Good stuff Dr.! Getting 4th nerve palsy surgery next month. Pretty much what you said. My left eye wanders up when looking up and/or right.

@jimmyk.shorts4631it was really good for 3 weeks but slowly came back. Not as bad as before but I feel like I need a 20% tuneup. I talked to my doctor and he said to wait at least 6 months before doing another surgery. I’m 5 months post op and It hasn’t gotten better so I will go through with a second surgery. My advice is to not get too happy for 1 month. Then you can celebrate. I’d say my eyes were a 3/10 before surgery , then about 4 days after surgery, they felt like a 9/10, it was amazing, I could hold a gaze to anyone without feeling like looking away but right now they are 6 or 7/10 , depending on how tired I am. I’m still nervous looking at people so I’m definitely getting a second surgery. Your eyes will be blurry for a few days but the visible clarity comes back so don’t worry like I did.

What about blepharospasm that only occurs when making certain movements: -when smiling and looking up -when closing the eyes and looking up Would it be a type of BEB or could it be secondary blepharospasm? I have never heard of such triggers for BEB. How do you differentiate between BEB and secondary blepharospasm? Thank you, and thank you for your great videos

How would you differentiate between primary and secondary? I have been diagnosed with CN4 palsy but I don't have some of the classic symptoms such as vertical diplopia on downgaze or a significant head tilt. I have horizontal mostly with a smaller amount of vertical diplopia and have some extortion. I have had these as long as I can remember. Do you have any suggestions for me? My doctor talked me into blaming it on an obscure childhood head injury but I'm not convinced. She put me in prisms but I'd prefer surgery.
