What is heart wrenching is that in their one awkward interaction back at the academy, Shiro told Mimihime he wanted to keep her in his room.(As a confused and somewhat weird way to express his affection) Mimihime then asked him if he'll stick her with needles or cut her up, to which Shiro promply replied with never. Fast forward some years, guess who ended up stuck inside a room, with needles and cuts all over their body. Mimihime also had the fear of sharp objects if that wasn't sad enough.

For Shiro it was emotional and also phycological knowing that his despite his best intensions could never truly help her only prolong her suffering as long as her will lasted.
