This video was from a long time ago, back when wrath of the lamb was first released it didn't drop from mom and you had to hunt for it if you wanted to get to the chest, they patched it since then.

The cancer trinker is useful if you have 5 rate of fire it lowers it to 4 or 3 so it will help with ur rate of fire which will be almost as fast as soymilk but with normal or 10 or 20 or 40 if you have ipecac
