I think Mario being fat is more of a running gag than anything. Him and SMG4 are the Fat Italians after all. He's probably getting plenty of exercise from all the running and jumping, and he's nowhere near as obese as Wario or Dr Eggman. Putting him on a diet is a funny idea though, and I love how much Meggy cares for Mario, even though he's sometimes too childish, stubborn, and dumb to realize it. I also love the animation and voice acting here.

Honestly I think this might be true in Mario canon, he eats unhealthy but gets enough exercise to not be like Wario, only thinking about this cuz of Luigi's Mansion 3 where Mario's room has a lot of pizza

Although Italian food can be healthy too if you take it easy with the meat and cheese, eating meatball pasta or pizza every day can't be good for you in the long run. But Mario still gets all the exercise he needs just from being the hero of his games, where as Wario is just a lazy jerk.
