Balalaika, the scary Russian lady, is indeed scared. She served as the leader of a black ops team in the Afghanistan when the Russians were invading. Was badly wounded and burned during a mission. Afterwards the soldiers serving under her ended working as disposable thugs for the Russian mafia. So she got out of retirement and called her solders to her. All of them are absolutely loyal to her. Also she is not actually Russian mafia. Russian mafia does not allow women in her ranks. She is more of a Russian warlord with connections to the mafia. It's just she is so scary that none of the heads of the Russian mafia are willing to try to move against her.

His name was Adrian Carton de Wiart and he was about as close to an OP anime character as a real human being can get. The dude won the Victoria Cross in WW1 - THE hardest medal it's possible to earn on the PLANET, fought in three different major wars across six decades, was SHOT IN THE EYE and BLOWN UP BY A GRENADE and kept going. And in his memoir/diary he wrote about WW1 - "Frankly, I rather enjoyed the war" .... WTF WHO SAYS THAT ABOUT A WORLD WAR?! Honestly, his story is SO nuts it's really hard to believe any of it actually happened. But, that kinda defines the winners of the Victoria Cross - especially in later years, since it's just got harder and harder to actually earn the thing. Each one of them could be their own anime protagonist.

So the thing about grenades. They're always shown as this big explosion making bomb almost, and there ARE some like that, but usually the shrapnel is more dangerous than the explosives in it. If what she's wearing is resistant to small arms, which is likely considering some later scenes, and she leapt out of the way as soon as the grenades' fell and the countdown started (Assuming they get armed right before dropping and not before), the spreading fire and concussive force would be more dangerous than the actual shrapnel. The other, less fun explanation is anime logic and rule of cool.
