I'm doing my trauma rounds right now for ER and this was a very well made video. I would add that if the bleeding persists despite having a tourniquet on, add one on top, DO NOT remove the existing one and replace it. And if you're worried about losing circulation to that limb, remember it is life over limb in emergencies, better to lose an arm then your life.

A 5 year old girl here in Thailand today or yesterday was shot above her left knee, bullet took out the femoral artery in both legs. Happened when 3 young children were playing and found a loaded gun :( She died. Nobody knew that it was important to stop the bleeding. Just phoned the police. When medical care people arrived, the girl was just lying "sleeping" on the floor in a big puddle. Not my child, but still crying!

@@MCTheTrash Art and music are important too, humans need to be creative. Even understanding religions is important but probably the least of all four. The real problem is that kids are being taught poorly and so they complain about everything, like this guy. School isn't meant to simply get you a job and teach you basic life skills, it's supposed to help you develop your intelligence and then give you the knowledge required to go to further education if you go there. So for working class people they might think it's a waste of time, because they didnt go to uni, but they are mistaken. A kid who doesn't learn all this stuff would probably be borderline intellectually disabled lol. Our brains need to be exercised like our muscles. If we don't use it while it's growing it's not going to grow very well.

I hate to play stump the chump, but some of that information was incorrect. Tourniquet comes after pressure and packing, not before packing. The reason for this is, if you already have a tourniquet on there is no need to pack or provide additional pressure, because the tourniquet is already cutting off circulation, therefore bleeding has stopped. Also, when it comes to tourniquets, you tighten the windless until the bleeding has stopped, not just until you feel that it is tight enough. Bottom line up front, pain is less important than saving someone’s life.

I think the comment about “getting the tourniquet as tight as you can” is because people using this video are probably amateurs. In a high stress situation it’s hard for your average joe to judge bloodflow, but it’s easy for them to just tighten until they can’t anymore

The argument can be made that depending on the situation, you can jump straight to the tq. If you can identify that the bleeding is indeed arterial, you are allowed to jump straight to the tq. Especially when there is multiple people or if you are in a dangerous situation. Every conflict is different.

i wish she'd mentioned this but if you don't have a tourniquet at hand, you can just use a piece of cloth like a t-shirt and then use a pair of scissors or a stick to tighten it and apply pressure/tension.

In an emergency don't worry about if there is no clean cloth use whatever is at hand .The wound will be cleaned and antibiotics given at the hospital. Just stop the bleeding.

With a major bleed that requires a tourniquet, if circulation is not completely cut off, the body will respond by increasing blood pressure to resume circulation. This can make an improperly tightened tourniquet result in worsening bleeds. Write the time on the tourniquet, very important, as the patient has 4 hours until those potential problems result in a likely amputation. But amputation is preferable over loss of life due to blood loss Emt's specifically recommend that you should not be able to feel a pulse

Joe Schmo (myself included to an extent) on the internet giving stop the bleed advice is a slippery slope, however, you are right about the pulse bit - checking for pulse is something that should be done at each stage of initial assessment, during and after care.

Sometimes a tourniquet is unable to fully stop the bleeding, only slow it. If that's the case, you either use a second tourniquet in addition to the first, or pack the wound and apply pressure to try to finish stopping the bleeding.

This issue is the risk of crush syndrome. When a muscle is crushed for longer than 15 minutes the body releases a toxic enzyme which is difficult to remove and poisons the kidneys which can result in the casualty dying.

Why would you pack a wound? If bleeding but tour not required. Use tourniquet if spurting blood; put 2-4 inches above wound, no packing, 0-2 hours leave it alone & doc takes it off. Buy quick clot, bleedstop, woundseal, gauze, tape, and tour kit and carry it with you. Use quickclot on neck w/ pressure.

Not saying that you are wrong and it won’t save a life, but I disagree with putting a TQ above a joint. The CoTCCC, Red Cross, and tons of EMTs state that you need to put the TQ as low as you can in order to save as much of the limb as possible, the CoTCCC states that you need to put the TQ proximal to the wound (about 2-3 inches above the wound), if you don’t know where the bleeding is however, CoTCCC states to put the TQ “high and tight,” but you do not have to place it above a joint if you don’t have to (NEVER place on a joint though). Additionally, CoTCCC and Red Cross teaches to tighten TQ until the bleeding stops, which might be as tight as you can get it, but you want to be very careful that you don’t break your TQ because you tightened it too tight, cause then it wouldn’t be a very fun day after that.

Wesland Kid with TQ placement, it also depends on your skill level and the environment you are in. If you have trained with a TQ once or twice, then you might not be able quickly isolate arterial bleeding and place 2” above it, so High and Tight would be best for them. Also, if you are 10-20 minutes away from a hospital (even farther than that), then High and Tight could rarely hurt unless there is some wound up high. Sure, placing on a two-bone compartment is always better, but as long as you stop the bleeding then that’s really all you have to worry about. And yeah, it would S U C K if you overtightened and the windlass or buckle snapped, that would be a bad day.

The way we learned it in the military is high and tight on clothing is for phase 1, where you're still engaged and you just gotta quickly stop the bleeding, and 10 cm/2-3 inches above the injury is second phase on skin-contact. The reason, as it was explained to us, for putting it 10 centimeter above is so as little of the leg as possible is lost should it need to sit there for so long that the leg needs to be amputated.

Another consideration for "high and tight" would be a gunshot wound on a limb. Bullets can fragment and those that come in at an angle could have caused an internal fragment to be 2 to 3 inches above the wound. If you put your tourniquet 2 to 3 inches above the wound and compress over an unseen bullet fragment, you could potentially do more damage to internal vessels. So for a knife cut on a limb, go 2 to 3 inches above (not on a joint) and for gunshots or multiple casualty (where you don't have time to identify the exact location of the bleed) just go high and tight.

If the tournique has been placed properly, you can move the person taking care not to mess with the tournique and always checking it after having moved them. Issue the can occour if the tournique has been on for more than 2 hours, the limb begins rapidly dying and releasing a toxic chemical that'll put the patient into shock and kill them. So basicly, if you're going to move them do it immediately after having applied the tournique. The longer it's been on, the less you should move them on the risk of knocking it loose.

I don't know actually what medical rules in US says about that, but in Russia you must loose the tourniquet a small bit for half-one minute every 1 hour when patient in warm place (summer) and every 0,5 hour (30 minutes), when patient in cold place (winter).

Tourniquet should be the last resort. I heard that it should be loosen every so many minutes (for just seconds I guess) but maybe that depends on how severe the bleeding. I guess it is to reduce the chance of loosing the limb as us the risk with these things but if it is life threatening, you may have to make the tradeoff.
