In the beginning of this episode Dean walks up to a mother on the playground and it definitely looks like Anna the angel that Castiel likes, maybe before she was used after The Fall. Supernatural is known for using characters from the cast more than once. The fat dude that owns the retro 70s diner and has powers also was a doctor in another episode, pretty sure he drinks from a flask or something.
