Could you tell me why eveytime when somebody missed you, they received prone?

Hi Holy lance helmet does 1d4 radiant damage if they miss and fail the save when ever we deal radiant/thunder lighting damage we inflict reverberation also with our armor and rings when we deal radiant damage we also apply radiating orb and from our boots when we inflict a condition on enemies we also inflict another stack of reverb(2) when enemies get 5 stacks of reverberation they have to do saving throw or go prone 😅
