My favorite secret death was in portal 2 beta where at the beginning of the game if you portaled to the moon you would die. (It was later removed and then moved and edited for the ending)

The tombstones across places in Albion in the Fable games, contain humorous messages that often contain references to films and video games in popular culture. "It's too dangerous to go out alone. Take this." - If you dig on this grave, you get a toy sword, in reference to The Legend of Zelda. "In Loving memory of Arnold. He wont be back." - This is a reference to The Terminator. There's also an ingame reference that supported the widespread rumour that there was a mythical creature to be found somewhere hidden in Albion, and for many years it kept gamers searching for it and posting their theories. No one has ever found this creaturep, but...there is a grave. Its tombstone reads: "Here lies the Sandgoose. Loved by all. Seen by none."

Okay, that last secret death was awesome. As far as Fallout 3 goes.... Gibson's note to search the house is for the player to search the small model of a house, inside an actual house. What you find inside isn't awesome or anything like that, but it's still a fun discovery.

This isn't really a SUPER secret death, but it sure is a secret: In the Legend of Zelda: Ocarina of Time, after Ganondorf takes over Hyrule, you can go to the back alley in Hyrule Castle Town and speak to a soldier sitting on the ground. Once he finishes his dialogue, Navi informs the player that he's not moving anymore.

in monkey island 1, there is only one possible way to die. when you're underwater and tied to the idol, do nothing for ten minutes. Then, later in monkey island 3, on blood island, if you wade into the lake 50 times in a row, he'll say fine, whatever, and finally go in, and then walk inyo the scene from monkey island one with the corpse of himself floating tied to the idol

Not sure if these count as death easter eggs, but there are two things I can think of from The Talos Principle. Both are in the snow level (one of the levels in World C.) If you walk along the edge and look down, you can find a Gnaar from Serious Sam hanging from the ledge. A voice will say "Fly, you fools" and the Gnaar will fall, referencing Gandalf's death scene. The other is a secret cave you can get to by jumping down a few ledges which leads to Sam frozen in carbonite like Han Solo.
