We think of pain as a symptom, but there are cases where the nervous system develops feedback loops and pain becomes a terrifying disease in itself. Starting with the story of a girl whose sprained wrist turned into a nightmare, Elliot Krane talks about the complex mystery of chronic pain and reviews the facts we're just learning about how it works and how to treat it.
Talk by Elliot Krane.

I was in a severe accident in which I was pinned by a semi between my car and the cement barrier on the highway, I broke my pelvis, and several bones including some ribs, and bones in my legs. I had to have both my legs amputated and now deal with blinding pain 24/7. I hardly sleep anymore because the pain won't allow my body to relax. I take several narcotic medications and hope to one day find the right doctor that can help me with my pain. I don't think most people realize how much physical pain some people have to deal with.

Narcotics isn't really helpful for legit nerve damage and chronic pain. Antidepressants and cox inhibitors are the best, truthfully. Strong opioids and nerve blocks did nothing for me. I'm deformed and injured and my nerves are scarred and OA/bone disease is all over so I can't be fixed. I can't relax cause things don't work right. I'm experimenting with Antidepressants and Celebrex and anticonvulsant medicine because those seem to actually do something. I hope you find peace.

Excruciating pain 24/7 can put you in a pretty vulnerable state of despair. Everything seems amplified, very loud in the brain. Losing sleep creates a volatile mood but baseline depressed. It's a common post hoc ergo propter hoc.

The sad thing is that this syndrome is present right at the start but few clinicians ask the questions or look for the signs that would identify it. With early treatment the excessive pain can disappear very quickly especially in children. The lack of interest in CRPS was the biggest frustration of my 30 years working in Emergency Departments.

Elliot Krane is doing important work helping patients with nervous system pain but most of the doctors don't know what it is living with Chronic Pain just like I don`t know what it is like giving birth to a baby being a male . I live in London , U.K, and have free medical services , eg the N.H.S but I have been living with Chronic Pain for 40 years . I went to a pain clinic a few times and the specialist said to me " Embrace Your Pain " and I never went to see her again. The words I have written in this article don`t give any good tips but I just wanted to say this to my other people who live with Chronic pain eg nerve , inflammation, spasticity, muscle pain etc . that I know what you are going through .

As a long term chronic pain patient, I’ve tried the physical therapy he mentioned and much more. Didn’t work. Then I got dependent on opioids that I was told would not be addictive. Long term use has caused numerous other physical problems and even surgery, but I’ve learned to manage them with help. My measure of success is “am I functional.” Can I do what I like to do? Avoid opioids if possible. Try everything else first. But if opioids kelp you be functional, then that’s your choice (if you can find a doctor to write that prescription.). I would like to watch a video by people who whipped chronic pain and knows what it is like dealing with the system. Have not seen any like that. Like a former alcoholic who counsels alcoholics. They understand. They have been there. Only then can you appreciate what we go thru to stay out of pain. This is not an endorsement of opioids by any means but everyone is different and should be treated that way.

I agree. Doctors are afraid they will get you addicted and under prescribe. Thank you big pharma. And PT is another money making scam "here, lay down and bring your knee to you chest, do that ten times a day and eventually your knee will touch your chest" well no sh*t? Or "strengthen your core" I don't know about you but I'm not about to use the ab roller when every muscle in my back is straining to hold my disc in place. It makes zero sense, costs tons of money.

You are welcome to reach out to me. I just graduated from Mayo Clinics Chronic pain rehab program. You learn the concepts and ways to deal with things, ill just flat out tell you though. There is no "fix" for most chronic pain. You get just get a better handle on it by doing small improvements daily. The program was 3 weeks long 8 hours a day, and I was scared I wouldnt even be able to do it at first. The first few days were very tough, but it got easier. At the end of all of this, I am not cured or anything, but I honestly can say I feel excited about the new me I can work on. I have to make some modifications in lifestyle, but don't try and get back to the "OLD YOU". That most likely wont happen, you have to be willing to accept and start working on the new you. Pain is processed in the brain, even though you feel it in the back arm neck etc... but there are 16 different areas in the brain that process pain. Its a COMPLEX issue, and we all need psych techniques to help calm the stress response in the brain. I had to hit rock bottom before I even got to this program at Mayo. I spent 100k on different types of treatments and medications etc over 3 years. Finally this program came and I was close to ending it all... please do not hesitate to reach out. I can not advocate for this enough. Chronic pain affects more people than Diabetes, heart disease and cancer ....and gets 1/12th the funding.

Hey, if you’re suffering from chronic pain you might find my new website useful: youngpeopleinpain.com I have designed it as a young person who has recovered my life back from chronic pain. I created this website to share all of the knowledge and experience I have accumulated over a few years learning about this condition and how to get your life back from it. On the website, there is a scientific focus, with friendly advice and summaries of key books to help you recover. I am not a doctor, and I don’t give medical advice. I simply explain what chronic pain is, how I recovered from it and point you in the direction of expert information that will help you get your life back like I did. I hope it can help you! I had a labor & breach delivery that went very badly 29 years ago June 22, 1994. Pelvic ring injuries, sacrum, si joints, L4/L5/S1 , muscle tissue damage, ligaments, nerves etc…..no cure or remedy at all for me. I struggle around the clock every day and night of my life. A constant BATTLE just to get it to a tolerable level. I think about ending it every day. That would free me of the misery, agony for sure….but then I would be dead,…so what good would that be 🤷🏼‍♀️😪😪

I have Chronic pain for 5 years now. I don’t get a day off. It may go down from a 10+ to an 8, but that’s as close as I get to a day off. I am on treatment day 3 of 10 Calmare Therapy treatments. They don’t hurt. It actually feels good. It’s best to be off of Gabapentin because it does interfere with your results. I am reeling from a high dose to nothing. I am now at a 300mg a day from 2700mgs a day. Whew...don’t do it. I had gone off it twice before with no problems, so of course I thought this would be similar. Nope! Check out Calmare Therapy in Gold Canyon, AZ. Doc is very knowledgeable and if Calmare isn’t the right avenue, he has a lot of other”tricks up his sleeve” to help with pain. Great guy! ❤️❤️❤️

Chronic pain is much of a problem than we think. I sustained a fracture on my tibia and almost after a year of the incident, I still feel a gloomy pain down there and that keeps me from continuing my sport (football). I get a flash back of the incident whenever I’m in the pitch and feel a sensation of the pain. I feel like maybe the fracture isn’t healed yet and I was abt to re-injure it again. I go for X-ray scans from time to time and the doctors keep assuring me that it’s healed, there’s no need😅💔

I’ve had a chronic headache since 2012 or 2014 (I was okay for 8 months in 2014, but then it came back. It’s low on the pain scale, it’s around a four or five all the time) I was born with hydrocephalus and I’ve had surgeries related to that thinking it would help, (which it did for a bit) but ever since it came back I’ve been trying to figure out what’s going on and who I could go see about it.

that aproach in attitude towards a sprained wrist you described might be linked to the reason these people develop this symptom. a healing proces isnt something you get for free from your body. focus will help you heal. to put a bandage around it thinking it will be allright if i give it rest is such a wrong aproach. structure is damaged, it needs to heal and buildt new structure that will handle large stresses, so you need to gradually expose it to larger stresses.

hat girl he talks about obviously suffered some type of nerve damage when she sprained her wrist, and it takes a long time for a nerve to heal. I believe not treaing pain properly and those left for long periods of time in pain without relief that it can become chronic. One example of this is phantom pain syndrome, where an arm or leg is removed and the person still feels pain in the removed limb.

Treatment for pain: Whole Foods plant base diet, get 8-10 sleep, regular exercise, fix posture and under active muscles, diaphragm breathing, destress. Check for SIBO for underlying gut issues regardless of symptoms: you’ll see malabsorption, and inflammation. Also do a comprehensive stool test and organic acids test. Cut out wheat because it contributes to inflammation. Check for small fiber neuropathy. Look for the cause and treat the cause most importantly. Lifestyle changes are the most important to get your back life on track. No alcohol, process foods, meats, & dairy because they all cause inflammation and destroy the body.

For me the best state is to be very hydrated and eat mostly non-starchy vegetables and some fruits. Nuts are good but not too many. Sugar makes me ache more, so do heavy meat meals. Juicing and fasting helps so I try to throw in a few of those days when I'm not in places I have to eat with others. It is a battle between indulging myself with things that are tasty because I feel so bad, and doing what is best. Living with pain is a burden no one can know who doesn't experience it.
