Might want to see about some awning on the door. That’ll help keep the splash down from rotting all that out again

The trim plate goes the other way but whatever. Also really it should be mortised into the jamb head piece. You lose 1/16 clearance doing it that way. Put a 3 inch screw through one of the holes in the top hinge of the inactive panel through the side jamb into the wall framing. This will rock the whole door that direction which will lift the opposite (bottom) corner away from the threshold relieving the rub on the threshold. If you got a roll of 6 or 8 inch galvanized flashing and put about a 7 foot strip against the framing below the door and behind that concrete it will help divert water away from the framing.
