Little Carmine is clearly the one with all the power considering that Butchie brought him to the meeting in the first place plus he looks at Carmine twice before making a decision. Little Carmine also has the majority vote from all NY capos ever since Johnny Sack became a mad-king that murdered a female associate, and Angelo Garape, a made-man and formal consigliere to establish dominance. There has never been a New York War since the "Colombo-thing." It's Little Carmine that okays the hit on Tony as revenge for Rusty Millio. Either Patsy or Paulie flipped on Tony.
