surgery, try an adhesion specialist for 5 treatments.

I really appreciate this video. I'm 34 my left shoulder kills me. But I've noticed when I'm working on projects that require hard labor and I just power through them, the next few days I don't have any pain. I'm not sure if this is the adhesion you're talking about

Hey friend, what we do here is different from Pt and we don’t do surgery here either, we help people fix their pain by removing adhesions which is most common cause of chronic pain symptoms, so an evaluation would be needed, are you in NJ area?

Here’s what my doctor diagnosed on my shoulder after I got my MRI: Posterior and inferior labral tear at the 6:00-9:00 positions. Multiloculated posterior paralabral cyst at the 7:00-12:00 positions extends into the spinoglenoid and suprascapular notches and measures up to 2.7 × 1.2 × 3.0 cm. Mid diffuse infraspinatus muscle edema is consistent with entrapment neuropathy. No muscle atrophy. I have surgery scheduled in 3 weeks and am a bit nervous. I’m 30 years old and have been lifting weights for the past 15 years. Would surgery help in this case?

Recovery is going really well! I’ve been out of a sling for about a month and have been doing a ton of physical therapy. I don’t want to speak too soon but I think my shoulder will feel significantly better in the near future when I get back to lifting heavier weights again.

Thank you for sharing. A specialist has recommended surgery for my SLAP. I asked for second opinion but he second specialist belong to the same medical out patient group, actually they see patients in the same facility. The second specialist did suggest cortisone shot to see if the slap improves before any surgery.

@@drchrisstepienclips They said I had “frozen shoulder”, which I believe is what you mean by adhesion. They put me in PT but still saw no progress and couldn’t afford PT anymore.

Just got my MRI report it says Moderate rotator cuff tendinosis/strain and subacromial subdeltoid bursitis. 2. Low-grade partial thickness interstitial tear distal supraspinatus tendon measuring 5 mm. 3. Appearance consistent with SLAP tear with a 5 mm postero-superior paralabral cyst. 4. Minimal changes of acromioclavicular osteoarthritis. 5. Subacromial spur. Do you think surgery would help? I’m 40 yrs old .. I don’t know what do 😢

Had surgery 10 years ago hasn't been the same since the joint is so tight, can't externally rotate it cracks and grinds and clunk's it's far worse than what it ever was before when it was injured I completely regret the surgery

I was bench pressing 315 pounds and my Right shoulder turned to jelly and the bar dropped on my ribs right side then after my arm was limp and my shoulder felt loose and weird. Scheduling an MRI takes 3 weeks before anyone can look to see exactly what's going on...no pain. Just shoulder no stability. Can not lift arm. No power. Just limp. Need to use other arm to lift right arm. Once overhead arm works. No pain. No pain really at all. Just no power won't move. Jelly like. Weird feeling in shoulder

The study seems biased to me because they did it on a slap type 2 tear and not slap type 3. It would make sense to not get surgery on a type 2.

Valid point. I'd agree that some people should get surgery. I'd say that decision is dependent on the severity of the tear (worse it is, more likely surgery is an option) vs. the severity of adhesion (worse it is, the more likely the pain can be helped conservatively). We've prevented some people with Type 2 tears from needing surgery, but we've referred some to the surgeon. So it depends??

I am 3 months now post surgery full tear on the labrum, partial rotator cuff cleanup/fray, bone debridement and tenodesis to take the pressure off of the labrum. I am 40 years old and have been working out since I was 15 so I'm hoping I can get back into the gym in a year or so obviously not too heavy. are these surgeries successful?

I injured my shoulder 2 years ago weight lifting and have had injured more. It;s a point where if i do i.e. a monkey hang on my bad shoudler or like a "throw motion" my shoulder gives a "ripping/ popping sound. I went to a ortho specialist and he said do PT and see. I did PT and went bouldering and my shoulder had that feeling and i was in intense pain and couldnt move my arm. so he ordered a MRI. basically i was wondering if you could tell me what this means. I can still powerlift but stuff like benching or over head press my ROM hurts my shoulder too much. 1. Hill-sachs deformity. 2. extensive tearing of anterior inferior labrum with detachment and tearing from 3 o'clock to 6 o'cloc position 3. Stripping of the capsular attachment as well as extension to the subjacent cartilage at the glenoid. 4. MRI also said i have a suggestion of a SLAP tear. 5. Acute edema on chronic injury.

Yes, it does help some people, but not the majority of people or else everyone would be getting PRP.

I have an inferior glenoid lebral tear - bankart lesion. Its been nearly one year. i can move normally not able to lift any weights. I am a national level athlete but i cannot perform like i did before. Is there any way to perform like before without surgery?

Yeah but once you do tear it only way to full recovery is to have the surgery cause if you fall down or sleep on it the wrong way or whatever the case may be it will hurt you for a while and not to mention as they get older it will hurt them. Trust me I've been through this process where I passed on surgery did rehab and the whole nine yards and the pain went completely away like nothing was wrong but then I fell down and it hurt me for a couple weeks

@@rupsdelson if it's only two months since the tear, it would be very unlikely to be adhesion. Adhesion takes time to build up.in the muscles around an injury. It's why one of our qualifiers is "6+months of chronic pain". What has your doctor said?

@@drchrisstepienclips they saw my MRI result and told me the only option to heal it is to get a surgery and no other options. They said that if I don't get the surgery my dislocations will get back once I go play basketball again. I message you my MRI image on your instagram and facebook to see if I need surgery or not.

I dislocated my shoulder and put it back on its original position from playing basketball. Xray result was negative. (2months ago) i can rotate my shoulder 100% without pain but when I lift heavy stuff I feel the deep pain from scale 1 to 10 maybe 3 already. MRI result was EXAM: MRI LEFT SHOULDER W CONTRAST 10/10/2022 10:13 AM COMPARISON: Radiographs dated 8/25/2022. HISTORY: Left shoulder pain. PROCEDURE: Multiplanar, multisequence MR arthrogram was performed. FINDINGS: LIGAMENTS AND TENDONS ROTATOR CUFF: No contrast within the subacromial/subdeltoid bursa to suggest full-thickness rotator cuff tear. Mild asymmetric atrophy of the teres minor muscle, without mass in the quadrilateral space. BICEPS LONG HEAD: Normal. GLENOHUMERAL LIGAMENTS: Normal. GLENOHUMERAL JOINT EFFUSION: The joint is well distended with intra-articular contrast. LOOSE BODIES: None. ARTICULAR CARTILAGE: Mild irregularity of the anteroinferior chondral surface. LABRUM: Tear involving the anterior and anteroinferior labrum with mild stripping of the glenoid periosteum. There is slight medial and inferior displacement of the labral ligamentous complex. There is an additional SLAP tear. CORACOACROMIAL ARCH AC JOINT: Minimal capsular hypertrophy. ACROMION: TYPE: 1. CORACHUMERAL SPACE: Normal. BONE/MARROW: Small Hill-Sachs impaction deformity (series 401, image 9). SOFT TISSUES, BURSA & QUADRILATERAL SPACE: Normal. IMPRESSION: 1. Recently dislocated my shoulder while moving in bed after 2 years of trouble free. Been boxing and working out hard past few weeks and it happened while I was moving in bed!! Had multiple 9-12 dislocations in both arms from 6 years of football. My left one was the one that ended up with surgery and so far no problems. However with my right arm when it happened last night i was moving myself and it was fine however I eventually felt a ripping feeling. Is it possible the Labrum retorn or too my understanding once it’s torn you can’t really tear something that’s well…already torn. Going too the doctors soon but in the meantime looking for info. Edit: I have full mobility of the arm and the pain is actually bearable. More of a slight full ache but I have a very high pain tolerance (surgery and been shot twice). Perhaps I tore scar tissue? Should I look into adhesion release therapy with my doctor? Also I suspect it tore more because I was actually half asleep so I suppose I was not fully engaging the muscle like I should’ve. Also I’m 20 so surgery is still a possibility however I wish to have a military career once I’m done with college. So I do have them to heal and recover however, while the recruiter has said one shoulder is ok so long as an MD has cleared it (which they have) I think 2 might just outright disqualify me.

Yes, you can retear the labrum again. It sounds like you have a lot of structural damage and you likely have tons of adhesionn. I'd find an Adhesion Release Methods specialist before considering surgery. But yes, surgery is probably warranted.

I'm 26 and I have been diagnosed with bankart lesion, hill sachs deformation and small fracture on my shoulder. I always loved doing sports and am planning to do it again in a secure way after I get my health. Now, after 4 weeks of my shoulder dislocation, I just want to take my shoulder back. For this reason, I have been thinking about if I get surgery to avoid high potential post dislocations. I know that I'll do physiotherapy either way. As you know, however; the chance of repairing bankart lesion itself is very very minimal. So, we came to the importance of bankart lesion for the post dislocations? I'm wondering about your suggestions. Thank you.

Same here i am 27 year old. Dislocated my arm 4 weeks ago. no fractures , no hurt muscles(that i know of). Torn labrum only. I am going for PT first and if my shoulder even subluxates once, i will get surgery. Shoulder instability and slap tears are somewhat different i guess . Anterior labrum tear seems to be connected with instability issues. Its better to get surgery right away if you dislocate second time so you dont worry about it anymore. Good luck to you

I just don't want to wait for that second dislocation in a high potential because of the labrum extension. I have a slap lesion and i am a boxer. The doctor says that i dont need to undergo a surgery for that and says that i should not lift heavy things should not swim etc for 6 weeks and if my aches reduce then i have to wait for another 6 weeks. I have tear from 12 to 8 and it really doesnt hurt or feel different i have full movement in my arm but the doctor said i need surgery, i dont know if i need or bot should i get it its gonna cost $2000 and he said because im double jointed there is a chance it will happen again so i dont know what to do?

If you have FULL ROM and still have pain, we would definitely refer you straight to the surgeon. It's possible you can get some relief with strengthening if you're really weak, but sounds like you may need the surgery.

It really depends Brut. I have no idea how much adhesion you have, It's probably a lot though. Can you fly to an adhesion doc? We just had two young baseball players fly from Dominican Republic and Puerto Rico to see us in NJ.

Hello Doc, big fan, I have hypermobility joints and 3 dislocation(Bankart Lesion) on right and 2 on left mainly due to rugby and my just profound condition , I am now 21 and have decided to retire from rugby, I am not fond of surgery, can I know whether the shoulder labrum tear can heal on its own by time? I know that the Hip does not heal, how about the shoulders. I also have shoulder clicking frequently whenever I move my left shoulder. Something pop up in my mind that even if I strengthen my rotator cuffs, it would only be temporary right? Because I can’t see my 60 year old doing such exercises which will then increase chance of dislocation when I reach that age. I really hope you can help me on these. Thank you in advance.

Hey. Meor, TY for kind words. At that age, it sounds like labrum is a big issue. I'd strongly consider surgery as it has a high degree of success the younger you are. I'm considering it at 38 for my hip.

The doctor said i most likely have a posterior labrum slap tear. I didn’t to the mri yet, but i hut my shoulder doing overhead press so would surgery be necessary since it was an abrupt injury?

Some people with labrum tears need surgery. Many don't. You need an exam to see how much adhesion you have and how much potential for strengthening around it there is.

I tore my labrum 3 years ago. Torn 270 degrees with 3 labrial cysts. I have been weight lifting my whole life and even with the pt exercises it's still not 100%. It doesn't slip out, it's strong, I can do almost anything with it. The only difference between the one that tore and my good shoulder is the pain. It just hurts after a set and feels like it needs a long warm up. Is there somthing I can do? The pain isn't terrible but when I move it, it sounds like Rice Krispies, and the sound I hear swimming underwater is horrific.

I have been diagnosed with superior labral tear extending upto equator. Its been 14 months im having shoulder pain and unable to bowl while playing cricket. Doctors here recommend arthroscopic surgery. Pls suggest.

I'd try getting adhesion removed over 5 treatments. If movement makes it worse, that's a good sign it'll get better. If pain is constant and it doesn't matter what you do, that's a good sign you just need surgery.

@@stufigol We don't have any Integrative Diagnosis docs in CA. The closest is probabaly CO if the pain is bad enough to travel. Dr. Scott Mills is in San Fran, but I don't know the extent of the results he gets. Good dude though.

I usually don't ask but here it goes.. I saw a pt and was told he suspects its a labrum issue. I felt it during a dealift workout. What's weird is that I have NO pain doing pressing motions like chest Dumbell press but do have pain when I squeeze or flex my biceps or do pull excersizes.. is this normal?

is there anyway stability gets better? i'm 44 and fell on mine yesterday, heard a click sound. nothing showed on xray...awaiting mri. the worst part is almost any movements can make it feel like its out of the socket...even letting my arm hang and letting it move in circles. i'm keeping it in a sling as much as possible and icing,

MRI is 70% sensitive for a labrum tear. Arthrograms are 90%. If you want a better chance of picking up a labrum tear, get the arthrogram.

Dr. Ive been having a strange pain in my right shoulder. I never injured it although i do hit the gym hard. I suspect its a slap tear. It just started happening i cant pinpoint when but i believe it was last year around november. Its somewhere in the back of my shoulder. I can hear clicking and cracking sounds to. Sometimes i can feel it all around.

I am not a doctor, but I have surgery scheduled for March 16. The symptoms you are describing are very similar to mine, I developed a SLAP tear over several years of heavy bench pressing. As you say, I cannot pinpoint an exact starting date, because it was a gradual process and the pain started mild. After that, I stopped working out cold turkey for 1 month, which made everything worse due to muscle loss. My doctor says that the clicking and cracking is not necessarily directly the SLAP lesion, but it will trigger it someway. My advice is to take an MRI to see if the labrum is torn, and if it is and you are a young person go ahead and take surgery. PT will work to some degree but if you hit a sport hard you shoulder will never be more than 70% (at least in my case). However, seek professional advice because there is a lot of issues inside (bursitis, bicep tendonitis, impingement, rotator cuff tears) that have other solutions. I would appreciate if you subbed to my channel, I will be doing videos after 03/16 (surgery date) narrating my experience post-surgery =).

@barefootrehab thank you in advance. I work manual labor this is were it occurred. Mr dr has went the conservative route with physical therapy my range of motion has improved from incident date. Over head throwing and swinging with velocity still give pain and my strength had yet to come. I'm male in my lower 40s. Do you think my tear will heal.

Yeah I have a SLAP tear with a rather large paralabral cyst. I've had a lot posterior instability and pain over the last 6 weeks.

@@SureFeelsGood I ended up getting the surgery in September. Turns out my slap tear was massive which was causing my instability. They repaired the tear and performed a bicep tenodesis. Shoulder is now solid as a rock but not quite back to 100%

Why not? Joint degeneration or tears characteristically shoot pain in specific patterns, this is one of them. Or it's an impingement with the joint not sitting in socket properly.

I was recommended for surgery. My MR arthrogram showed a posterior tear. I have been having instability issues for years. I got opinions from 3 different doctors. Would it be worth seeing you, you think? Surgery recovery seems awful, 6 months before getting back to full activity

If it was me, I would see us. Adhesion causes a lot of pain that can prevent surgery. But we would know that at the exam. Sometimes we do the exam and send people straight to the surgeon. I have been having pain for a while now. Probably around a year ago and I play softball and i am a catcher so I think it’s from overuse of throwing. It hurts when I throw and I feel like I have no control over my throws. My shoulder also sometimes shifts out of place. but after resting it didn’t work I got an MRI with contrast which shows irritation which could be a tear but they don’t know 100%. I’ve been to many doctors and I’ve done physical therapy for 4 month 2 doctors said it’s most likely a slap tear and I need surgery or try PRP injection. I decided on surgery and had a consultation with the surgeon but he thinks I should try 6 more weeks of Physical therapy and work on getting flexibility in my internal rotation. It’s 2 weeks into my new physical therapy it doesn’t feel any better and he and my old physical therapist both think it’s a slap tear. I just want to get surgery soon because I start college softball next spring and I need to recover fast. Do you think surgery is the best option?

I'd do adhesion release first since we save many people from surgery. If you're not better in 3-5 treatments, then you'll need surgery.

It's ok no apologies needed, I'm looking for info about this. I had an MRI done recently and that was my diagnosis (Slap type || Tear).. and I'm leaning towards surgery. Thank You for responding. And for much appreciated info. I did watch the whole video till the end.👍
