he didn't grab it because it is not a so great item and familiar kind of items belong to the conjoined transformation which I believe gives a tears and dmg down, so basically he didn't grab it because there are a lot better options or either reasons to get that transformation. Also since you are new you should keep in mind that you might find stuff that you could not want later in game or stuff that could be just useless (there's a lot of stuff of that kind). And also also it is pretty important to unlock the runes from the challenges asap because then you might be able to reroll an item that you know you'r not gonna grab from a floor (if not playing having another resource of rerolling in that situation, like the D6)
