It's not the first time he finds an item of a mod and chooses to make it dedicated. Again now instead of it being in the halfway of another mod, he tries it out from the start. I think the sound of the arcade opening repeatedly on Depths 2 was because of Mercurius. Since it always opens every door in the room even if it's uncleared, it was probably opening the doors in the already-cleared room, which included the arcade. Also, Mattman wanting to play a crane game and blowing up the Blood Donation Machine instead of hitting it when he has Wafer is a Mattman Moment™.

if i may recommend an item to try out: Entropy, an item from "some alphabirth items" basically, whenever you shoot a tear, there's a high chance another tear fires (at a random trajectory), also giving a -2 fire delay (fire rate up which goes past cap, i think it's a multiplier tbh) it works for any tear that you fire, including familiars and the like Edit; apparently the page was taken down, big sad

Hello Matt, I don't know if you have already tried this mod but I would like to recommend it to you anyway, the mod is called "Improved & Reworked Foes" and it basically changes the mechanics of certain bosses and enemies in the game (it makes it more complicated in a few words). Also if you try it I recommend going to the chest since the bosses are better, greetings

About the Mom's Knife + Brimstone synergy, I feel like it should just be the knife storm without the regular knife like you said, but instead it keeps the original Brimstone laser underneath the knife storm, and releases both at once. That just feels like it'd be more satisfying and could lead to some other cool synergies
