The scene at the start when Vivi bows her head down was an important lesson for Luffy, it showed him what it takes to be a good leader and not to just fight through every situation blindly. Vivi showed that even with her being royalty it didnt matter she had to lower herself and ask for help for the sake of her friend. You can see it on Luffys face when he looks at her and says you're amazing.

The doctor thing might seem random but Oda puts tons of real world politics in the story - you got a taste with the Fishmen and effects of racial oppression (more on that to come later). In Drum Island's case Oda is criticizing the Privatized Healthcare systems where only the rich can afford good doctors/health insurance and the poor get put into years of debt from a single ambulance ride.
