Wouldn't myocarditis also be a STEMI equivalent? I watched videos from Amal Mattu's ECG course before all those videos went down and he had a few videos about patients that looked like they had a STEMI on the 12-lead but it was actually myocarditis and he mentioned that myocarditis can imitate an MI to the point that you can't tell them apart via 12-lead alone as well as the typical EKG findings for myocarditis.

Caters Carrots hello and thank you for the great question. I believe you are confusing STEMI Equivalents with STE-Mimics. A STEMI Equivalent doesn’t look like a STEMI on an ECG, but the patient is still experiencing ACS. The STE-Mimic is an ECG pattern that looks like a STEMI, but the patient isn’t experiencing ACS. Pericarditis is a STE-Mimic. I hope that clarifies things. And keep watching Mattu’s videos. He’s a master.

Can I give the patient with STEMI equivalent ECG thrombolytic therapy if I am not in a PCI capable center???????

Do you have a local cath lab equipped hospital? How do you manage STEMI patients, do you thrombolyse them or do you send them to a different hospital? STEMI equivalents are not inferior to STEMI in terms of importance, these are MI secondary to full arteries blockage which does not reveal themselves as ST elevation on ECG
