I live 31 years with T1D and not experience it like that. As said by others, it is just live for me. You can still do all what you want. Ok, you need to keep it constantly in mind and think whatever you do how I have to mention this with my diabetes but there are much worse deceases to have that result in much more problems then that. We are also lucky that diabetes is a decease that many poeple have (14% of the population) so there is an industry investing in it, a lot of research that come with a lot of good treatments that become better and better. That luxe have poeple with some rare deceases not. So I feel me lucky that I have T1D in place of one of those other deceases.

Hands down, best pump video ive seen so far. Im a RN & work with grade K through 4th grade children. Ive been comparing use of OP5 & Tslim on the children that I service. I feel like both companies should colab on a new shared hybrid pump, talking the best of both OP5 & Tslim. A pump like that would be a game changer. Love your vidoes. I may be trying to contact you in the future to clolab on a special project with far reaching potential. Untll then, you remain my favorite T1DM blogger. Kind regards, Michael T.

Thank you for your insight. I agree a tubeless solution sounds awesome. But, I'm going to stay with my Tandem Pump. It offers the flexibility I need to best manage by diabetes. Lastly, I truly appreciate your feedback regarding these options. You've been a huge help for me during my journey with diabetes. I've been a type 1 for 40 years. Going from 6 doses a day to a pump has been great for me. Thanks again, and I'm looking forward to your next video.

I am using the Omnipod Dash pods with DIY Loop and absolutely love the set up. I love Omnipod in general because the pump is much more convenient for my life (easier for travel, less intrusive, easier for use around water). I used tubed pumps for many years and really felt free when I no longer had to deal with tubing. For me, Omnipod is also much easier to deal with through insurance because it's all covered under the pharmacy benefit (it's also a lot cheaper for me). DIY Loop has worked so well for me and I can use it with the Dexcom G7 so have no compelling reason to switch to the Omnipod 5. I can also use it with just my iPhone, which is so nice.

When I retired and Medicare became my health insurance, I got a financial jolt when I had to pay for my supply of OmniPods. Because they are not durable medical equipment, like my new Tslim pump, they cost me several thousand dollars a year. My Tslim and all its supplies are covered under Medicare Part B. Medicare and my medigap policy cover the whole cost. As soon as I could switch from the very, expensive OmniPods to the Tslim, I did.
@MWS1960
@MWS1960 4 ай бұрын
Omni pods are covered through prescription. I just talked to my Omnipod rep and a one month supply is $50. Now Medicare is doing away with the donut hole for 2025. So it should not cost you thousand anymore. I have a Medtronic pump as my back up and get the supplies through plan B. Omnipod as you know is Plan D

Thank you so much, Andrea! I was finally able to switch pumps last month as my warranty with Medtronic came to an end. After watching your videos on the Tandem X2, I knew that’s what I wanted. This video, plus my last month of experience with the X2, confirmed for me that I made the correct decision. My CGM lines have never been “straighter” 😂 For example, my average glucose for the last two days is 119 with 100% TIR! Game changer for me as a busy mom of four. 🤩

Nice video! I have never considered the Omnipod but for my use the T-Slim is awesome. I had been on Medtronic’s pumps and was getting very frustrated with the way their system worked. A1c were always higher than i wanted and the number of alerts the pump gave off quite frankly drove me insane. After several conversations with my doctor we switched me to the T-Slim X2. There were some hoops as my Medtronic was still in warranty but Tandem had a program in place when i switched (not sure if they still do so you might want to reach out to them if interested). All of that to say that after a little time we got the T-Slim dialed in and my A1c for the past year and a half have been Human normal! 5.4, 5.5 and in range numbers are well into the 70% + range most days. Really happy with the T-Slim and the tube while occasionally frustrating is manageable. This was a great video. Thank you for providing the community with such valuable information and insight.

Thank you for all that you've written here and shared with us all! My first pump was a Medtronic 640g and I have to say I didn't love it. And now that I've had experience with other pumps I can say that it was my least favorite. That's not to say that it's not good, but like you, I had frustrations with the alarms and just generally how the thing ran. I'm so glad you love your tslim - and that you were able to get it before your warranty expired - this is definitely something I'm going to keep in mind for the future in general - thank you, thank you for sharing!!!

As much as I really want a tubeless pump, the micro-bolusing on Tandem is a dealbreaker for me. I also like using exercise mode to give myself a higher glucose target of 140 regardless of whether I'm exercising or not, while still keeping the autobolusing. I'm very excited to see what the Mobi brings when it finally comes out!

Thank you so much for taking the time. After using both with my kids we have gone back and forth because tslim algorithm, but tubeless ugh the decision sucks to have to make. Recently we went back to tslim partially because I was irritated with the algorithm and mostly because in sports the pod became a hazard. In football my youngest got tackled and the pod left an indent in his leg. We ended up ripping it off in the middle of the game. Today with the tslim site it wasn't an issue and I bolused him at half time. Seemed to workout well. We'll see if mobi is the winner soon enough. My oldest is tempted to go back to injections just because it seems like too much hassle at times.

I am on the Tandem T:Slim X2 as my everyday pump and use the Omnipod Dash for swimming as I swim once a week for three hours. You really hit on one of the shortcomings of the Omnipod 5. It has a non-modifiable algorithm that determines that X percent of your total daily insulin is basal. It does not "learn" what your actual basal requirements are. I am on a very low carb diet and the automatic function simply cannot keep up. I get the maximum microbolus constantly and my BSs still run off the chart. I hate tubes though, so now just run the OP5 in manual mode and get great results (5.8 A1C). It is just more work. We all make choices. :-)

@Neomet010
@Neomet010 8 ай бұрын
@@kellyremender0930 Hi Kelly. I do get very good results in manual mode, but it is way more work. I am in manual because if I stay in automatic I always run high. The algorithm of the pump does not work when I only eat 25-30 carbs a day. Are you getting good BSs on auto?
@Neomet010
@Neomet010 7 ай бұрын
@@kellyremender0930 Hi Kelley. I do get better results in manual, but this is driven by me having very low daily total carbs which makes it impossible for auto mode to keep up with my basal needs. If you are picking up carbs through what you drink or other sources auto mode might work better for you. You definitely will know if auto isn't the best for you. In auto, I could not get blood sugars below 200 without continually bolusing even though I wasn't eating any carbs.

Thanks for such an honest review! I had been on a T-slim x2 for the last 4 years and it was a game changer. I’ve just changed to the Omnipod 5 and see exactly what you’re saying. I’ve actually adjusted my carb to insulin ratio to be more aggressive because I’ve seen the same thing with the high post meal spikes. I’ve certainly made different diet decisions because of a lack of extended bolus in auto mode. It seems like the cannula holds up better in the Omnipod. I’ve been very frustrated with cannula failures with the t-slim. My early take on the Omnipod is that overall I love it. I miss some of the adjustability with the t-slim, but my sugars have overall been better since switching and life without tubes is so nice

Thank you so much for doing this comparison! I've wondered about the difference between the 2 pumps. I'm on the tslim and love it, but I look forward to the future where it has more customization for targets and certain aspects of basal dosing. I have found though that when I run in sleep mode all the time, I'm much more in range because the basals stay moving with me and it seems to be a more proactive way of using the pump. Just my experience, but I've come to love this pump!

I loved the Pods, no tubes, but would sometimes forget my PDM, then I couldn't eat. I like having the control with me all the time more than being tubeless, but I may go back now that you can use your phone or a PDM, but then what if I lose or forget my phone. Until today I was still tempted to try it but after seeing your video and reading some of the reviews I may want to reconsider. I am just majorly anti tubing and the new Mobi isn’t tubeless or even close IMO. I will probably still pull it out every time I go to use the restroom. Great insights to differences between pumps. Used Omnipod, before #5 ie after Animas stopped and not automated, too gross in it's basal profiles: 0.05 whereas tandem is 0.025 mmol. Big difference when you're insulin sensitive and rates change dramatically thoughout the day: 0.7 U/hr during dawn phenomenon to 0.3U/hr mid afternoon with daily basal 8-10 U, TDD varies 15 -22U in summer to fgreezing cold Canadian winters with same CHO content in food eaten. Use tandem cartridge for at least 6 days and never fill it completely, after removing airbubbles, 150-200 U max. Thanks for video, well worth watching .

Being extremely insulin sensitive with another comorbidity, I appreciate the conservative nature. First night on omnipod 5 it pushed me into a low. I had to go on manual mode. Pretty darn scary. So for those who don’t have extreme insulin sensitivity, I would imagine that right customization without having to be conservative is the priority.

Thanks for this video, Andrea. Omnipod, TSlim, Medtronics... At a time when a need to switch from insulin pens to a pump is drastically needed, I keep on scratching my head on the model to use. The tubulure is certainly a constraint for someone who likes running and swimming and who has a tendancy to bump into everything. The T-Slim seemed interesting and discrete but my endocrinologist was rather encouraging to try the MedTronics pump mentioning a better algorythm. If you had to add Medtronics to your video's comparison (and being well aware that the choice of a pump is a personal decision), where would you place it? Thanks a lot.

I recently had to choose a new pump after my last pump (a Tandem T-slimx2) ran out of warranty and then broke. I haven't had problems with my tubing getting caught on things and I feel that the omni-pod would be uncomfortable as it looks too large on people's arms. If the omni-pod is in an uncomfortable location, I can't put it anywhere else until the three days is up. Therefore, I chose the Tandem T-slimx2 again. I was going to maybe choose the new Tandem Mobi which is supposed to be released in the U.S. in the beginning few months of 2024. It is supposed to be 1/2 the size of T-slimx2 with a 4-5" tube which sounds awesome but since it is controlled only by phone and I don't always carry my phone around with me, I decided to go with the T-slimx2.

I'm working on this video now! I will get it up asap! Thank you so much for watching the videos and for your kind words!! I was DESPERATELY searching for any help when I first started the OP5 becuase, like you are experiencing it sounds like, I just found the system WAY too conservative - and too slow to adapt to days I need more insulin - especially problematic with the Holiday Parties...eh!! I haven't figured out the trick to get it to adapt quickly, but I do pop in and out of manual mode during these more tricky times where I need more insulin, and I pretty much always have to bolus aggressively with this system. I also just started Fiasp and that faster acting insulin has been extremely helpful in now letting those lows run away from me (Lyumjev is another one in case your insurance doesn't cover Fiasp - I actually prefer Lyumjev) - sorry that's a lot!! I hope this helps!!!! Sending you all the very very very best!!!!!

Great video. After watching your other video and I used your hack of increasing my basal by 35% when starting the first pod with the O5. I'd recommend this, or possibly higher, given the overly conservative dosing, especially on the first pod. I also have been employing the "pump and dump" technique of dosing 9-10 units into the garbage before a pod change, trying to make the algorithm give a higher background basal based on the "increased" TDI. I agree with you that we shouldn't have to try hacking the system in order to get the proper amount of insulin. I went from an even 50/50 basal/bolus split on the old omnipod to a 40/60 bolus-heavy split now on the O5, but my TIR is still mid 90% and (actual) TDI hasn't changed.

Great video. Your point about pre-bolusing with OP5 is completely spot on and can't be said loud enough. Once I figured that out, my success greatly increased with the system. After being on shots for 20+ years, and constantly snacking that entire time, with OP5 I can go huge stretches of time with flat blood sugar and zero eating. The result is I've lost over 20 pounds without even trying. The downside is that once I do eat and have zero insulin on board, I would go sky high and stay that way for hours. That's where pre-bolusing is essential. Sometimes I'll give half the bolus an hour before eating, then the other half closer to mealtime when I have a better idea of how much I'll really eat. I'm having great success with OP5, but it's not nearly as hands-off as I had fantasized. I want to see what the Tandem Mobi really looks like. I'm wondering if you can tape it to yourself and have it feel quasi-tubeless with a super short tube. At some point, I bet insulet will make their algorithm more flexible, but that'll be a few years I bet.

Your comment is so so SO well said. As I was reading all of it I was just nodding along truly I couldn't agree more with absolutely everything you've written so eloquently here. I just recently discovered you here and love your take on things. I am a new pump user (2 months) and only know the Omnipod. I was so frustrated with my numbers constantly wanting to drift higher. Especially during the night when they would continue to climb throughout the night and hover just below 180. If it can keep me at 180 why can't it keep me at 110?! I did the reset thing after watching your other video and loved it for the first pump but then it started climbing again. I am to the point now that I only go into auto mode at night and I do manual all day long. my control is better than it has ever been but I am super vigilant. I am considering doing a reset with every pod just to enjoy a couple nights of good control. Thanks for your content!
@ShesDiabetic
@ShesDiabetic Жыл бұрын
OMG...the reset with every pod...I TOO HAVE SERIOUSLY CONSIDERED THIS!!! I recently was ill and eating very little so the past few pods ended up delivering way less insulin and I feel like I'm fighting my way back up to a proper amount of insulin. Something I read on fb was an interesting suggestion, people suggest doing a 'pump and dump' where at the end of the pod you take it off your body, pump 8-10 or whatever units, and then deactivate the pod. So the algorithm has more tdi in its memory. It's not been perfect, but since doing this I think I've also seen some overnight improvement and general improvement as well. But like you said so perfectly, sometimes I just wonder if I want to reset it every time because those first pod days seem like the golden period! Anyway - long comment here - just wanted to share the pump and dump thing with you and tell you I'M RIGHT THERE WITH YOU!!! Thank you so much for watching and sharing!!

Thanks Andrea, great to see you back and looking so well! Awesome video, thank you so much. MDI here and the only thing that would get me on a pump would be the ability to keep my a1c starting with a "5" with only the minimal amount of carb intake information, and even then only if it was tubeless. Shame the op5 isn't more aggressive. Do you know what your a1c was with the tsx2 and op5? That would be an interesting comparison. As an aside, i know you can calibrate the dexcom G6 to report higher or lower. I wonder if you could "fake" calibrate it to read 1mmol higher in order to get your omnipod 5 target to 5mmol instead of 6 (or whatever)
@SwissPGO
@SwissPGO Жыл бұрын
I used that exact tactic with the medtronic pump that was way too conservative. I always carry some sugar with me and I'd rather a have a few lows per week that can be easily corrected that staying for hours at around 10 mmol/l (180mg/dl for US readers) . The problem is that the clinical trials and diabetes specialists have defined a KPI (key performance indicator) called time in range. And they set that range usually as 70-180 mg/dL or 3.9-10.0 mmol/L. It would be way better to get that range defined as 3.4-8 mmol/L I believe.
@friedelpas5637
@friedelpas5637 Жыл бұрын
@@SwissPGO I'm not sure about that. I believe that the time in range is more important than changing the range. Especialy lowering the low boundery seems to me dangerous. In the Tandem you can change the range bounderies, but I have not done that and so also between 70 and 180 mg/dl. But I try to have that in that range more then 90% (in most cases over a period of 30 days I succeed to get that between 90 to 92 % in range). I got with that a A1C of 5,1%. Last time it was 5,3. I had the time before covid and as result of that the time in range was reduced to 88%. So for that reason I believe that the 70 to 180 bounderies are good, but that you need to achieve more the time in range. The new medtronic pumps have an algorithm comparable with this of the omnipod 5 and possible there this can be a better option because you have less flexibility to react and to get the values in those bounderies than you have with the Tandem pump.

Very good review. I was on OmniPod 5 for less than a year before switching to Tslim. I’d been on Medtronic pumps for 32 years prior. Insurance was the reason I switched. Over the 5 year warranty period (Medicare), OmniPod 5 would cost me about 4 grand more. I do find the Tslim gives me better numbers. With OmniPod, I was in range about 92 % of the time. With Tandem it a smidge better at 93%. The Tslim is better at correcting highs. I still have 4 more years on my warranty. By then, there will be a completely new technology, I imagine. Of course in 4 years, the cure for diabetes will be only be a year away and I won’t need to be so concerned about pumps, right?😅

I love how you said that you have to hack the omni pod for it to work the way you want it: This has been my experience too with the various pumps I used in the last 15 years. It's time for these pump producers to open source their software and published their API's and communication protocols so that WE (the T2D community - who likely has way more collective knowledge that the manufacturers), can personalize our algorithms, using the mobile phone technology we want (e.g. as an iPhone developer) and integrating activity monitors and GPS location. And thanks Andrea for your reviews! One question: what insulin type is used with these pumps? I was an early adopter of fiasp as it was noticeable more reactive, and I use it with my current pump who is not certified for it and does not even have an option to configure insulin activity duration. But that "hack" resulted in getting me down a bit faster from highs.

I'm using fiasp also on my previous pump and that was a game changer for me. For tandem they recomend not to use fiasp because they result with this pump more to blockages. I'm using now lyumjev what have the same extreme fast working as fiasp without the blockages of fiasp in this pump. The reason is that the proteïnes make the insuline ticker what give problem in the small lines of the pump. I'm now very happy with lyumjev and have even better results then with fiasp. But that can be of course also because of the excelent Tandem controlIQ algorithm

The great thing about the tslim is that it doesn't really need things to be too too too perfectly dialed in - as long as your basal rates aren't wildly off (making you go low or high on a consistent basis) then I think you're going to be really pleased with how quickly the system improves your time in range, at least that's been my experience, and would be my hypothesis for yours. WIth you being stop and start all day - oh you're just the PERFECT candidate for the Control iQ - I can't impress that on you enough - keep fighting for that Dexcom in whatever way you can becuase I really believe it will improve your quality of life!! I personally don't think I will ever go with the omnipod. I cant count how many times I have knocked off my dexcom, and I know that with an omnipod I would be in constant fear of knocking it off my body through daily routine. I am very active, so this is a high likelihood. If all one does is sit in front of a computer or TV, then I guess the omnipod is the way to go, but for active people, I would definitely recommend a wired insulin pump. When exercising, I use a leg strap and the tubing and pump is neatly tucked away under my gym shorts and does not get in the way of any of my exercises. other times, I just have the insulin pump in my pocket where it is out of sight with the tubing tucked away in my pants. It is simple and discreet and there is no fear of knocking the pump off your body as with a pod.

I don't know if I found it less daunting because I really didn't know what I was doing anyway and was learning a lot in a very short amount of time, but like any open source software, I found the AAPS community to be very informative and helpful. I'm probably also fortunate that my endocrinologist has not had any concerns with it, maybe because my most recent A1C was a 5.6 and we confirmed that my honeymoon. Was probably measured in weeks at best. So I credit the software with being relatively solid considering it's probably not fair to credit me (I'm still not sure. My carb counts are even close to accurate) haha. I haven't had a lot of success in fully closing, but as long as I deliver a reasonable guess on bolus I stay fairly well in range.

That is SUCH a hard question. To be honest I think coming from tslim over to OP5 is the hardest option becuase they require such different user behaviors and interactions. SO if you were starting afresh from no pump at all I'd say it would be an equal jumping off point - but I think the tslim is the better algorithm so perhaps would be inclined to say that pump. However, if you'd never experienced a pump before the benefit of the OP5 is that there are no tubes, which is one of the hardest things to get used to when you go from no pump to pump. Gah! Sorry I know I'm not flat out answering your question - I apologize! It's just tricky to nail down one direct answer - it's all so personal

Bloodsugar drops under 75 but there was a huge (child-)meal with 45 carbs. As I got it, you cannot give a bolus until the bloodsugar gets back to 140+.
