Not overly convinced by the view that aviation has had just cultures for years; we have talked of it but remain very reactive still. Its easy to convince yourself or be convinced that a just culture exists yet most organisations have had minimal training on what it actually means, put little effort in to creating the right culture beyond the policy and often remain focused on individual acts or omissions. I would argue that most mistakes and deviations remain hidden and only if there is a visible negative outcome do they get reported. Whilst Just culture is, I believe, still the right thing to do, it has been hijacked by old school mindsets that feel a policy & some training changes culture.
