Your escapement needs to be adjusted. If you loosen the nut that faces you and slide the pice of metal attached to it to the left in small increments it will come into adjustment you will know when you have it right when it dosnt chatter or lift the bobbin case up and down :)

Your hook timing is too advanced. The needle should be rising as the hook passes.
