The dice rerolls all pedestals in the room, INCLUDING empty ones (apparently), which is what caused the glitch. The empty ones respawned new items after reroll.

I think the reason it spawns new items on empty pedestals is because the workaround they found to reroll items to any pool like chaos is simply replacing its pedestal with one that will generate a item from any pools, the thing is that they could just add a something in the code that checks if there is a item on the pedestal, but they didn't, hence the glitch occuring.

The color theme might be because the dice is based on a "Spindown D6" which would reroll and then spindown, which basically made it a chaos dice since it changed the item and changed pools. But I don't know for sure

An improvement to perfection smuggling i would make is to drop it at the start of every room instead of the start of the floor, that way it's easier to remember and a bit faster than going all the way back to the start when finishing the floor

You can Perfection smuggling + rock bottom, to keep it in case you get more luck up. You can also get use to drop it when you go againt a boss which you are afraid to get hit by (gideon). You can also get it with you and drop it at the start of the floor if you dont have any items based on luck.

i don't know if you'd read this, but a fun challenge with a mod that another isaac creator frost prime does might be fun for your stream! the mod is called "The Rules", and for whoever gifts 5 subs or donates a certain amount of money, theyre able to make a new rule for you which could help you (for the believers) or sabotage you (for the doubters), i thought it'd be interesting for you to try if you'd like! rules can be like "if you take damage, it triggers D100". as always ive loved your videos and your silly noises and excited to see what you do next ^__^
