Look closely at the icons and names of the weapons, there are several that look the same at first glance but have little modifiers on them to indicate that they are a variant. You missed a salt rifle and a firework launcher in the second level.

Btw, some things may behave strangely or not work in co-op. Sgt Johnson also was supposed to give you a special kick move in the first level. There's also two other upgrades that have the same issue. As for d20s, they can give you stuff that may not be in a campaign level. That is if it doesn't kill you
