What type of surgery is best for a patient struggling with facet joint issues or PARS a fracture. I'm assuming it's most likely a fusion but wondering can an ALIF fusion work for facet joints issues or do you have to have a posterior fusion. I can understand a posterior fusion maybe more viable for a pars defect, but would like your thoughts? I asking for ALIF because i think it less destructive in comparison to posterior surgeries.

dr.diuglas I have spinal stenosis and it will mean the world to me if you can look at my MRI, I have been in pain management for many years and had my first spine surgery last January 9th, 2020, I am in so much pain I can't sleep, I have hip stenosis tendonitis and fibromyalgia as well, I need some honest advice. Please help me.

Hi Brenda. I have FM too. Found in my mind all like us have a sacral Tarlov cyst. If you look at the imaging studies? Never seen on your MRI imaging? You wont because doctors don't know the proper MRI technique to detect it. Go get a sacral EMG . Get a bilateral sacral nerveblock every quarter. If timed right you'll start to get your life back?One way to self diagnose. #1way to aggravate the cyst is reaching down into the sink doing the dishes. If ths makes sense? Probably got it. Dont fret. It's more treatable than FM. You going to get from away the cascade of pain your feeling. Jordan

What is your reference for this statement? According to Bogduk, who is the undisputed number one authority in the lumbar spine in the world, annular tear within the intervertebral disc is the number one cause of chronic low back pain. Zygapophyseal joint dysfunction this second place, followed by sacroiliac joint dysfunction.
