Heel pain is a painful injury that commonly results in prolonged and severe disability. While heel pain is the subject of a growing scientific literature, the accumulation of studies is so fast that it is difficult for the busy physical therapist to keep up and engage in leading-edge evidence based practice. This course provides recommended interventions based on the clinical practice guidelines for heel pain/plantar fasciitis and applies the interventions through a patient case scenario. Specific practical approaches and pearls will be highlighted, in order to help the learner integrate best practice recommendations from the clinical practice guideline.

Do you do this after the person just finished an activity involving a lot of foot use like running etc? My condition is painful after any foot heavy activity no matter how often I do them and in a lost cases. After the activity and upon after sitting for a short while… pain is terrible.

So if either test provoked pain, would that rule in plantar fasciitis? What if the test was negative unloaded, but positive when loaded? And vice versa. Thank you for the video. If pain still persisted, and both tests were negative - what would the assumption be?

Great question! Here is the response from the instructor: "This is a good question. In the video, I demonstrated an unloaded windlass test. It was intended more as an aid to palpation of the plantar fascia, which would be the provocative maneuver. Based on the patient's level of symptom irritability, however, one might consider increasing load and intensity of the test. A way to do this, as the commentor rightly pointed out, would be to do the test in a weight-bearing position. Weight-bearing would increase foot pronation, which should pretension the plantar fascia. There are other ways to also preload the tissue. Another way I have found useful is to have a patient perform their activity to the point of symptoms, and then conduct the nonweightbearing test that was demonstrated. If the windlass test remains negative despite preloading the tissue in a manner consistent with patient's symptom irritability, one can surmise the plantar fascia may not be the culprit tissue." I’ve had no trouble with my plantar fascia until last week, I was doing a plank for as long as I could and had to stop 3 minutes in due to my foot experiencing quite a bit of discomfort. I walked this off and thought nothing of it. The fascia is noticeably tighter than the other foot when stretching, but I do not have any increased pain in the mornings or after periods of inactivity. Additionally, my plantar fascia is sore to press along the arch of my foot, not on the insertion point (which I hear is the most common point for pain). I have experienced many persistent injuries of late which have stopped me from playing football! Any thoughts? Thanks!
