For pedia if convulsions present with hypoglycemia then phenytoin 15mg/kg wt with 2ml NS with slow IV and if no convulsions and if breastfeeding baby with hypoglycemia then encourage for breastfeeding

Hi, 1. First diazepam Iv and then if not controlled , we use phenytoin. phenytoin itself takes 20 minutes for action after starting the loading dose.. So patient would have crossed completed 30 minutes - refractory (as per your statement) Should we start aphenobarbitone or shift to ICU ? 2. What is the rate of infusion for phenobarbitone line ? Is it 50m/kg?

I was given 12 mg of ativan/lorazapam in 2mg doses when I was in status epilecticus. When that didn't stop seizures I was given 1500 mg of IV Keppra and a phenobarbital which eventually worked to stop them. Guess it must depend on the hospital 🤷‍♀️
