HCC with normal afp ( in up to 20% cases ) should have PIVKA ( peptide antagonist of vit K2 ) checked. BCLC is usually followed in the western countries . In Asia several experienced centers have their own guidelines ie Korean, Hong Kong, India, Japan and China wherein surgery is also an option in resectable and operable cases for the Intermediate stage BCLC (B). Since LTx and resection are the only potentially curative treatments followed by local ablative therapies ( heat, chemo or radiation) local treatments may be also use in the neo-adj setting prior to surgery. Immunotherapy is available for the advanced and metastatic stage or as an adjunct to other modalities to downstage locally advanced tumors so as to make them eligible for curative treatments ( LTX or resection).
