Sam Harris speaks with Jeannie Fontana and Robin Carhart-Harris about the TREAT Initiative in California and the growing promise of psychedelics for mental health care.
Jeannie Fontana, MD, PhD, is a multifaceted leader in healthcare, specializing in Internal Medicine, Environmental Toxicology, and Biochemistry and Molecular Biophysics. She initiated an ALS-focused research team at Sanford Burnham Prebys and established the Cedars-Sinai ALS Center of Excellence, driving policy change and boosting federal funding for ALS research. As a founding trustee of the California Institute for Regenerative Medicine (CIRM), she played a pivotal role in creating the world's largest stem cell granting agency with an $8.5 billion budget. Under her influence, the agency has achieved FDA approvals, fast-track designations, and launched groundbreaking clinical trials. Additionally, CIRM catalyzed the creation of over 55,000 jobs, 50 new companies, and $10 billion in added state revenue. Now, Jeannie is poised to address California's urgent mental health crisis through her role in the TREAT California Initiative.
Website: www.treatcalifornia.org
Robin Carhart-Harris, PhD, moved to Imperial College London in 2008 after obtaining a PhD in Psychopharmacology from the University of Bristol. He has designed human brain imaging studies with LSD, psilocybin, MDMA and DMT, and several clinical trials of psilocybin therapy for severe mental illnesses, including 3 key trials in depression plus 2 further trials in anorexia and fibromyalgia syndrome. Dr Carhart-Harris founded the Centre for Psychedelic Research at Imperial College London in April 2019, the first of its kind in the world. In 2020, he was named among the top 31 medical scientists by The Times newspaper. Dr Carhart-Harris became the inaugural Ralph Metzner Distinguished Professor of Neurology and Psychiatry at the University of California San Francisco in 2021, and in the same year, was listed in TIME magazine’s ‘100 Next’ - a list of emerging leaders from around the world who are shaping the future.

As a medical student interested in psychiatry and neurology, listening to this chat about psychedelic therapeutics has me beyond excited. As mentioned, there is immense potential to change the landscape of how we treat and understand mental health disorders. We're talking about potentially shifting away from the traditional pill-for-an-ill approach to a more holistic and profound therapeutic experience. For those of us diving into the world of brain science, this is a thrilling time - as it is for so many patients being failed by current treatments. Gone are the days when these substances were purely associated with countercultural movements or recreational escapades!

Yes, and hopefully these psychedelics will be made legal for use by the general public sometime soon as well! Like shrooms. I don't mind if they are regulated and come with booklets on safe usage, but there are so many incredible benefits it is almost criminal to keep them illegal just because we have people in parliament or congress have an irrational fear of drugs (mostly conservatives)

I share your excitement. There is a lot of work to do to get to acceptability, however. I work as a social worker in the outpatient psychiatric clinic of a hospital. I was recently discussing the potential of these drugs with a few collegues, especially with patients having difficulties identifying purpose in their lives. The suggestion that we should envision these treatments at our clinic was met with great opposition from more experienced team members. Their argument was that it makes no sense to suggest drugs to an addiction prone patient population... it will take time to deconstruct the recreative narrative around these compounds.

on one hand I am excited about the research on psychedelics. I just think the woman scientist in this podcast is just a little naive and I am concerned that big pharma and government want to control the mushrooms medicine too much for profit and that it won’t be available as a natural available resource for all.

@@xRedSmartiesxi disagree regarding the view and prejudice for recreational use. I believe it’s generally pretty safe and that recreation is not negative although I agree that it can be even more helpful in certain serious clinical settings. Psychedelics are not addictive at all and are known to help people break their addictions. There is just still too much fear of nature, which we are a part of .

I was on Methadone for several years with the V.A. I finally managed to get off the medication after a 9-month weening period. I am currently involved in a Ketamine therapy protocol. While Ketamine is classed as a dissociative, it has recently been classed as a psychedelic in some circles. I fell into a deep depression after getting off Methadone. Ketamine has proven to be the most profound and quickest acting anti-depressant that I have ever experienced. The efficacy of this medication is nothing short of mind-blowing. Thank you so much for your insight into this emerging field of therapy.

Sam, I think you are in a unique position to educate other well educated people that psilocybin is safe and doesn't need to be controlled by insurance company's rules. One of my most powerful experiences was finally experiencing the "no head" perspective that I have tried for years to get after having read "waking up". I would have never had it if I was not reaching for a glass of water. I saw my own arm in front of me and realized it didn't reach back to a head, it reached space to space. and then I realized we are all space and energy. I had the most beautiful breakthrough in that moment, seeing the spaciousness of my being. This will not happen with a mask on in a therapist's office. and this is just one example. I cringed imagining my experiences being subject to two therapists in a room with me instead of the experiences I've had with nature. Our mind will do the therapeutic work without that container, in my opinion. Sure, make it available to people that want therapists there- but please do not force people to use this medicine in only one perscribed, insurance company devised way. Your own experience (I'm going to butcher this description) where you described experiencing witnessing yourself without caring about what another person thought of you is also so valuable. How would someone come to that while blindfolded with clinicians? That can only occur in relationship.

It's been 3 years since I tried DMT, can't find a plug anywhere in my area. Back then, We used one of those vaporizers with the big bags. The first time I didn't do enough and I just seen shapes and colours. But the second time I managed to get another hit in before I got the light headed. I remember floating above the country side flying and then all of a sudden I zoomed down into a deer and went right inside of it and I was the veins and the blood and flowing through this deers body. It was very strange lol

I Strongly recommend just sticking to shrooms if you want an hallucinogenic experience. I was given “DMT" one time by someone I trusted (at the time) and it turned out too intense that the trip lasted for 24 hours. Can't really be sure what you're getting aside from shrooms. Cause I personally have never had a bad experience with mushrooms though.

When I was about 28 years old, I had a dream where I learned how to fly through the air. When I woke up, I felt this euphoric, spiritual calm that EVERYTHING was as it should be. The normal self talk was gone, and just an overwhelming feeling of peace, was inside me…all day long. The next morning, it was less pronounced, and then the next…until if faded away. The dream triggered something in my mind, I wish I could duplicate WITHOUT getting addicted to drugs. I feel so fortunate to have experienced it, in my lifetime. So…if people can at least feel this once in their life, it might be worth it.

I got those quite often. You believe in the possibility of this whilst in the dream state. The people who witness seem to believe it too. I have always considered that you've tapped into some kind of extra or forgotten ability. I'm early 40s and they usually come around every 5 years or so. Dreams aren't something you can control but that dream is something else. I've reached a heightened level of control of it in the past and like you say the next day it can be pretty overwhelming.

If you can understand my point, that will turn into the most powerful medicine for you:spirituality:Quran says: “Allah:there is not God except he”:The Neccessary life/consciousness,sustainer of life/consciousness.” Wire like neuronal structures that conduct electricity via ions/neurotransmitters in the CNS/PNS possess no attribute of thinking/life and yet that has “randomly” led to life. Consciousness/thinking is an innate idea(“Fitra”)that is distinct from carbon skeleton and yet the materialist scientist believes that chemistry turned into biology via “god of randomness”/”Emergent property”/”law of nature”. Consciousness can only stem from Necessary Consciousness (Allah-one/indivisible/loving/self-sufficient perfection).

@@bluesky45299 That's exactly what I would say if I had no way to explain how thousands of religions exist simultaneously each with their own moral code and their own idea of God. It's just not real as sad as it is. Why bother be Islamic when you can just be Buddhist or something else your entire life with simpler praying and still make it into the same heaven? They can't all be real while saying yours will be the true way to get into heaven.
@bluesky45299
@bluesky45299 6 ай бұрын
@@SeanBeansDad Well, I will simplify it for you. You must examine the central doctrine of each faith. Here is the central doctrine of Islam:”There is nothing worthy of worship except Allah,Muhammad is the slave/messenger of Allah”. Now, this doctrine is direct counter to liberalism( maximum freedom/equality), which translates to worship of self( desires) and satanism(“do what thou will”). Any religion that seeks to establish partners with Allah in forms of “son& daughters” or in forms of “desires” and informs of “idols” can automatically be eliminated. Because there is only one Existence Worthy of absolute surrender: Allah(one/indivisible/self-sufficient/infinite/perfect/unique). As Quran says:”There is nothing similar unto him”.

I agree. To say that people need to have "agency" and then also take it away by adding plant psychadelics to the same system that relies on insurance companies to be gate keepers is going to be a nightmare. We're going to have the same issues we have with pain medications and other drugs that people need. Right now, ketamine is legal for therapeutic use and how many people have access to that. I found that I can access it if I prove I've tried multiple anti-depressants and they didn't work and then an "augment" - meaning a doctor also tried to medicate side effects of depression like sleep issues. No, this is so much gatekeeping and control. This is not "agency".

At 36:12 Fontana, after admitting that she only relatively recently became aware of their presence, describes the concept of what a Trades Union is, as though the knowledge of such an organization would only be known to those who specialize in the arcane. Thanks for a balanced, level-headed look into this exciting new frontier of public acceptance with psychedelics. I am delighted with the medical and social un-demonization of marijuana and psilocybin. Both may permit one to disconnect from ego, which could allow one to examine herself or himself and their situation more clinically, more kindly-less critically. I know from personal experience that among other possible benefits, psilocybin allows one to break negative mental process habits; unhealthy loops in thinking and behavior. I believe it is these negative loops that in part prevent us from feeling entitled to real meaning and true happiness in our lives. Thanks again. - Jim

Quran says: “Allah:there is not God except he”:The Neccessary life/consciousness,sustainer of life/consciousness.” Wire like neuronal structures that conduct electricity via ions/neurotransmitters in the CNS/PNS possess no attribute of thinking/life and yet that has “randomly” led to life. Consciousness/thinking is an innate idea(“Fitra”)that is distinct from carbon skeleton and yet the materialist scientist believes that chemistry turned into biology via “god of randomness”/”Emergent property”/”law of nature”. Consciousness can only stem from Necessary Consciousness (Allah-one/indivisible/loving/self-sufficient perfection

@@bluesky45299 The Latin gives it a fancy, authentic-looking touch, but Descartes was _French._ It's also worth noting that Descartes' argument attempts to appeal to the intuition, and thus it establishes nothing empirically; the claim just _seems_ axiomatic, but consciousness still might not _be_ what it _feels like._ Though the famous phrase attempts to be philosophy's First Principle, it has settled nothing-except perhaps that it's now clear a flag has been planted by Romanticism, which has "established for us" a formalized space for irrationality. The old school of thought is called Dualism, and it's dated. At its best, the pile of rubble has been re-arranged into an intellectual spaghetti called Compatibilism, in order to try and to give the old mind/body separation a gleam of legitimacy; but when it comes down to the nuts & bolts of it, there is only observable reality and those who wish there were somehow more to it. As if, as one looks up and out, it weren't already vast enough! From here, there can be seen both the infinite and the infinitesimal, and yet you'd still prefer it if there were an infusion of magic to add to the complexity. Maybe re-examine some of that intuition.
