As promised, following on from my t:slim Insulin Pump Review Video, an in depth look at the Control iQ software. The pros, the cons, and my personal experience with it.
Please know, this is only my perspective on the system. All diabetics have a unique and varied set of requirements depending on what they want and need out out their device, so I can’t stress enough how important it is to do your own research in finding the right system for your needs. And I really hope this is a helpful resource in that quest!

Awesome review Andrea! 100% agree with everything you said. Just two notes to add from my experience: 1) The Control IQ lets me run bit higher than I would want when not in sleep mode. Usually around 6.5-7.0 (120-130). I'd prefer to be 5.5-6.0 (100-110). It resulted in a better time in range for me (improvement from high 70s to 90s) at the expense of a slightly higher HbA1c (from 6.0% to 6.3ish%). Some people only run the control IQ at night and stay in "manual mode" during the day having more "human control" and being able to run lower. It requires a lot of efforts though and it's not worth the hustle for me. 2) The signal loss is real when I have the pump on the other side of my body than the Dexcom G6. In your case it probably doesn't do because you are very fit and the signal can probably "make it though your body". I have more fat on me so it doesn't always work and sometimes it deactivates the control IQ mode. I think this is directly linked to a body shape of a person:)

Thank you so much!! For your kindness and for sharing your thoughts that I absolutely and utterly 10000% agree with!! I sometimes wonder if running the sleep mode all the time would help reach those lower A1C numbers 😆 and the loss of connection…SUCH a real issue and totally strange and unacceptable to me!! Hopefully they change this with the G7 and with future updates of the Tslim software even maybe!! Thank you again!! You are so awesome!!!

I'm so thankful that my daughter was diagnosed at this point in technology! She recieved this pump as her first pump and it has made life soooo nice! She only went about 6 months as a Type 1 diabetic without a pump.

Hi my name is Ariana, my son just been diagnosed 3 months ago. He got this pump almost a month ago. I haven’t connected him yet. Because I’m scared. Is it scary for a child or you feel confident? My son is three.

very lucky to have this technology. 30 years ago when I was diagnosed, I was the "oddball"...as I grew up into highschool more people I knew were getting Type 1...my first endocrinologist used to have to pee in a pot, boil it and get her sugar readings that way...so a lot has changed...I will say tho, I was expecting the "continuous loop" system to be available WAYYYY before this and hte mini med came out...not sure why it took them so long to come up with a system like this...good luck to you and your daughter!

Good stuff. My son is 13. diagnosed 3 years ago and we have been strictly on injections and using dexcom. We just ordered our tslim x2 and are waiting for it to arrive and we will be going on a pump for the first time. Nervous about all the new stuff we need to learn, but hopeful about being able to sleep better at night while also having good control. I've been watching your videos for a while and really appreciate all you do. thanks.

My partner has T1 diabetes and just started their first pump (t:slim w/ Control iQ) about a month ago after about 20 years on MDI. It has seriously been life-changing. There were many days when their blood sugar was less than 20% in range, high A1Cs, etc. and since they have been on the pump, we're talking consistently above 70% in range. I am so grateful that they have access to this technology, and I am so grateful to you for all of the wonderful advice you share. It has helped me immensely to be a supportive partner to someone with this condition.

Hi Andrea and everyone. What an excellent vlog. I've just started on the medtronic 780g with the the G3 sensor. I totally agree with your comments in relation to improved sleep patterns and the available exercise programme in helping to avoid crashing hypos which is great for us. Things can be easily tweaked and changed because of the support and training.The only difference I see is that with my pump I can suspend my pump. The costumer service is there for me as well and is absolutely great. The algorithm for these pumps has made a massive difference in terms of meeting my individual needs. I love it and I would push everyone in our community to embrace this technology and use the support network if they get the chance. This coming from a total technophobe but this technology has made such a difference. I feel less stressed but more importantly I feel well. Keep up the good work and everyone keep well.

So glad to see you back Andrea. I was diagnosed 7 weeks ago at the age of 33 and you were the first KZbinr I found that spoke openly and honestly about the condition! Hope your are enjoying the new space.

Keep your head up Kevin! Diabetes can play with your mind as much as it does your body. Depressions? Possible. Miserable at times? More likely. Just brush it off and get back up because there is always someone more challenged than you and always believe "this is a ground ball, I can do this" - Be well

I was diagnosed 28 years ago with T1D and have pumps since 2001. So I'm now about 20 year on pump therapy and the thechnology evoluates amazing. I'm currently on the Medtronic 640G but have to switch to a new pump. The dexcom G6 is not yet in the insulin pump convention of Belgium, but in July that will be change and then I will move to the Tandem Control IQ technology with the Dexcom G6. I have now time in range mostly aroud 82% and a hemoglobine A1C around 6. I hope that I can improve that with the new system and have less to care all the time by my self. So thank you for this review. I had to choose between this pump or the Medtronic 780G and I believe this one is the best choice for my situation. So a very big change that in july my 5th pump wil the Tandem X1 with control IQ.

Thankyou so much for that re-assurance Andrea. I expect to get Control IQ later this year and am currently using Basal IQ. So I am really looking forward to the next upgrade.

I still think of my middle school nurse often. Haven’t seen her in over five years but her compassion and help while navigating a diagnosis at 13 still warms my heart. You guys have more impact on us diabetics than you’ll ever know!

I started the tandem t-slim iq pump 6 weeks ago. I’m still in the learning process and making a some tweaks with my dr’s assistance. I hope that I can maintain an A1C of 5.1-5.3 as I have been doing manual injections. I do find myself intervening when my numbers are not where I want them, my dr doesn’t like me doing this, but I tell him and her that I want to live to be 100 years old, I’ve deem diabetic for 30 years! Thanks to you Andrea, I decided to give the T-slim a try! Hope all is well, see you next time!👍🏻

In your cons section you mention that you can't suspend insulin while in Control IQ. Maybe its a software version thing but I got my pump a month ago and I'm able to stop delivery while in Control IQ, it's the first option in the options menu in fact. My trainer actually told me to ensure that I did suspend whenever I had the pump disconnected so it could properly track the fact that I was not receiving insulin for that time period. Anyway, just want to say that I love your videos and and they have been so helpful for me. Since getting my T:Slim my time in range has gone from 50 - 60% on my 630g to over 90% with the Tandem. I'm so amazed with this pump. LOVE IT! Still trying for a 100% day, the best I've been able to get so far is 99% in range.

99% is PRETTY DARN AMAZING!!!!!! You are right about the ability to suspend - I should have been more clear in my delivery - basically I meant you couldn't temporarily suspend, rather you would have to stop insulin completely and then manually start it up again, as opposed to putting a timer on the suspension - if that makes sense. Still, such an amazing pump - I totally agree - and am so glad you're loving it too!!!! And getting such incredible results!!! WELL DONE!!!

@@ShesDiabetic Sorry for replying to such an old comment, but for anyone wondering there is a way to get this done. While this would require temporarily turning off CIQ, you can actually go into Options>Activity>Temp Rate, and program a temp rate of 0% for X amount of hours. You'll have to do manual correction boluses if you opt to do this, but you wont have to worry about getting basal deliveries for the time frame you selected. Once your temp rate ends or you opt to end it prematurely, just turn CIQ back on to resume.

I just switched from the 670g to Tandem and Dexcom. It has been amazing and so much better than my previous system. Wish I would have known about this sooner. Thanks Andrea for the amazing reviews and support for diabetics. I’ve been Type 1 for 40 years now and have seen lots of changes in treatment plans.

@@benwahhhh I have the 670G. But not the Tslim yet. In another 3 months I will switch. Main problems with the 670G are 1. Inaccurate CGMS. The 670G aims for 120. But your real blood sugar according to the meter is 170. So it is always trying to keep you at the wrong target. It thinks you are at 120, but your blood sugar is really 170. Issue 2. The 670G is not aggressive enough. It just does not give me enough insulin to bring my blood sugar down. Its aggressive basal doses are not enough. It allows blood sugar to climb too high and just does not give enough basal quick enough to keep blood sugar at 120. Its incorrect CGMS says I am at 150 and arrows going up. But it will not give me enough basal to even bring it down to 120. And then add in issue 1 of my real blood sugar being 50 points higher anyway. The 670G just wants to keep your blood sugar in the high 100s. 170 to 199.

It s better than the gl3 but the 780g is the same as the 670 and I totally agree with you, it s not aggressive enough where going high or staying high... The Cgm is not accurate I sense the hypo or hyper way before the pump it s not just the lag of the interstitial vs direct blood method. Anyhow I ve never been at such level of hba1c or so much "in range"... 8.2 to 6.7 and around 80% with only a few hypo/big hyper in a month compared to 3 hypo per day before So let s hope the algorithm improve and that s it...

Thanks Andrea... I've been using the Minimed 780G since few months which has similar software. This is my 4th insulin pump, the first commercial product claiming to be closed loop, and playing catch up on several open source solutions. The software algorithm looks similar the medtronic. I can set my target a bit lower: 5.5, but it only reaches it during the night. Sleep comfort is indeed the most important effect I noticed: I now have enough confidence that I block all alerts during the night. My personal target range is 5-8.5 (I'm not happy at 10). The 780G system is rather slow reacting to highs, so it can take a while to drop down to below 8.5. In automatic mode there is no way to override and give a higher bolus, which is a problem if you want to take a warm long bath: no (official) possibility to bolus before connecting. Solution is to simulate a "fill cannula". Contrary to your pump, while in automatic mode it is possible to stop delivery. But it's not possible to specify a time limit. No temporary basal rates too. The algorithms typically learn from previous days to adapt the basal rates - but they don't have integrated yet the notion of weekends /workdays. I go hiking in the mountains during weekends, so basal rates requirement are very different. Typically that does not pose problems during the hiking, but during weekdays, the basal rates are typically too low to reach my 5-8.5 target range. In conclusion: Yes, good progress has been made, but T1D is complex and very personal, and the current algo's should allow a lot more personalization. I see a lot of difficulties with regulatory bodies to fully open the control software to allow the patient full programmatic control, yet I strongly believe that this will be the only way to have a perfect automated control. Someone started offering a full featured build it yourself insulin pump for $50 details here: kzbin.info/www/bejne/aXPYo4lnjNJjq68 (including dexcon based open source closed loop system). He also showed how to replace the batteries for the dexcom. Since 2 years, he has gone silent though . I's a big $$$ market, and offering a near free alternative would make a lot of ennemies. I hope he's ok.

I've been diagnosed in 2009 and just now considering getting a pump because my reading looks like a roller coaster. My sister use the dexcom CGS and absolutely love it. Based on her review, I'm looking for the t:slim X2 because it works with the Dexcom system. Your reviews of the pump and software are so helpfull and full of tips and advises. I'll definitely ask my doctor for that pump next week. (Sorry for any typo and stuff, english is my third language) Thank you so much!

Great video series, I moved from using Loop/Omnipod in Jan 2021 to Tandem tslim x2 with control IQ. You can suspend insulin by going into the tslim x2 pump menu > options > stop insulin. The pump will alert of if you don't resume insulin within about 10 minutes. Also you can reconnect at least on the iphone by going into settings > bluetooth > and tapping the tslim x2 option to reconnect.

Thanks for your sharing and skills of clear communication! Type 1 for 47 years. Nighttime, while asleep, lows were far to common while injecting with multiple shots per day. My wife would at times need to break out the glucagon, or call 911, to “bring me back”. Not one time since Tandem Tx2 using Control IQ, have I been that low at anytime of night. I agree my within range has become almost non-diabetic levels while sleeping. Don’t care for things hanging out of my body, however I would double the clutter for the blessing of my wife not having to put up with the seldom, but annoying fish flop from me. ;)

Wonderful video packed with great information. One con of control IQ for me is the lower number of the basal setting range. I would prefer the option to set it lower than 120. Using basal IQ I have it set at 85, and have been 95% - 99% in range since I began using the Tandem. Perhaps one day in the future that will become an option for control IQ as well. Now in my 44th year as a T1, this pump has been such a game changer, totally love it!

Been a T1D for 29 years. I have the t:slim control IQ along with the Dexcom G6. I decided to get off the Control IQ the other day. I’m use to insulin pens. A lot easier for me. With using the insulin to carb ratio. And my time in range is always over 88%. I may go back to it (Tandem) but not rn. Great video, very detailed. Ha1c is 5.3 btw

Sometimes it’s not the latest and greatest technology that’s the answer for all, and that’s A-OK! Thanks for the video. Nervous but excited to start my NHS funded tslim in June. After watching your vids I decided on the tslim and also decided to self fund dexcom to get the most out of the pump. I have already started wearing the dexcom and wouldn’t be without it. Got in to the London marathon 2021 and couldn’t see how I was going to achieve this using pen injections. Hopefully pump and dexcom CGM combination will help.

Oh I'm so so very excited for you Scott!!!! I think you've made the best choice given all the choices on the market currently - just my opinion! I hope you love your tslim and I'm so pleased you love your Dexcom!! Thanks for this, I agree with everything you pointed out in this. I'm new on the t:slim CGM for 4 months now, and it's a real life changer, but i sincerely hope they'll fix the Exercice Mode so it's more accurate. Another annoying spec of the system for me is that you can't change the 6.1mmol objective that goes with Control-IQ. I, for instance, am feeling way better when my rates are on 7-8, and an objective as low as 6 is constantly getting me into hypos. Still, it's an huge improvement and a great tech !

For me, I actually wish I could target a bit lower - just goes to show we're all different and in all respects it would appear they're airing on the side of caution rather, which is understandable - but still! I hope for more patient driven customization in the future as these things develop. Still, like you, I'm grateful - agreed!!

Day one of the pump done. I've lots to learn about insulin to carbohydrate ratio, the control iq algorithm has already brought me to fast and bad hypos today a few times. A lot for me to learn in the next few weeks.

This review is unsurpassed. Thanks for such an informed and detailed review of this system. Next, i talk to my Doc about changing over to the Tandem t:slim as the benefits, as you explained, are within reach of anyone with Type 1. As i have been on a medtronic pump for years, there are some leftover supplies I have which others may benefit from. Any ideas or advice on that? Again, thanks for sharing your detailed knowledge.

Great review of Control IQ! I agree with everything you said. I am also excited about being in range so much more now. I am blown away when I see 90% and above for most of the day. I feel your excitement for the overnight results. I used to get up every night at 2:30 to test my blood sugar. Thank goodness for the Dexcom!

I wish my insurance would cover the dexcom. They cover insulin and insulin pumps pretty well but no glucose sensors and they don't even cover glucose strips. I'd love to have this so much because for me everyday is different, I have no idea how work is going to be so at work I either go supper high or supper low and at night I don't go to bed until it seems like my glucose levels seem to be ok. Hi Andrea, Very interesting video, how wonderful to be in range in the mid to high 90's. It makes me really want to go to the pump, though starting to feel to old for starting to learn all over again. Sounds complicated, and as you said need to have pump calibration settings spot on before going to 'auto" close loop mode. I am seeing my endo later this month, maybe fully closed loop coming soon. I totally understand your concern about learning things all over again, however, I really can't stress enough how doable it truly is!! For sure there's a learning curve that one must contend with - but honestly - it's not that bad at all, and SO WORTH IT!!! I'm going for my second vaccine dose today! As a Control IQ user I agree with the overnight benefit of the system. Love waking up to flat lines around 100 each morning. Lowered my A1C to about 6.5 from over 8. Really helped me get my glucose numbers down to where they should be. Cons - Wish it had a memory to remember that it keeps micro-bolusing me a little too much after lunch. I keep going low every afternoon around 4 without me bolusing at all. So I want it to not try so hard in the afternoon. Other than that Control IQ does an excellent job. I haven't had connection issues, either between my Dexcom or T:Connect app on my phone. Took a month of tuning to get everything right, and is stable now. Remember that delay before insulin kicks in. It matters.

In terms of your afternoon lows, it might be worth looking at what your correction factor is at that time, as the system uses that as the basis for correcting, which may be why it’s over correcting so consistently. Of course this may not all be the case, but I just thought I’d mention it since I had a similar issue when I first started on Control iQ and changing the correction factor really helped!

Exercice mode identical with 670 or 780g... You have to activate it 30min at least before.. It s not yet a perfect mode I think it needs to have data from heartrate oxygen steps etc to have something intelligent enough Also same issues with the connection between sensor and pump through the body... Sometimes at night when rolling on my bed the pump starts vibrating because of a connection loss... (I have gl4 and it s the same as gl3) But gl4 is a very big improvement not to have to calibrate all the time

Yes! Your pump reads in whatever unit of measurement the device is 'locked' to - so since I got mine in the states it reads mg/dl, whereas if I got it in the UK it would read mmol/L - I hope this helps and makes sense!!

Currently on Medtronic 670g, warranty up in September 2022. I am very interested in the Tandem pump but having only ever used Medtronic for 16 years I'm almost afraid to try anything else. The Guardians sensors work very well for me. I know they don't for some people but for me they do. What doesn't work is Auto Mode. Keeps me very high all the time. I'm in the process of paying to upgrade to the 770 in hopes I get to try the 780 before my warranty is up to see if they have improved Auto Mode.

The new Medtronic's are supposed to be more aggressive in bringing up down, as now you can correct down to 5.5 rather than 6.7. The 780 will also have auto boluses for forgotten or miscalculated boluses.

I was on the old school Medtronic (no CGM) and switching to the Tandem is a piece of cake (poor phrase for a diabetic : ) I know). Being new to Tandem I did have personal eye to eye training.

Hi Andrea - Q. I too have Lows during the night and I have tabs by my bed to take when that happens. The tabs don't taste that bad, all considered. But it can be a bit much when you are woken from a sleep. Do you know of any tabs, or other options, to use for middle of the night lows that are mild when taken. It can be like drinking OJ and then trying to go back to sleep - not an easy thing to do.

I totally hear you! I keep juice boxes by my bed, and that works well because they're room temperature and easy to consume even while laying down thanks to the straw! Another option may be glucose gel, which you just have to squeeze out of a tube and put around your gums and it absorbs and it very fast acting! I hope these things help!!

Thank you so much for watching! I think a pump is IDEAL for an active lifestyle and it sounds like you're headed in an excellent direction! The Control iQ aspect does 'predict' oncoming lows and tapers (or sometimes completely shuts off) the basal insulin to avoid or at least blunt the effects of the oncoming low. It's an AWESOME function! You just have to pair it with the Dexcom G6 in order to make this function work! Good luck with starting your pump journey - I'm excited for you!

i am Hyper/Hypo unaware, type 1, does IQ help prevent that low or high? i am in process of swithcing to the tslim with dex due to the unaware ! does change in motivation effect the slim ? my bg lows 30-40s and i hit highs 400 plus for log periods, dex has showed me the time frame

I started using Dexcom G6 and have had 4 sensor issues. And last night once I put on a new sensor, during my sleep it kept beeping every 30 mins and didnt give a reading but showed LOW and when I tested myself I was at 7.8. So I still dont trust the Dexcom G6 and skeptical to try the Tandem IQ since it relies on Dexcom G6.

I’m supposed to get my tslim in the next few weeks. I’ve been on Medtronic for about 15 years but have been using the Dexcom cgm because of accuracy. So looking forward to it. May I ask what kind of diet plan you use? I’ve been type 1 for 42 years. My last A1C was 6.8 but I eat all over the place of that makes sense. Im looking for some sort of structure in this aspect of my life. I have seen tons of changes since I started this journey at 14.

Oh I hope the Tslim switch goes well for you! I love mine so of course I hope you love yours too!! Personally, I eat mainly plant based and try to keep my carbs on the lower side. I find this helps me with my diabetes along with some stomach issues that I’ve experienced over the years. I truly believe there’s no perfect diet, and everyone should eat according to their own preferences but I’ve always thought vegetables must be good, so I try to eat as many of those bad boys as possible! That’s my line of thinking!

Starch is one of the things I can eat because it’s easily digested but horrible for my diabetes. Thank you for the encouragement. I’m hoping I’ll love the tslim also. I personally don’t have any experience with Omnipod, so I can’t speak to it in terms of a comparison, what I can comment is I know a lot of people have had trouble with Omnipod site failures, and this can be discouraging, so perhaps something to keep in mind and speak to your team about. I really love my TSlim and get such good sleep with it keeping me more in range with the Basal iQ - so basically I highly recommend it - but that’s just me!

I have the Apple watch 3 and it displays my Dexcom readings through the Dexcom app on my iPhone. Nice to see at a flick of the wrist. Does the 7 talk directly with the Dexcom transmitter I'm wondering?

@@dwightl5863 No, they have yet to get the apple watch to talk directly to the Dexcom sensor. In talking with my pump trainer I now understand that the sensor will only communicate with two devices at a time so your options are a bit limited (phone + T:Slim and you are done). However Jakub is referring to the fact that the Apple watch 7 MAY have it's own built in light based glucose sensor. Not a lot of information out about it yet but I have seen that the sensor will only be available on "high end" models so expect it to be quite expensive if they do release it.

You guys have got it spot on here. Actually just the other day my fiancée was asking me about this. Basically, in my opinion, Apple MAY come out with something that could predict trends, say, 12 hours in advance, but I really don't believe they'll have any real time glucose measurements from infrared technology alone. Of course - I could be proven wrong! I just don't think it will be as advanced as what's already on the market CGM-wise with no needle being introduced. But we shall see! One thing I can say for sure is that I love the market getting populated with these types of things because it just means more tech for us to have and make use of to hopefully continually improve our control and lives living with Type 1 Diabetes!

@@danielhernandez2942 I switched from Medtronic (old school pump meaning no CGM input) to Tandem. I was in fair control with Medtronic and they also had 9 months of Dexcom history to look at when I started the Tandem. So my Tandem trainer input the settings from Medtronic to the Tandem with a few adjustments. Major improvement in control from that point.

If my Dexcom is off I will calibrate it, but it's it off consistently I will call customer support and have them walk me through what to do/if I need to take it off and send it back to get a replacement (which they are very willing to do more often than not!) If yours is consistently 20-50 points off I would urge you to call Dexcom's customer support and see what's going on because that absolutely should not be the case. I hope this helps!!

I was going to upgrade to control IQ but when I saw the range that it aims for, I hesitated. I’m still using basal IQ. I like to keep my glucose under 100 so I wasn’t thrilled with the range that Tandem decided to select for control IQ. Correct me if I’m wrong but doesn’t control IQ suspend or decrease basal at 112.5?

I also had that concern when starting CIQ! Really wanted to keep that tighter range (3.9-8 mmol), but the regular algorithm was pushing me higher. I just go on sleep mode 24/7 now (besides for longer exercise + running). So glad my NP made that suggestion. Took some setting tweaks, but it worked out!

@@eastonguy62 Sure! 30 and 90 day averages are 5.7 mmol (104 mg/dL). I'm sure you could optimize CIQ for your own personal targets :) Hope it works out!

Wow - this is great info to have! And so interesting about keeping it in sleep mode!! I love that idea! I wish I could have as tight a control as you guys have - but alas! However, I completely hear you - I think basal iQ does end up setting me up a bit higher overall as a result of the algorithm adjustment level being set to 112.5....you know what I may just try out this sleep mode and see where that gets me! Thank you oh so much for sharing this info you guys and for inspiring me to try something different myself!!

Just wondering why you didn’t try the Medtronic 780G as it’s also closed loop and you were with Medtronic already. My 13 year old, after being on MDI for 9 years, has just done a 3 week trial and is sold, so getting her own 780G.

Great question! A few years back I had a really bad experience with the Elite sensors. They were always was off and thus threw my blood sugars into chaos. I'm also a Dexcom girl and will always prioritize a pump that works with my Dexcom as I feel it's the best CGM on the market currently. BUT! That all being said - this is only my opinion and perspective! I know a lot of people LOVE the Medtronic 780G - so I have high hopes that this will be awesome for you and your daughter!!! I'm wishing you guys all the very best!!

I have now to choose between de t:slim control IQ and the 780G. In the hospital they tried both systems last year, also at the douchter off the person responsible for pump therapy. There are several differences. The T:Slim, you define the parameters and can set up several profiles with different parameters for different situations. It can start controlling the situation imediatly after warm up of your first sensor. The 780 G you have much less parameters to define but at first use the pump take a long time to learn this parameters. It is more Artificial Intenigent based. So the pump collect a number of data, learn trends from this data and will take decisions on based off that. For that reason you can make less decisions by yourself because that disturb that learning process. So fot those who known their body quit well and know in that situation I need a bit more like this, the 780G is not the best choice, because he can not do that in the auto mode while in the t:slim that is still possible. The learning process is still something that need to be finetuned based on the information my hospital shared me. When you are in a different situation, the 780G will try to act in what he has learned the last time and it take a long time that then this is not similar to what he have already learned, and back it takes also somet time. The 780G will also act more with microbolluses every 5 minutes based on the situation then. So when your sugar is increased the last 5 minutes, he will give a mricobolus at that time. While the t:slim try to predict what will the value over 30 minutes and calculate a correction based on that value, apart from increased basal. But then it take a long time before a new correction is given to avoid stapling insulin. In my hospital more patiënts where more time in range with the t:slim then with the 780G. The number of patiënts is limited, so too less compared data to take definitive conclusions. Also the dauchter that has moslty bad regulated values is now close to 90% of the time in range. The 780G had with some patiënts better results, but the avearage time in range and values were less good then the t:slim. So it will be different from patiënt to patiënt which of those is the best pump, the 780G seems to be better for those not taking to much care and not like to take own decisions. The t:Slim is better for those patiënts. 780G is also better for poeple who have most of the time a quit regular stable live without to much different situations or the different situations at different parts of the day. Based on this information from the hospital, video's off Andrea and others I think I will decide to go for the tandem t:slim in place off the 780G, but I'm sure that for other poeple the 780G will be a better choice.

With the exercise mode have you experienced how it reacts to different types of exercise? (or anyone else who can answer me) I know for strength training, I only need a 70% temp basal, whereas with cardio I need 30%. Swimming, I need to have no IOB and suspend. For all of these, I need to enact the temp basal an hour minimum beforehand. I'm a little concerned with how CIQ will handle all these different types of exercise with one 'exercise mode'.

I'm a cyclist and do 20 to 60 mile rides regularly. So far the exercise mode has been my least favorite thing about the T:Slim pump, and having said that is still LOVE LOVE LOVE this pump. Right now I'm averaging 91% time in range over the last two weeks. On my 630g I would set a temp rate of around 50% 30 minutes before I started riding and that would pretty much keep me from going low. The T:Slim does not allow for a temp rate while using Control IQ and even starting exercise mode 30 minutes before I ride and ensuring I start out with my blood sugar up in the 140 - 150 range I still drop into the 70s shortly after starting and I have to eat something. After that first snack though it seems to keep me in range the remainder of the ride.

I hear you!! I definitely do need less/more insulin depending on the exercise! Control iQ should in theory accommodate for this, depending on what it sees your blood glucose doing, but I must admit, I still remove my insulin pump for certain exercises and take insulin for others. Just can't really rely completely on the exercise mode if I'm honest. But it can be played around with, just like with the pump and you end up finding your own coping mechanisms I find. Just my personal experience! I hope this helps!

The Tslim x2 in combination with the Dexcom can really really help blunt/eliminate lows! Take it one step at a time and talk to your doctor about maybe changing your goal range of bg's in order to make sure you stay in a safe range that you feel comfortable with!! But seriously - the Tslim x2 is going to help you I really think!!! ❤️❤️❤️

The target range is too high. The Medtronic 780 has a much lower target. I'll be curious to see if the next gen will be lower

First, thank you for your review of the software. My 16 yr old daughter and I both have the Tandem T-slim X2 and use Control IQ. Now my question, when you say "hard unclip" do you mean you literally disconnect yourself from the pump? Why not just stop insulin, top choice when you tap options, and restart when you are ready?

That was my thought exactly in stopping the insulin delivery. Also when stopped, as you suggested, the pump will alarm every so many minutes to remind you you have stopped all insulin deliveries.

Excellent suggestion! (and yes - spot on - I mean literally disconnecting from pump like you described) I'm basically being VERY lazy here, in that I don't like the alarm that it gives you when you've stopped insulin, especially if I'm out with friends or something like that - but you're so right - this would be the sensible thing to do, and I think I just may give it a go moving forward in the appropriate situations because of your comment - so that you so much!!

@@ShesDiabetic oh yes the alarms can be annoying. Especially since you cannot modify the one for inclusions, stopped insulin and low glucose readings (not the you’d want that one changed). The way I see it, I’d rather deal with an annoying alarm every 5 minutes or so rather than loose precious insulin. We already loose 10-20 units priming the tubing depending on length but that is just me. 😉

I don’t understand when you say you cannot suspend the insuline delivery of the the Tandem pump and that you are disconnecting it. I can suspend the insuline delivery as the very first option in the menu of the pump…

You can ‘suspend’ but not temporarily suspend, like I was used to on my Medtronic 640g. To be honest though, I should try to hard suspend more, you’re totally right!!!

For tightest control, you have to tell it two lies: 24/7 sleep mode and the maximum allowed weight of 308 lbs. Hanging out at 150 mg/dL is not my ideal.

Yes, as in you can take insulin without it initiating it. And you can also turn control IQ completely off which will then simply revert back to your pre-programmed basal settings. You’re able to do everything you would without the system as in take boluses , correction doses etc. I hope this helps!

I still doubt it that being in a range between 6.2 and 8.7 mmol/L as standard throughout the day isn't beneficial on the long term? For a healthy person the pancreas keeps the blood sugar almost always perfectly in range around 4.6 mmol/L. When I was misdiagnosed with diabetes I could follow my blood sugars for about four years until the pancreas stopped working and my blood sugars rarely came above 6.0 mmol/L. Have to say I started a strict and pure Ketogenic diet after misdiagnosed. Point making, never above 6.0 mmol/L after eating my meals! Now my pancreas isn't working properly anymore and in the first two days I tried a standard carbohydrate diet and I can tell from that experience alone that it's almost impossible to stay in a normal healthy range, let alone use little insuline. I don't think that these pumps are beneficial on the long term and I even think they're damaging at the long term because of the constant high blood sugar circulating the veins. My philosophy is if you want to be as healthy as possible down the road try living like a healthy person does and keep the blood sugar in normal healthy range as much as possible preferably always! 4.6 mmol/L must always be the target aiming for 🙏🏻

I totally agree! I use pens and a very low carb diet and my A1C is 4.8. My Father died last year (Also a Type 1) from complications caused by his diabetes, including heart disease, amputations, etc.. He kept his A1c between 6.5 and 7 for about 35 years and it killed him. His vascular system was completely destroyed by have high blood sugars. There is only one way to beat Type 1 diabetes and that is with insulin and a very low carb diet. Any other method (for now at least) will end in failure.

Terrible pump IMO hasn’t helped me what they aren’t telling you is it can be hacked and can cause you to have higher blood sugars. My first pump was and now ithe second is doing the same.

This is NOT close to being an artificial pancreas but an automated glucose meter and a motorized syringe, neither of which is very smart. The system is designed to help a PWD whose glucose is in range stay in range more often. It can't prevent hypo and doesn't respond well to hyper. I have experience with Basal IQ and Control IQ. I've gotten better results from bothover MDI and BGM, but not proportional to the $10K/year street price or the added complexity of dealing with the hardware. It was the best availiable in mid 2022, but has a major weakness. Dexcom G6 CGM may be the best available, but CGMs is very imperfect sensing because of the unrelaibility of interstitial sensing. Sites aren't reliable or consistent within one person. Non-invasive CBGMs are needed to fix this defect. I'm able to use it to get the results I want, but with more effort than I was lead to believe would be needed. (No more fingersticks is double-speak that means means no less, not none.) While I'm dissatisfied with how crude and slowly this tech has arrived, I'll use it until something better becomes available as a commercial, supported product, either affordable or covered by insurance. The largest ROI/benefit of this system would be for PWD who have great difficulty maintaining good glucose control, can afford the tech, can operate it oir have a careguver who can and have the ability to make rational decisions when it isn't working well. It can make a BG management better by 0.5% on average and bring out of control BG to closer to control. That reduces total long term cost of health care for all PWD. (But not as much as universally affordable insulin could overnight. ) My blood glucose control during sleep is not better with Control IQ and it's half-measures to bolus for high blood glucose under the very limited situations where it acts, has rarely made a difference in my TIR. It relies too much on carb input values and estimated IOB and a CF that isn't right for high BG, too little to a rate of rise that isn't matched to time of bolus or insulin diffusion. I've often seen no C-IQ reaction bolus for more than 90 minutes after a premeal bolus if I rise above 180. To get excellent control (+90% TIR with no time above 225mg/dL) I often need to manually bolus within 90 minutes after a premeal bolus and do micro-boluses to bring the level down to target. For many people this would be too difficult to do chronically and well. imo Basal modulation for in-range and below-range glucose is the major benefit of Control IQ, and it could completely replace the bolusing for predicted rises over 180 mg/dL if it wasn't for arbitrary zone boundaries and basal limits. The performnce is limited by the algorithm's assumptions.
