Hi. Love your content. Quick questions about class II restorations, is it somewhat acceptable to discharge patients with slight molar-premolar (tooth 25 and 26) open contact on restorations with class II open sandwich technique on both mentioned teeth?? Also, with the open sandwich technique, if there are maybe small surface imperfections (e.g. holes/bumps the size of dental explorer tips), then what can be done instead of removing the entire restoration? I understand neither are ideal situations but as a student, I realize I've left these situations and discharged patients which is kinda haunting to think about as you learn more...

Thanks for such a real world question, Jiveshvil! Open sandwich or no sandwich (making me hungry...) open contact is not desirable. Is it the end of the world? No. In the wrong patient might it result in perio issues and recurrent caries? Yes it could over time. Monitor very closely and make sure OH is spot on. If patient is struggling then redo one restoration and nail that contact! With regards to porosities - they will collect stain and food packing - somtimes with a very thin tip of flowable composite and high magnification you can fill in some voids...other times it's a remake and you learn a lesson and sleep better at night :)

@@protrusive thanks for quick reply as always :) I'm taking about removing interdental pappilla? We don't have thermacut bur so in that case can we do it with round diamond or carbide bur? And how to stop bleeding after that , what's best practical method ?

You don't need to buy any special "BURS" just remove the diamond particle from any diamond u have using an abrasive disc. Thermacut burs basically are a regular bur without diamond particles. @@naeemandalmasdentalclinics

@@naeemandalmasdentalclinics that's quite risky for iatrogenic damage. consider a soft tissue ceramic bur if not thermacut. OR get a round diamond bur and drill off the diamonds using another diamond bur to make your own thermacut! When you use something like a thermacut, it has like a cauterising effect so bleeding stops quickly. If needed can also put viscostat clear on a cotton pellet and apply with pressure on the bleeding site
