Can you please show us exactly what you would write into the boxes for the medications that were not given. For example, would you only write "6" next to the prescription and then elaborate in the last box of the chart for omitted medications or do you have to sign by the number 6 next to the prescription? Also, in the last box, would you write "6. Wrongly prescribed medication." or would you say something more specific like "Medication already prescribed and given at 1300"?
