I retired from a dual-diagnosis program in 2001. I was sitting around and began to think about the days I had to manage a program and write clinical notes too. I was reflecting on how overwhelmed I felt at times. I found your channel and found it to be so refreshing to watch. I hope others will come to your channel because there is so much to learn here. the examples such as the tired client is just too pat and easy. Clients often do not present so neatly for you to write a SOAP note. Try to give us a real example of a real session not one that is just made up to make a point about SOAP notes.

I agree! Trying to do one for a trauma client and having difficulty translating into this form.

For the purpose of teaching and learning, it's easier to use something as mundane as "client was sleepy." this is a foundation one can build on. That's the art of a good teacher.
