The Ponseti Method; Casting and older baby

With the impact of COVID, some babies have had their treatment delayed, however successful Ponseti Management of babies over 3 months is possible.
The Ponseti Method requires a minimum of 2 trained practitioners. If your baby is older than 3 months, one practitioner should have experience in managing treatment for babies over that age.
Older babies might be more aware of the procedures and therefore will require to be comforted. It advisable to have a parent nearby and feed the baby during casting.
Casting older babies with more underlying adipose tissue may present more challenges and some adjustments may be required for successful molding and application of the cast. A key element is to ensure that the knee and upper leg are stabilised to enable the person undertaking manipulation and moulding to obtain a good fit.
Cast for older children may need to be reinforced as the baby might be more active and damage the cast. Some centres use fiberglass casts to reinforce above and over the knee, but it is perfectly acceptable to use ordinary POP. Casts can be kept lighter when doing so by using a plaster slab under the foot and over the knee rather than wrapping extra layers around the leg.
It is important to cut the cast really well above the toes. The toes should all be exposed, thus reducing the possibility of the baby using the big toe as a lever to push the cast down the leg.
Casts applied to older babies may be more likely to slip. The treating team should have procedures in place to respond quickly and effectively to cast slips.
An older baby in casts should be held in this particular position (insert photo of Denise holding the baby) which provides support under the casts, reducing the possibility of cast slippage.
If there is a lack of progress of correction after 5 casts and before 7 casts have been applied, your treatment team should seek advice from a high volume specialist Ponseti service.
Useful Advice:
Take a photograph of the cast and the skin around the edges of the cast before leaving the treatment room. It will help you to communicate with your treating team in case of issues with the cast.
It takes several hours for the cast to dry fully. During this time, please take extra care not to disturb the plaster in any way.
Check your child’s toes are of normal appearance and warm
Check that the toes are exposed and clearly visible at all times
Change your child’s nappy frequently to avoid soiling the plaster
Check the skin around the edge of the cast for any signs of redness or soreness.
It is important to contact your treating team if;
o You cannot see your child’s toes as this indicates that the plaster has slipped and will no longer be correcting the feet
o Your baby’s toes change in appearance and become cold
o The plaster becomes loose, cracked or crumbly.

after ponseti method the right foot is normal but left foot is still under disability, can left foot corrected without surgery? Now the baby is 3 months old.... Is there any solution for this?
