It's not a PRO. It's to do with theta roles. (It's in one of his previous videos... let's see if I can find it: kzbin.info/www/bejne/bYmah2utpc-Ufas I think that's the one.) Basically, a verb's theta roles can only be assigned to something directly before or directly after the verb. This means that if in 7:49 "hurt" wants to give "John" a theta role, "John" has to appear next to "hurt". Only after it gets its theta role does it move up to the SpecTP, as you can see at 7:49.

Isn't "John" actually the complement N of the DP (even though the D is null)? So, when we write down the DP without using the triangle shortcut, it doesn't have a sister. How can it c-command anything?

No. John would be in the Spec of DP, not an NP complement to DP.
