TR Martin, looks like just a flesh wound but what one needs to worry about is getting a concussion when one blacks out or in Riley's case, very close to it. If he was nauseous, vomiting or felt the need to sleep he should get medical help asap.

1st rule of First Aid: Always LIE about the damage, no matter how bad it is. You can send a patient into shock/death by over reacting to a bad wound.

Elayna, get some lidocaine, and a syringe on board. Learn to do proper stitches. One of these days he's gonna cut himself deep, and bad. And you will need to give him subcutaneous stitches. There are a million you tube videos on how to do it. Great video guys!!❤#rileyneedsahelmet

Scott Oswald Completely agreed! Sooner or later a bad injury will happen. As one of two people will be the only chance of self rescue, I would highly recommend taking courses on medical stitching for both of you and keep current on it!

Scott Oswald I agree Lidocaine for sewing up wounds. Learning how to sewup wounds is a excellent idea. Riley needs to learn as well. Accidents go both ways, male and female. Proper sEwing needles also as the skin tissue is very tough

For a wound like that a lime-wedge will disinfect / cauterize and close within a few minutes. Press the lime-wedge into the laceration, the deepest part will heal first. Also works great for cat-scratch infection, my friend got scratched up and bitten playing with young tiger in Mexico, he complained and would not take my advice for two days, the lime neutralized in few seconds.

Hahah he seems to hurt himself lots when I'm around too. The first night we met up in Malta, he walked straight into an unmarked hole on the footpath and cut his big toe pretty bad.... Maybe we need to wrap him in bubble wrap from now on!!

If he had lost consciousness, unless he was tied in or wearing a life jacket it would have been nearly impossible to turn the boat around, find him, and then get his head above water before anoxic brain injury changed your lives forever. Responsible boat owners practice MOB techniques every year, and I can tell you it is much harder to do those maneuvers in the panic of the situation than one would think. Outcome is often dependent on the weakest members. In this case the most important person would be the one continuously pointing at the victim so you don't lose him when turning around. Would your guest even have know to do that? Recognize this experience as a near miss, a wake up call, an opportunity that winners will rise to.

At 5 knots, in only ten seconds the boat has moved away by 84 feet. Ever tried to swim 84 feet in cold water with your clothes on, even if you can find him from the water? Nowhere in the world do man overboard protocols include jumping into the water, and most specifically forbid it. You end up having to find and save two swimmers, and it would not be uncommon to lose both or even run over one of them in bumpy water. For bystanders, the most important thing is to continuously point at the swimmer, because the greatest danger is losing him in a bland, featureless sea. The crew will be focused on sails and turning the boat, so their attention is critically diverted. When the skipper looks up, he/she needs all the help you can offer to find the swimmer.

I have been sailing for 10 years and always using life jacket. I can´t understand why they don´t use life jacket ( life vest ), and in bad weather also be tied to the boat. I´m getting nervous when they are climbing on the boat way out in the ocean with no security.

@Mikkel A lime-wedge will disinfect and cauterize. I don't know why it works but great emergency medicine -- pain free other than the pressure on the nerve endings in the wound. Closes the wound from the deepest part out, much like seawater.

Tie the hair together in lieu of stitches. I cut a gash in my head on a diving board when a boy. Doctor offered stitches but warned about a ridge that will catch a comb later in life. Doctor suggested using head hair as the mechanical means to hold wound together. He disinfected, tied my hair across wound, and I live with the memory 45 years later. Proof that there is more than one way to solve a problem. Yes yes yes

When working in the ED during my internship we would staple these wounds together after irrigation. I did have an older, ED nurse describe the exact technique you mention if the person has long enough hair, if you had too and weren't in a situation to get proper medical care.

Hi Riley ~ When you jump off the moving boat, make sure you jump off the same you would as if the boat were docked and motionless. The forward motion of the boat is also moving YOU forward at the same speed when you are on it. So, although the water is moving quickly away from the back of the boat, you are moving forward at the same speed as the boat. To jump far enough away from the boat so as to prevent hitting the boat, you need to power launch yourself in the same way you would if the boat were sitting still and docked. Please stay safe!

So my good man do you enjoy sailing about on your beautiful craft with a wonderful companion and meeting interesting people? If you do then stop taking it for granted and temper your impulsivity while on the high seas. Yes it could have been worse. You were a bloody whisker from meeting your maker. In my experience unconscious people drown rather quickly. We are hopeful for many more episodes and so does Elayna.

Never hurts to take a suture response course. Having the knowledge to repair a more serious injury that needs stitches but you can't get to a hospital quickly. Keeping lidocaine and some packs of suture on board is a good idea in my opinion. In some situations you can even call a doctor on the phone and they can walk you through some of the prep, etc. also follow up to prevent infection. Glad that Riley is ok!!

Riley, decision making time. Either take an extra moment to consider your actions, or start wearing a flotation device. You're starting to make these kind of injuries in the water a routine. Sorry for the stern words, luv you guys.

Completely agree and came to say this same thing. Repeat incidents within a short time span speaks not of coincidence and bad luck but lack of attention and care. With just the two of you to count on, there is a huge dependency on each other when out at sea. You should take MORE care not to expose to unnecessary risk for the sake of a good shot or hotdogging, etc. ALSO A HEALTH CARE NOTE: Deep lacerations / cuts on the scalp are NOT an injury to take lightly. Besides the risk of concussion / brain trauma from the impact, cuts should be properly flushed / irrigated, disinfected, treated with appropriate antibiotic / steroidal treatment, closed (properly), dressed and checked regularly. The risk of infection and serious complications is too high to haphazardly treat. Much love to your craft guys, please do it with care.

Ocean salt water has some disinfectant properties but still contains microorganisms, debris, etc. It's just important to irrigate with (if possible) saline or clean water to clear trapped material as much as possible before closing the wound for self healing. Ocean bound sailors can get away without it but an investment in a emergency first aid course to understand basic procedures / sutures / CPR / shock prevention / IV hydration / etc could be life saving when hours or even whole days away from professional medical assistance.

Judging by what Riley said he was seeing and feeling, I would say with certainty that he suffered some level of concussion. I hope that you sought medical attention upon immediately reaching Tunisia. That wound is quite deep as well and should be treated and kept clean. You probably should have shaved around it as well to help keep it clean. Safe travels to you, and I'd recommend putting down non-skid abrasive strips to prevent another slip and fall accident, don't allow someone to go off of the stern at such a high rate of speed, and always wear a life preserver. If you had been rendered unconcious and went under, they might never have found you by the time they had come about to pluck you out.

The eyes don't lie. I'm sure he hit his head hard enough to get a concussion. Not because of the cut but his actions. When someone hits their head you have to be very careful. Learn the proper procedure to see if you are looking for other problems etc. you may end up saving a life. The cut was the least of the problems that could occur. I had one that led up to a three week stay in hospital. Would have been nice to see a PFD to begin with. Get a ski vest that will keep you afloat.

Riley, I feel your pain. Yesterday , I slipped on ice in front of my home and rocked my head on the ice. I momentarily blacked out. I have a concussion which hopefully will go away over the next few days. Keep up the good work, I have been following you guys since the beginning.

I work in Middle East with a lot of Tunisians and v nice people in my experience at least - Re wound, usually I'd trim the marginal hairs so they are not dangling into wound and so delaying the healing. Scalps gen. bleed a lot so glue can be difficult and sometimes useless or worse, an impediment to healing when dipping into the wound rather than "roofing" it. An old technique is to 'suture' the two sides with opposing strands of hair [in small clumps not singular strands of course].

Oh man that was scary! Just shows how on the ball you’ve got to be living aboard, but it’s hard when it’s your home, easy to get complacent. If you’re ever near us will show you how to do sutures etc! Moments like that make me very glad we have a full veterinary kit on board that can also be used on human-animals! I heard with a head wound if there is sufficient hair you can just tie the hair together around the area to close it up, using the hair sort of as "built in stitches." Anyhow I'm sure it all worked out. Love the video's guys, I will be out there on my own boat someday and I hope our paths cross! Cheers!

If you don't have lidocaine and sutures/stapler, you can use the hair to close a scalp wound like that. Make sure the wound is well cleaned and then use strands of hair from each side of the wound and tie them together, just tight enough to close the wound. That said, I'd use this as a reason to take some more advanced first aid lessons and stock your vessel with disposable lidocaine, suture kit, and skin stapler.

Seriously, I am not one to lecture at all but maybe they should practice a recovery event just to be sure they know the fastest procedure to follow? I heard something about "...putting the sails down and motoring back to him", but that's like 5+ minutes versus marking the position on GPS immediately and circling, no?

Great having Josje onboard, those 3 were some of the best crew on Delos IMO, Expierence sailor right there Josje be. cool cool. would like to see her comeback on. Now onto Riles fail of a launch off the back Dive, well don't do that again Riley! Although I'm sure pro divers in the Olympics all have had that happen soo chances it was gonna happen. but yeah. That had could ended a lot worse. It is quite dangerous to jump of a moving vessel, you can easily underestimate the speed and get your head knocked down the back end of the boat. I dont wanna be sceptic but as a tip i would recommend wearing some kind of lifejacket, a man overboard manouvre costs more time then you would think. And if you get K.O. death is certain. That's so weird cause one of my best mates, his girlfriend has been teaching Engligh in Tanisia for the last two years, she loves it. He's going on another two week trip there to see her next month, his second trip and no terrorist activity to be reported anywhere there've been . So awesome to see Freda and Max again!! Mum Blic Yes. Casual short hop off the back without being careful to clear the step. Also, not wearing pfd when boat is underway at speed is risky. If Riley had lost consciousness, he could have easily slipped below the surface before he could have been recovered.

I'm going to sound like an old nagger, but geez Riley your apparent disregard for safety really is frightening. No PFD's worn by crew on nightwatch, not to mention the lack of harness being worn at least outside of cockpit. You are the master of that vessel and responsible for the safety of everyone on board. Sliding off the back under sail. Geez. God bless you. All we can just hope is that a serious incident never befalls you guys. I do love your work, I just want you to be safe.

Guys, a suggestion. Not sure if you have tried emergency maneuvers but they are a good habit to get into just in case a key crew member ever winds up over-board and you need to react quickly. In your case it’s just the two of you and that is a lot of boat to handle for one person especially in heavy wind and seas, When I first taught my girlfriend in the 80's (now my wife) how to sail we began with tossing a fender overboard, started the clock and I watched her; toss the life ring, drop sails and start the engine. As expected, the initial trials were a nightmare, all I heard was “WHAT DO I DO” and I replied I can’t answer because I’m not here, I’m in the water drowning. What a great first date LOL!!!! After just one day we got initial reaction time from 5 minutes down to 90 seconds. Time as you both know, is critical on the water and you have a much bigger boat than I had at that time so it is going to take you some time to manage the process. Riley (sorry buddy) did NOT have a life jacket on and had he been knocked out, trying to stop the boat and retrieve a waterlogged lifeless body is incredibly hard to do. You need to have an emergency lift plan in action also as bringing Riley on-board would not be an easy task given such circumstances. Knowing CPR is a must also. As always, wishing you both smooth sailing with the wind to your backs.........
