Essential parts of a Medication Chart

Want to know what MUST be present on a medication order to make it a legal order?
Full name of the person
Date and time the order is written
Name of the drug to be administered (generic preferred over brand name)
Dosage of the drug
Frequency of administration
Route of administration
Signature of the person writing the order
If any of these essential parts of a drug order is NOT present or written incorrectly (including unable to be clearly read), then the medication chart is NOT legal and can NOT be used.
