Facetious devil's advocate but if this town had never seen a crossdresser and it was to them inconceivable that John would wear a wig from how they knew him, it probably didn't even cross their mind. He ends up with DID due to the CA and SA he suffered as a child and teen because of his late mother, who’s clothes he wears when the female identity is in control.
