Todd is the prime example of an insufferable Idiot that is a Genius in one thing so you just cant afford to get rid of him for good. He is an exceptional Surgeon (shown multiple times across all 8 Seasons) but in everything else he just fails :P well besides in making us laugh.

I get where you're coming from, but they work great as support characters, they don't have much going on and are usually only 1 or 2 dimensional, and you like them because they work in small doses, and can be sprinkled in anywhere for a guaranteed laugh...think of how much Joey the show sucked, as opposed to Frasier which had its own beat and universe. A show with the Todd, Hootch, or Ted would never work because wouldn't be able to get behind it, they wouldn't able to support the protoganist

Why is Tod's comment to Elliot (is it greasy outside) funny? What does it mean?

You can't see it very well in the video, but Elliot's forehead looked especially greasy. This is because other female staff were making snide remarks about Elliot because they were jealous of her looks. Elliot wakes up at like 3 or 4 am to make herself look gorgeous for work, while other female hospital staff just tie their hair in a ponytail. The comments and general hate (due to their jealousy) got to Elliot so she stopped making her self look pretty, thus causing her greasy face.
