Honestly it's tough. You end up selling stuff mostly and not finishing quests as much because the innocent things all become murder in the end. There are multiple endings and even in a nice play through I didn't get a great one.

Its true that he had nothing to win by no killing him, but he also had nothing to lose. So of course hes gonna spare him. Its just the most Kevin choice. Either you take out a murderous canibal in a position of power, in a totally corrupt city where theres literally no other way to stop him, or you left him go to continue his murder spree. I think its prety clear why he choose what he choose. There are no good deeds in this playthough, not even by accident.
