He thought Head Truck, his head was taken off by a truck. Also kinda rhymes with drunk and skunk “he was drunk as a skunk” but the last part might be a stretch

The graves are very shallow. Probably because Schrutes don't have the greatest history of getting it right when declaring someone dead, so it just evolved for practical reasons to make it easier to dig someone up. Probably before the tradition of unleashing a round of buckshot into the coffin started.
