@@LollyMarie1oh yeah, this isn't talked about often enough: this element of self-therapy. Various psychedelic substances (according to a friend, of course) seem to help you "see through and beyond" your own mental barriers, false assumptions or defense mechanisms, and that can happen quite consciously, where you truly experience how, and why, you are getting in your own way, and can clearly see yourself dispensing with that mental construct. It's like a defense comes down, and you dream a more useful version of yourself. I'm no psychologist, but that sounds to me like how you defeat some types of depression at their core. So: it's not just a molecule that resets something in the brain plumbing. It supports a sort of spontaneous self awareness therapy.

Same! My GP has strongly recommended me getting ketamine infusions! It's not covered by insurance (yet), but I've heard that there's a week long one that can be covered if signed off on by a psychiatrist! There are also things like mindblook (the box they advertise where they send it to your home), but there's less advertised ones that seem to be way cheaper and more highly rated and recommended. My friend works in the medical field and got access to infusions when they were first approved. I've even thought about microdosing on psilocybin as I've heard that can be helpful for treatment resistant depression and help with ptsd and cptsd. As much as it sucks to deal with the symptoms of complex ptsd, I am grateful we are in a time where these drugs are being looked at as options for addressing mental health. 70 years ago we might have just been lobotomized! Hope you find healing and happiness ❤ Edit: I am currently trying to see if I can get approved, if not for the week long ketamine treatment, to get my therapist to be allowed to be my "trip sitter" to help guide me through a mail-order treatment process. Dunno if it will go through or be approved, but there's so many possibilities! Stay hopeful!

It's a huge deal. I don't want to rain on anybody's parade, BUT it's still pretty questionable whether MDMA will end up receiving approval. Just a month ago, the FDA committee evaluating MDMA as a PTSD treatment voted against it and there have been concerns about the validity of the research used to support its approval. The committee's vote doesn't directly decide whether the treatment gets approved, but it can impact the FDA's decision. They have certainly approved certain treatments, even when the committee votes against it. Guess we'll find out August 11th...
@Maquis19436 ай бұрын
@@neurotransmissions I was in a MAPS phase 2 clinical trial for MDMA-Assisted Psycotherapy for PTSD. The validity of the data is incredibly strong, much stronger than the data for SSRIs, for instance. The effect size is huge with over 68% of clinical trial participants *no longer meeting criteria for PTSD*, with the effect size only growing over time. There has been a concerted and deeply dishonest campaign by a small disgruntled group of former psychedelic proponents who are using fear tactics to undermine the study. Fear gets eyeballs on twitter, propagates an "us vs them" mentality, and in an age of vaccine denial, science skepticism, and Q-Anon-like conspiracism, poisons the well. The FDA Advisory Committee vote, while purported to be based solely on science, was affected by this negative social media campaign. In addition, their concerns were fully addressed in the sponsoring agency, Lykos's, proposal, including things like the difficulty in blinding. It's important to understand that the FDA has worked *literally hand-in-hand* with the sponsoring organization to construct the clinical trial, addressing all of the points that the advisory committee raised. Pharmaceutical companies have gone into these meeting with *drastically* less data supporting their drug application and have gotten through. Lastly, note that this is an Advisory Committee-not the actual FDA-and that one out of three applications that have been approved by the FDA have NOT been approved by the advisory committee, so there is 33% chance that MDMA-assisted psychotherapy could still be approved in August. For those concerned, and waiting for this potentially revolutionary treatment modality to be made available, even if the FDA does not approve in August, all the data is there, save for some physical data not taken during the clinical trials. This means that there may need to be an additional study done on physcial effects of MDMA dosing *more than* the three dosings done in phase 3 trials. This modality will not be stopped. The data is very strong. The disgruntled group of naysayers cannot change what we 420-odd participants in the clinical trials know to be true: that this treatment DOES have an effect on PTSD. With guardrails that satisfy the medical community, this modality WILL be approved.
@Maquis19436 ай бұрын
@@neurotransmissions Hmmm. Why was do my comments *keep* getting removed? It's pretty disconcerting. I think being an MDMA-assisted psychotherapy for PTSD clinical trial participant with direct knowledge of the FDA approval process and the Advisory Committee meeting, my experience might offer useful information to your viewers (especially the ones who are struggling with PTSD and are concerned about the possibility of recieving treatment.)
@Maquis19436 ай бұрын
Don't give up hope. I am a Phase 2 MDMA-Assisted Psychotherapy for PTSD clinical trial participant. The data from the sponsoring organization, Lykos is incredibly strong. A small group of disgruntled and slighted former true believers have poisoned the well with conspiracy theories, manipulation of information, hearsay, and a deep misunderstanding of the research, clinical trial, and FDA-approval process. None of them are clinicians or scientists and have a political axe to grind. The FDA has worked hand in hand with Lykos over the past five years to ensure that the clinical trial was done to the FDAs high standards. The Advisory Committed meeting's decision, as stated in the comment below, is not binding. 1/3 of all new drug applicants are rejected and still get approved by the FDA. And even if there is more data that needs to be collected (e.g. physical data like heart rate, EEGs, liver and kidney function tests, etc), this is easily done with a likely limited clinical trial. This treatment can't be stopped-the 425 or so clinical trial participants who either *no longer meet criteria* for PTSD or whose PTSD symptoms are markedly reduced, are here, surviving, and will not allow a small group of naysayers to manipulate our experiences to advance their careers or further create division. See you on the other side!

I signed up for a series of 3 -6 session programs and in less than a year unpacked a lifetime of avoidant adaptations. When I finally stopped the withdrawal was worse than anything I've ever been through, including kicking heroin and fentanyl. I actually used alcohol to help taper as alcohol also has (admittedly mild) nmda antagonism too. A lot of the WD I experienced from k seemed to mimic the symptoms of alcohol induced DTs too, including psychosis and hallucinations (in addition to absolutely insane levels of pain. 8:30 it's not so much about glutamate system dysfunction, but rather that glutamate signalling influences BDNF release. Blockage of NMDA ligand gated ion channels via Ketamine leads to rapid BDNF realease. This is turn promotes the formation of new synaptic connections (neuroplasticity and ability to adapt/learn/change thought patterns) and has anti-inflammatory and anti-apoptotic effects. Other things proven to improve depression, such as TCAs, SSRI and SNRI also lead to increased BDNF levels (with TCAs being the most potent), as do psychotherapy, doing sports, environmental enrichment and serotoninergic psychedelics (i.e. psilocybin, LSD - interestingly enough they also bind to 5-HT2A-mGlur2 and 5-HT2A-D2 receptor heteromers. many other receptors can fuse with one another aswell. This opens up a whole new field of study and implicates a vast amount of yet unknown interactions and mechanisms). Anyhow, BDNF is where most of the different depression models converge, since marks the point of intersection between them.

Dissociated for 3 weeks I was either asleep or feeling like I was coming out of general anesthesia. It was NOT pleasant and I cannot imagine people enjoying this. All that being said, everyone is different and if you aren’t responding to other treatment you should give it a try. A lot of people are improving. Disassociating is pretty fun and i think a big aspect of why its so effective, though. I think of it sort of like a sliding scale. The more you take the further away from yourself and your worldly issues you go. A little bit will make you analyze your issues in a more objective and less biased (by mental struggles) light which helps you contextualize your problems properly. Take a lot and you end up in the shadow realm thinking you are thinking like god does. You know the usual

I always find it interesting that they want two other types of antidepressants to fail first before trying the one that works when those ones don't. Those ones can lead to suicidal thoughts, Spravato can relieve suicidal thoughts. Hmmm... Let's try the one that causes suicidal thoughts first... twice. I know someone who was heavily depressed about 11 years ago. Zoloft had done nothing in the past, cymbalta gave them daytime hallucinations, and then Wellbutrin almost led to them ending their life. I didn't find out that they we're having suicidal thoughts until a year later. Maybe start with the safer one first (I know it's not without risk) and if that one doesn't work, then try the one that can cause suicidal thoughts.
@eray99344 ай бұрын
I think just about everything can potentially cause suicidal thoughts, my guess is ketamine can too.
@LadyPantera574 ай бұрын
@@eray9934I don't think it works in the same way that certain antidepressants do. I know someone who had never had suicidal thoughts, no matter how depressed they had been in the past, and then they took an antidepressant to try to get better and all of a sudden they started planning to end their life. Considering what Dr Tracy Marks said in the video, the way that ketamine works seems to be completely different from to traditional antidepressants and therefore might not have the ability to trigger suicidal thoughts in the way that traditional antidepressants can. There may be some people however who don't see a reduction in their suicidal ideation, there isn't much in this world that works for absolutely everyone. I just hope that this medication becomes more accessible for those who really need it.

I've threaded my own Depression with Ketamine, this was dangerous, but I had enough trust in myself. It helped immensely. One thing that I rarely see discussed that Ketamine helps overcome thoughtpatterns. You can think about a topic in a new way, and after the trip keep the insight... this amazing in dealing with difficult topics where your mind seems stuck, where you're unable to come to different conclusion, or approach a topic from different perspective. Having said this, I also had issues with addiction, where in very stressful weeks it helped me calm down and relax. But also was okay to get out, by not having any at home and relaxing my environment. I would not compare it with Opiods, where you're body is craving for it. Also these habits only developed after 5 years of taking it the first time. It's very unlikely to get hooked on it after a few uses. I actually helps a lot with other addictions. From seeing the good and the bad, I would highly recommend trying it, in a safe environment, and being smart about it.

@@pidgepagonis I think I misspoke a bit. I meant your ability to use your muscles, like your coordination and reflexes. With esketamine, or normal racemic ketamine (mix of both arketamine and esketamine, which is most common), you tend to lose coordination and your reflexes are highly delayed, so on medium, or even low, doses, you will find yourself stumbling and tripping on stuff. With arketmine by itself, you remain way more coordinated, and can walk to the bathroom without stumbling, or catch yourself more effectively when you trip. Although, if the dose gets high enough to put you in a k-hole, you still end up immobilized and falling to the floor with arketamine, but before that point, you can basically physically function like a normal human, instead of your coordination being comparable to being drunk like esketamine causes.

@DatflyincatАй бұрын
Ketamine’s dissociative effects alone make it a recreational substance, and while it can cause hallucinatory states, they’re not like those from classical psychedelics. For instance, diphenhydramine has hallucinogenic properties, but it’s classified as a deliriant because the effects are different. I think “dissociative anesthetic” is just a more accurate term. I’ve used ketamine and other dissos, and while dissociation can include hallucinations, it’s not the same as the effects of traditional psychedelics. So while I get your point, I still think dissociative anesthetic is the better classification, Plus she literally used the word psychedelic, not hallucinogenic, dissociative properties innately induced hallucinations, but ket vs a real psyc is a huge difference, So it's hard to call it a psychedelic.

So are you saying that the additional effects on opioid receptors and HCN receptors is due to ketamine directly binding to it also? Or just downstream effects of the NMDA antagonism?

I wonder if clinically prescribed ketamine has less side effects on the bladder than recreational/street ketamine? I've considered hopping on ketamine therapy, as I loved the antidepressant effects I experienced from recreational K, but I noticed unpleasant effects when urinating after just a handfull of small doses. Maybe sublingual dosing would have a different effect on its processing than through nasal insufflation?

unless you take a gram a day you shouldn’t feel bladder issues but some do, is it like hard to urinate for a few hours after or? because that’s normal side effect wise
